We report a patient who developed mesenteroaxial gastric volvulus after a liver transplantation. We hypothesize that this complication may have been related to the ligation of the hepatogastric ligament done to mobilize the liver during hepatectomy.

A 26-day-old male neonate presented with an acute gastric volvulus which was reduced with a nasogastric catheter. The patient was then subjected to laparoscopic anterior gastropexy. Six months after the procedure, the child is thriving and completely free of symptoms. We present this case as the first wherein a laparoscopic gastropexy has been done for gastric volvulus in a neonate. We have found the procedure acceptable for the treatment of neonates with gastric volvulus.

Traumatic diaphragmatic hernia is rare, but is of utmost importance due to its high morbidity and mortality. It is markedly important in patients with blunt abdominal trauma, and diagnosis is difficult because of the numerous associated injuries. A patient with few symptoms of chronic traumatic diaphragmatic hernia is described, who underwent surgery due to a gastric volvulus. Laparoscopic surgery permits repair of these injuries through an abdominal approach, avoiding a thoracic incision or selective intubation.

The authors present a case of a diaphragmatic hernia with gastric volvulus, 29 months after a left thoracophrenolaparotomy. The surgical repair was performed by laparoscopy. The authors discuss the laparoscopic approach in the treatment of diaphragmatic disorders.

We report two cases of gastric volvulus. A discussion of the epidemiology, pathophysiology, clinical presentation, diagnosis, and treatment of this rare entity is presented.

The traditional surgical treatment of gastric volvulus involves upper abdominal laparotomy with gastric detorsion, fixation, and, when present, repair of associated diaphragmatic hernia. We describe a case of organoaxial gastric volvulus associated with a paraesophageal hernia in a poor risk patient, which was successfully treated with laparoscopic detorsion and percutaneous endoscopic gastropexy. This approach avoided the morbidity of a laparotomy and allowed the rapid recovery of gastric function.

Gastric volvulus is a rare condition in childhood. Most of the reported cases have been acute and secondary in type with predisposing factors. Between 1965 and 1988, 49 patients were treated at this institution for chronic idiopathic gastric volvulus. Patients were divided further into two groups according to age at admission. The main symptom was vomiting in the neonatal and infantile group, whereas it was abdominal distension, vomiting, weight loss, or constipation in the older age group. This clinical feature was different from that of acute gastric volvulus in which the symptoms resulted f

Presenting with severe stomach pain from sigmoid volvulus, a woman in her 90s was effectively treated with colonoscopic desufflation. Interestingly, serial imaging over 16 years demonstrated gradual cranial displacement of the right kidney to the hepato-diaphragmatic space without prior trauma, surgery or congenital abnormalities. The patient remained asymptomatic with respect to renal function despite significant anatomical displacement. No specific intervention was required for the ectopic kidney. It is important to recognise the potential for organ migration associated with ageing, signific

An otherwise healthy 17-year-old boy presented to the paediatric emergency department with acute severe epigastric pain. An admission abdominal radiograph demonstrated gastric dilation, associated with an elevated left hemidiaphragm. Subsequent barium contrast imaging confirmed the diagnosis of organoaxial acute gastric volvulus (AGV). Emergent exploratory laparoscopy revealed AGV with migration of the stomach, spleen, pancreatic tail, splenic flexure, left kidney and adrenal through a left-sided Bochdalek diaphragmatic hernia. Following careful mobilisation of the displaced structures, a mesh

BACKGROUND: Nasogastric tube-feeding often is necessary in the treatment of chronically ill patients. The disadvantage (irritation of the hypopharynx, dislocation, stigma) can be avoided by the percutaneous endoscopic gastrostomy (PEG). In childhood there is only limited experience with this techniques.
PATIENTS AND METHOD: We report about 89 children with PEG. Indications were central dysphagia (67), dystrophy caused by chronic renal failure or congenital heart disease (15), application of special diets (6) and gastric volvulus (1). The endoscopy was done in sedation.
RESULTS: The PEG was p

Hiatal hernias occur when intra-abdominal contents protrude into the diaphragmatic opening. Of the four classifications, Type 4 hiatal hernias are the most rare and severe. They develop from herniation of the gastroesophageal junction and abdominal viscera other than the stomach into the thoracic cavity. The resulting increase in intrathoracic pressure can cause a wide variety of symptoms on presentation and potentially lead to misdiagnosis. We present a rare case in which a 78-year-old woman presented with nonspecific symptoms and was diagnosed with incarcerated Type 4 hiatal hernia with sigm

A 56-year-old woman presented to the accident and emergency department with peritonitis 2 days after a routine oesophagogastroduodenoscopy. She was taken to theatre with the finding of gastric necrosis. Blood and peritoneal cultures grew group A haemolytic Streptococcus. Histology revealed normal vasculature, no volvulus but marked neutrophilia in the submucosa with an intact mucosa. The stomach was resected and the patient recovered in the intensive care unit but overwhelming acidosis progressed to multiorgan failure and treatment was eventually withdrawn. Acute phlegmonous gastritis has been

During 15 years (from 1959 to 1973) the author observed 753 patients operated upon for acute intestinal ileus; 161 of them aged above 60. An early diagnosis of the condition is difficult due to blurred main symptoms against the background of pronounced age changes and concomitant diseases. The most common non-blastomatous forms are the following: adhesive intestinal ileus (16.8%) and sigmoid volvulus (14.3%). Cancer of the colon with acute symptoms of intestinal ileus was noted in 23.6%. Stomach resection was carried out upon 37.9% of cases. Postoperative complications constituted 34.1% and le

Gastric dilatation-volvulus (GDV) was diagnosed in four adult guinea pigs with histories of lethargy and progressive hyporexia. Clinical examination varied between cases and was not specific for gastrointestinal disease. CT revealed a severely gas-dilated and malpositioned stomach in all cases. In three guinea pigs, a "whirl sign" was centered around the esophagus. Whole body CT facilitated the detection of significant concurrent pathology such as a splenic mass, obstructive ureterolithiasis, and aspiration pneumonia, leading to euthanasia in two cases. One guinea pig was successfully managed

Bochdalek hernias in adulthood are rare. Symptomatic Bochdalek hernias in adults are rarer, but may lead to fatal complications. Patients with acute gastric volvulus on diaphragmatic hernia are a diagnostic and therapeutic emergency. Here, we report a case of a 56-year-old woman diagnosed with epigastric pain, cough, vomiting since 2 weeks and shortness of breath. Complicated Bochdalek hernia was an incidental finding, diagnosed by chest radiograph, computed tomography (CT), and barium swallow study. Stomach was within the thorax in the left side due to left diaphragmatic hernia of a nontraumat
