Dynamic axial carpal instability: a case report.

The concept of distal carpal row dissociation is not new. It has occurred secondary to high-energy crush or blast injuries, with resultant axial dislocations of both carpal rows and metacarpals. Axial carpal sprains without disruption or dynamic axial carpal instability have not been previously described. The evaluation of this new type of carpal instability with radiographs, tomography, arthrography, and magnetic resonance imaging failed to demonstrate its etiology. Arthroscopic evaluation of the midcarpal and radiocarpal joints demonstrated a dynamic axial carpal instability with incompetenc

An analytical model of lumbar motion segment in flexion.

OBJECTIVE: To develop an analytical model of the lumbar motion segment and to determine the following under the application of flexion physiological loads: (1) the force displacement relationships of the lumbar motion segment; (2) the forces in the ligaments, disc, and facet joints; (3) the strains in the ligaments; and (4) the effect of the transection of the ligaments. DESIGN: Computer modeling. SETTING: Spinal Ergonomics and Joint Research Laboratory at The National College of Chiropractic. PROCEDURE: Computer model simulation of external loads and simulation of ligament transection. ME

Peroneal subluxation: a comprehensive review of the literature with case presentations.

Subluxation of the peroneal tendons is a rare injury that is often misdiagnosed. This injury is usually related to and often accompanied by lateral ankle ligament sprains. A comprehensive review of the literature, as well as two cases of surgically treated subluxing peroneal tendons will be presented. Anatomy, etiology, and diagnosis will be discussed. Both conservative and surgical treatment methodology will be discussed in detail.

Sensory changes associated with severe ankle sprain.

An intact afferent nervous system is important in providing the feedback necessary for effective motor control. Joint injury may influence afferent feedback and, if the lower limb is involved, lead to a decrease in stability. Accordingly, the association between severe ankle sprain and local sensory deficit was examined. Measurements of vibration perception, two point discrimination and balance in one legged standing were made in subjects who had sustained a previous severe unilateral ankle sprain and in subjects with no history of lower limb injury. Comparisons between the two groups showed t

Ligamentous injuries of the wrist.

Wrist injuries occur commonly. Significant wrist injuries such as perilunate dislocation and scapholunate dissociation may occur without carpal bone fracture. The emergency physician can recognize these ligamentous wrist injuries by noting abnormalities of the shapes, joint spaces, and alignments of the carpal bones. Early diagnosis allows for prompt referral and optimal outcome.

Management of acute and chronic lateral ligament injuries of the ankle.

Acute and chronic lateral ligament injuries of the ankle represent a common but persistent problem to orthopedic specialists. A comprehensive physical examination and classification system were proposed as an approach to this problem. Guidelines for both conservative and surgical management for acute and chronic injuries are described. A comparison of the most common lateral ligament repairs is provided, with emphasis on the benefits of the so-called anatomic repair.

MR imaging of the most commonly injured ankle ligaments. Part II. Ligament injuries.

Thirteen patients with clinically diagnosed sprained ankles underwent magnetic resonance (MR) imaging. Five of these cases are presented to illustrate the potential of MR imaging to enable identification of both primary and associated ligament injury sites, grading of the severity of the injuries, and visualization of the associated findings of tendon sheath and joint effusion. The appropriate combination of foot position and imaging plane is essential to achieve full-length visualization of each ligament. Two patients demonstrated findings compatible with total gross disruption of the anterio

Elastic bandages and intermittent pneumatic compression for treatment of acute ankle sprains.

The efficacy of elastic bandage alone and with intermittent pneumatic compression (IPC) treatments in the rehabilitation of 44 acute ankle sprains was studied. Lower-leg dysfunction was assessed by measurements of edema, degree of ankle motion, pain, and limb dysfunction when the patient was first included in the study, after treatment for one week, and after a four-week follow-up. For all the parameters studied, elastic bandage with IPC treatment resulted in highly significantly (p less than 0.001) faster rehabilitation during the four-week follow-up than did elastic bandage treatment alone.

Acute lateral ankle ligament injuries: a literature review.

The average general orthopaedic surgeon examines and treats a considerable number of acute lateral ankle ligament sprains in a busy office practice. A cursory review of recent articles published on this subject will present a confusing picture regarding diagnosis and treatment of Grade III injuries. An air of controversy surrounds the interpretation of diagnostic x-rays, and the management of these common ankle problems. This review article is presented to summarize current thoughts on the anatomy, biomechanics, diagnosis, and treatment of acute lateral ankle ligament sprains.

Medial gastrocnemius hematoma mimicking deep vein thrombosis: report of a case.

Tennis leg, strain of the medial head of the gastrocnemius muscle, may follow a trivial trauma in athletic and nonathletic activities. Complete or partial tears of the musculotendinous unit may result in a hematoma. Clinical manifestations of gastrocnemius hematoma in a 64-year-old man with local swelling, pain and tenderness that was aggravated by passive dorsiflexion of the ankle joint, mimicked deep vein thrombosis. Failure to appreciate the precedent trivial trauma and the localized swelling and ecchymosis led to delayed diagnosis and inappropriate heparinization. The definite diagnosis wa

[Sprains and dislocations of the wrist joint and carpal bones].

In case of persistent swelling and painful limitation of mobility, "distorsion" can be accepted as the definitive diagnosis, only if osseous and ligamentous injuries of the wrist and carpus have been ruled out with sufficient certainty. According to current knowledge multiform injuries of the carpus originate from a three dimensional impact of forces, which besides the well-known hyperextension and ulnar deviation of the wrist include intercarpal supination as a rotatory component. Depending on the effective forces and due to the biomechanical strength of the restraining ligaments (which chang

Kinesthetic awareness in subjects with multiple ankle sprains.

The purposes of this study were 1) to determine whether decreased kinesthetic awareness occurs in individuals with recurrent ankle sprains and 2) to determine whether the one-legged standing balance test can be used to differentiate between ankle instability in injured and uninjured ankles. Thirty athletes between 18 and 24 years of age with multiple sprains of one ankle and no reported sprains of the other ankle were tested to compare their ability to detect passive plantar flexion and standing balance in each ankle. Luce's choice theory was used to analyze subjects' responses. Subjects had s

Arthroscopic surgery of the ankle.

Ankle arthroscopy is a useful technique, especially for intra-articular problems and osteocartilagenous lesions. Indications for this diagnostic and therapeutic technique include: ankle sprain unresponsive to the usual course of treatment, complaints of intermittent locking, pain and clicking, limitation of motion, and painful conditions with no obvious etiology (especially in a worker's compensation beneficiary). A review of arthroscopic, radiographic, and clinical data of all patients undergoing ankle arthroscopy at our center provided the following diagnoses: talar dome osteochondral fractu

Rehabilitation of the ankle.

The ankle is the most frequently injured major joint, and ankle sprain is the most frequent sports injury. If the injured ankle is managed correctly from the beginning, pain and swelling and the resultant disability will be kept to a minimum. Early return to participation predisposes the athlete to reinjury. Not only must strength and flexibility be restored, but the athlete must also be pain free and have regained his or her proprioceptive abilities.
