We present the case of a patient with bilateral distal humerus osteonecrosis following chemoradiation for lymphoma. Treatment consisted of a right total elbow arthroplasty with a distal humerus megaprosthesis and a left custom-made distal humerus hemiarthroplasty, replacing exclusively the humeral articular cartilage while preserving the native medial and lateral humeral columns. By using a custom implant of this design, we were able to avoid the weight-bearing and lifting restrictions to which patients have to adhere following a typical total elbow arthroplasty. Thus, we were able to preserve

INTRODUCTION: The flexible spastic varus foot in cerebral palsy is commonly corrected by split-tendon transfer of tibialis anterior or tibialis posterior. These tendon transfers are said to preserve hindfoot motion, which is until now not been proven. Therefore, the aim of the study was to show the hindfoot motion following split-tendon transfer in comparison to a midtarsal arthrodesis.
MATERIALS AND METHODS: A retrospective study was done on patients with flexible spastic varus foot in cerebral palsy who underwent a combined split-tendon transfer of tibialis anterior and posterior. Patients …

In patients with severe thenar atrophy secondary to carpal tunnel syndrome, we hypothesize that following open carpal tunnel release, concomitant transfer of the abductor pollicis brevis (APB) origin to the flexor carpi radialis (FCR) tendon will lead to improved patient function restoring palmar abduction and thumb opposition. We evaluated 14 patients through questionnaires and seven patients through additional physical examination (thumb range of motion, ability to tip pinch, grip/pinch strength) for a mean follow-up of 2.8 years. All patients showed evidence of palmar abduction with 71% dem

BACKGROUND: Patients with obstetrical brachial plexus palsy (OBPP) sequela exhibit adduction and internal rotation contractures. The muscular imbalance may result in secondary bony changes. Tendon transfers and muscular releases may improve shoulder function in these patients. The aim of this study is to evaluate the functional and radiological results of pectoralis major tendon Z-plasty with transfer of latissimus dorsi and teres major tendons to rotator cuff tendons without release of subscapularis muscle in patients with mild sequela of OBPP.

BACKGROUND: Arthroscopically assisted lower trapezius (LT) tendon transfer is an effective surgical option for irreparable posterior superior rotator cuff tears (PSRCTs). The present study aims to compare postoperative clinical outcomes between patients with normal weight (body mass index (BMI) 18.5-24.9) and those classified as Overweight (BMI 25.0-29.9) following arthroscopically assisted LT transfer for irreparable PSRCTs. It was hypothesized that BMI does not influence postoperative functional outcomes.
METHODS: Patients with irreparable PSRCTs who underwent arthroscopically assisted LT t…

BACKGROUND: Posterosuperior irreparable rotator cuff tears (PSIRCTs) are challenging to manage. Lower trapezius tendon (LTT) transfer using an interpositional graft has shown promise in PSIRCTs, with tubularization of the graft potentially contributing to improved outcomes. This study investigates the graft's morphological changes and their impact on clinical outcomes.
MATERIALS AND METHODS: A retrospective analysis was conducted on 105 patients who underwent LTT transfer using interpositional grafts-65 Achilles tendon allografts and 40 fascia lata autografts-for the treatment of PSIRCTs, wit

UNLABELLED: Latissimus dorsi tendon transfer is a surgical option for treating irreparable posterosuperior rotator cuff tears, notably when attempting to reconstruct active external rotation. We hypothesized that the positioning of the transfer's point of fixation would differ depending on the desired elbow-to-body external rotation or external rotation with the elbow abducted.
MATERIAL AND METHODS: Seven shoulders from four whole frozen cadavers were used. We created two systems to install the subject in a semi-seated position to allow external rotation elbow to body and the arm abducted 90°…

PURPOSE: Transfer of the extensor indicis proprius tendon to the distal extensor pollicis longus (EPL) tendon is a standard operation to restore thumb extension. However, several postoperative hand therapy regimens exist. The previously described early dynamic extension splinting protocol has become our standard, and we now compare it with an early active protocol in a prospective randomized study.
METHODS: Twenty-one patients with a closed EPL tendon rupture in zones T4 and T5 were treated with an extensor indicis proprius tendon transfer and were randomly divided into 2 postoperative physic…
