Synovitis, Acne, Pustulosis, Hyperostosis, Osteitis (SAPHO) Syndrome: Clinical and Therapeutic Aspects.

Synovitis, acne, pustulosis, hyperostosis, osteitis (SAPHO) syndrome is a rare entity. It is frequently under-detected. We report the case of SAPHO syndrome in a 38-year-old woman, seen in consultation for pain and swelling of the anterior chest wall affecting the sternoclavicular and sternocostal joints predominantly on the right, and low back pain with an inflammatory appearance with peripheral damage, especially in the legs. We also found in our patient episodes of palmoplantar pustulosis. The diagnosis of SAPHO syndrome was retained in accordance with Kahn's diagnostic criteria, and the os…

Complications After Total Knee Arthroplasty in Patients With Pigmented Villonodular Synovitis.

BACKGROUND: Pigmented villonodular synovitis (PVNS) is a locally destructive histiocytic proliferation most commonly occurring in the knee. Extensive local joint destruction can indicate the need for a total knee arthroplasty (TKA). The objective of this study is to evaluate PVNS of the knee as a risk factor for complication after TKA. METHODS: Patients who underwent TKA with a diagnosis of PVNS of the knee from 2007 to 2016 were identified in a national private payer insurance database. Complication rates for emergency room visits, readmission, revision, stiffness, infection, and death were …

Failed Radiation Synovectomy in Diseased Knee Joint with Missed Tuberculous Synovitis.

The authors in this case report highlight the poor outcome of radiation synovectomy (RSV) for repeated knee joint effusion in a patient with histopathologically proven nonspecific arthritis. There was partial response initially following RSV but later follow-up showed recurrence in joint effusion with limited and painful mobility of the knee joint. Subsequent surgical synovectomy and histopathological examination showed it to be tubercular in origin. Thus in a country endemic for tuberculosis, an alternative infective etiology should also be kept in mind before subjecting the patient to RSV.

Computed Tomography and Magnetic Resonance Manifestations of Spinal Pigmented Villonodular Synovitis.

OBJECTIVE: To study computed tomography (CT) and magnetic resonance imaging (MRI) findings of spinal pigmented villonodular synovitis (PVNS), with the aim of improving diagnostic accuracy. METHODS: Findings from CT (n = 7) and MRI (n = 5) of 7 cases of spinal PVNS, diagnosed by pathological examination, were retrospectively analyzed. RESULTS: One case presented with a small lesion, whereas the remaining 6 cases showed lobulated soft tissue masses centering on the facet joints, with lytic expansive destruction of the bone. The CT density of lesions in 3 cases was similar to that of the surrou…

68Ga-DOTA-Siglec-9--a new imaging tool to detect synovitis.

INTRODUCTION: Vascular adhesion protein-1 (VAP-1) is an adhesion molecule, which upon inflammation is rapidly translocated from intracellular sources to the endothelial cell surface. We have recently discovered that sialic acid- binding immunoglobulin-like lectin 9 (Siglec-9) is a leukocyte ligand of VAP-1 and that 68Ga-labeled Siglec-9 motif peptide facilitates in vivo imaging of inflammation. This study evaluated the feasibility of 68Ga-DOTA-Siglec-9 positron emission tomography (PET) for the assessment of synovitis. METHODS: Rabbits with synovial inflammation were injected with 18F-FDG or …

[Bone transplantation with total hip endoprosthesis for diffuse pigmented villonodular synovitis].

Pigmented villonodular synovitis (PVNS) is a rare benign but aggressive disease of the synovium. If the hip is involved early destruction of the joint is common due to the tight structure of the capsule and arthroplasty is unavoidable in these cases. We implanted a cemented total hip replacement in a 17-year-old female patient who had histologically confirmed PVNS. Because of massive bony destruction in the acetabulum a reconstruction with homologous bone (two femoral heads) from the bone bank was necessary. After 5 years the bone transplant had become integrated, there were no signs of recurr…

Simultaneous anterior and posterior synovectomies for treating diffuse pigmented villonodular synovitis.

BACKGROUND: The current treatment methods for diffuse intraarticular or extraarticular type pigmented villonodular synovitis (PVNS) include arthroscopic synovectomy or staged anterior and posterior open synovectomies. However, it is unclear whether simultaneous anterior and posterior synovectomies achieve local control and recovery of function. QUESTIONS/PURPOSES: We therefore determined the recurrence rate and function in patients with diffuse PVNS treated with anterior and posterior synovectomies and adjuvant radiotherapy. METHODS: We retrospectively reviewed all 19 patients with diffuse P…

Ultrasound in rheumatoid arthritis: volar versus dorsal synovitis evaluation and scoring.

BACKGROUND: Assessment of synovitis in rheumatoid arthritis (RA) is a major issue for a proper treatment administration; it has been proven that ultrasound (US) examination could be of valuable help and it is currently being investigated as a possible outcome measure for the disease. It is, though, of greatest importance to accurately establish the place of US scores among the already validated outcome measures, according to Outcome Measures for Rheumatoid Arthritis in Clinical Trials (OMERACT) filter. The present study is designed to compare the results of gray-scale ultrasound (GSUS) and Pow…

Arthroscopic management of intra-articular pigmented villonodular synovitis of temporomandibular joint.

This retrospective study reports on four patients with intra-articular pigmented villonodular synovitis (PVNS) of the temporomandibular joint (TMJ) who were managed with arthroscopy between 2002 and 2009. There were three females and one male, with a mean age of 46 years at diagnosis. The common symptoms were trismus and pain. No pre-auricular swelling or mass was detected. Magnetic resonance imaging (MRI) and arthrography showed an anteriorly displaced disc, disc perforation, osteophyte of the condyle, or increased joint effusion. No neoplasm was suspected radiologically. Under arthroscopy, a…

New insights into synovitis, acne, pustulosis, hyperostosis, and osteitis (SAPHO) syndrome.

In 1987, synovitis, acne, pustulosis, hyperostosis and osteitis (SAPHO) syndrome was proposed as an umbrella term for a group of diseases with similar musculoskeletal manifestations, in particular hyperostosis of anterior chest wall, synovitis, and multifocal aseptic osteomyelitis, observed in association with dermatologic conditions such as palmoplantar pustulosis, severe acne, and hidradenitis suppurativa. Despite recent advances in our understanding of the epidemiologic, pathophysiologic, and immunogenetic mechanisms involved in SAPHO syndrome, etiopathogenesis remains poorly understood. Pr…

Cementless total hip arthroplasty performed in patients with pigmented villonodular synovitis.

The authors retrospectively reviewed all patients with pigmented villonodular synovitis (PVNS) of the hip treated by cementless total hip arthroplasty (THA) combined with synovectomy. Eight patients were reviewed and observed for an average of 8.9 years (range, 4.3-13.5 years). Mean preoperative Harris hip scores improved from 49.3 to 96.6 points, and all patients were able to regularly perform moderate daily living activities. None of the patients had clinical or radiographic evidence of recurrent PVNS. Osteolysis occurred in 4 hips, and 2 revision surgeries were performed during follow-up. C…

Pigmented villonodular synovitis of second toe interphalangeal joint: Methods of imaging.

Pigmented villonodular synovitis is a rare proliferative disorder of the synovium of unknown aetiology. We present a case of pigmented villonodular synovitis in an unusual location, the interphalangeal joint of a toe, diagnosed by ultrasound with colour Doppler. The role of various imaging modalities in the diagnosis of pigmented villonodular synovitis is discussed.

Remitting seronegative symmetrical synovitis with pitting edema following acute intracranial hemorrhage.

Remitting seronegative symmetrical synovitis with pitting edema (RS3PE) is an unusual inflammatory arthritis with unknown pathogenic mechanism, and is characterized by an acute onset of symmetrical synovitis with pitting edema of the hands or feet. Numerous diseases are associated with RS3PE, including neoplasia, infection, Parkinson's disease, and other rheumatic diseases. Neoplasia is the most common condition associated with RS3PE. We report a case of RS3PE that occurred following acute intracranial hemorrhage in an 80-year-old man with a 20-year history of hypertension.

Pathogenesis of haemophilic synovitis: experimental studies on blood-induced joint damage.

Hemarthrosis is a common manifestation of haemophilia, and joint arthropathy remains a frequent complication. Even though the exact mechanisms related to blood-induced joint disease have not yet been fully elucidated, it is likely that iron deposition in the synovium induces an inflammatory response that causes not only immune system activation but also stimulates angiogenesis. This process ultimately results in cartilage and bone destruction. Investigating the processes that occur in the early stages of blood-induced joint disease in humans has been very limited. Therefore, the use of haemoph

Pigmented villonodular synovitis causing painless chronic knee swelling in an adolescent.

It is uncommon for an adolescent to present with a painless chronic knee effusion without a history of trauma. Acute knee effusions are usually caused by internal derangement, patellar dislocations, or fractures. The differential diagnosis of a chronic knee effusion must include the more uncommon diagnoses of juvenile rheumatoid arthritis, septic arthritis, reactive arthritis, synovial osteochondromatosis, synovial hemangioma, synovial sarcoma, or pigmented villonodular synovitis. Radiographic imaging, plain radiographs, and magnetic resonance imaging in addition to a biopsy specimen are most …

Remitting seronegative symmetrical synovitis with pitting edema syndrome: followup for neoplasia.

OBJECTIVE: To investigate whether the remitting seronegative symmetrical synovitis with pitting edema (RS3PE) syndrome may represent a paraneoplastic disorder in a significant percentage of cases. METHODS: Patients diagnosed with RS3PE syndrome at the Medical College of Wisconsin before 1995 were telephoned and asked about their rheumatologic course since initial diagnosis of RS3PE and whether they had been diagnosed with any cancer. If so, permission was obtained to review tissue pathology. Criteria used for diagnosis of RS3PE syndrome included sudden onset of painful diffuse swelling of bot…

A unique presentation of juvenile rheumatoid arthritis: proximal interphalangeal joint synovitis.

Juvenile rheumatoid arthritis seldom involves the small joints of the hand. We are reporting three patients with proximal interphalangeal joint synovitis as the initial manifestation of juvenile rheumatoid arthritis. This form of presentation of the disease should be recognized so that treatment will not be unduly delayed especially for the ophthalmologic manifestations of juvenile rheumatoid arthritis.
