Incidence of extensor carpi ulnaris pathology in patients with triangular fibrocartilage complex foveal repairs.

We analysed the association between extensor carpi ulnaris pathology and triangular fibrocartilage complex foveal tears. In total, 178 patients (58 patients and 120 controls) were retrospectively recruited. The patient group comprised individuals with foveal tears. Extensor carpi ulnaris pathology was defined as the presence of tenosynovitis, tendinosis and high-grade tendon subluxation in magnetic resonance images. Tenosynovitis and a high grade of subluxation were more prevalent in patients than in controls (43 vs. 23% and 26 vs. 14%, respectively) whereas the incidence of tendinosis did not…

The Natural Sequence in Which Subclinical Inflamed Joint Tissues Subside or Progress to Rheumatoid Arthritis: A Study of Serial MRIs in the TREAT EARLIER Trial.

OBJECTIVE: The natural trajectory of clinical arthritis progression at the tissue level remains elusive. We hypothesized that subclinical inflammation in different joint tissues (synovitis, tenosynovitis, osteitis) increases in a distinct temporal order in patients with clinically suspect arthralgia (CSA) who develop rheumatoid arthritis (RA) and subsides in a different sequence when CSA spontaneously resolves. METHODS: We studied 185 serial magnetic resonance images (MRIs) from CSA patients with subclinical joint inflammation from the placebo arm of the TREAT EARLIER trial: 52 MRIs from 21 R…

Ankle and foot pathologies in early rheumatoid arthritis, what can ultrasound tell us?

BACKGROUND: Rheumatoid arthritis (RA) is a systemic autoimmune polyarticular disease. Despite being commonly affected in RA, the ankle and f … LTS: In a cohort of patients with early RA with median duration of 1 month, tibialis posterior (TP) tenosynovitis (45.4%) was the most common pathology, followed by tibiotalar (TTJ) synovitis (39.8%), and peroneal tenosynovitis (39.1%). In terms of disease duration, TTJ (P = .001) foot pathologies were less common in early RA and tended to worsen over time, whe …

Trigger digits: diagnosis and treatment.

Stenosing tenosynovitis of the thumb and fingers is a very common problem seen by the primary-care physician, the orthopaedic surgeon, and the hand surgeon. Primary stenosing tenosynovitis is usually idiopathic and occurs more frequently in middle-aged women than in men, but can be seen even in infancy. Secondary stenosing tenosynovitis of the digits can occur in patients with rheumatoid arthritis, diabetes mellitus, gout, and other disease entities that cause connective tissue disorders. The diagnosis of triggering digits is generally not subtle and can be made on the basis of an adequate cli…

Osseous involvement in de Quervain's disease.

Osseous involvement is usually not appreciated in de Quervain's stenosing tenosynovitis. A case of stenosing tenosynovitis accompanied by significant local osteoporosis in the radial styloid area and a corresponding positive area on a technetium-99m polyphosphonate bone scan provoked the authors' interest in this entity. In seven consecutive patients with de Quervain's disease, six had evidence of osseous involvement of the radial styloid as demonstrated by roentgenographic studies and/or technetium bone scanning. In the process of differential diagnosis of a lytic lesion in the radial styloid…

Magnetic resonance imaging-assessed synovial and bone changes in hand and wrist joints of rheumatoid arthritis patients.

BACKGROUND/AIMS: Magnetic resonance imaging (MRI) is a sensitive and useful method for the detection of synovitis and joint destruction in rheumatoid arthritis (RA) patients. However, the patterns of MRI-detected bone erosion, bone marrow edema (BME), synovitis, and tenosynovitis have received insufficient attention. Therefore, this study evaluated the patterns of bone erosion, BME, synovitis, and tenosynovitis, and calculated the RA-MRI score (RAMRIS) of patients with RA at the carpal and metacarpophalangeal (MCP) joints using MRI. METHODS: MRI datasets from 43 RA patients were analyzed. All…

Recognizing the Unexpected: A Rare Cause of Rice Bodies in Brucellosis.

Brucella is a gram-negative zoonotic agent transmitted by consumption of raw milk and infected meat. Among musculoskeletal manifestations, axial involvement such as spondylodiscitis and sacroiliitis is well documented, while peripheral manifestations like tenosynovitis and rice bodies remain underreported. In this case, flexor tenosynovitis and associated rice body involvement due to Brucella, which developed progressive swelling, pain, and restricted movement in the wrist and third finger of the hand after a minor abrasion, were investigated ultrasonographically. As far as we know, this is th…

Hazards of steroid injection: Suppurative extensor tendon rupture.

Local steroid injections are often administered in the office setting for treatment of trigger finger, carpal tunnel syndrome, de Quervain's tenosynovitis, and basal joint arthritis. If attention is paid to sterile technique, infectious complications are rare. We present a case of suppurative extensor tenosynovitis arising after local steroid injection for vague symptoms of dorsal hand and wrist pain. The progression of signs and symptoms following injection suggests a natural history involving bacterial superinfection leading to tendon rupture. We discuss the pitfalls of local steroid injecti…

Traumatic pathologies of the calcaneal peroneal tubercle.

The peroneal tubercle occurs in about one half of human feet. It size varies from a small nodule of less than 1mm height to a prominent spur up to 1cm in size. It had been noted to cause stenosing tenosynovitis of the peroneal tendons, especially in association with an accessory peroneal bone. The current manuscript describes two cases of symptomatic tenosynovitis developing after an ankle sprain with fracture of the peroneal tubercle. The tenosynovitis caused dysfunction of the peroneal tendons unresponsive to prolonged conservative therapy. Symptoms were resolved following surgical resection…

An uncommon cause for a common complaint.

Infective tenosynovitis is an uncommon cause of a common condition namely carpal tunnel syndrome. Following an extensive review of the literature, we report what we understand to be the first published case of Mycobacterium kansasii (M. kansasii) causing tenosynovitis of flexor tendons resulting in carpal tunnel syndrome in Australia. Our case highlights the need for a high level of suspension, histology and appropriate culture with specific microbiological tests for atypical mycobacteria where tenosynovitis is present at carpal tunnel surgery, even in patients who do not appear to have risk f…

Mastocytoma in the common carpal sheath of the digital flexor tendons of a horse.

A 13-year-old Morgan gelding was examined for right forelimb lameness and tenosynovitis of the right common carpal sheath of the digital flexor tendons. The horse had moderate right forelimb lameness at the trot and marked effusion of the right common carpal sheath of the digital flexor tendons. Ultrasonographic examination revealed a soft tissue mass within the proximal pouch of the affected tendon sheath, located adjacent to the distal physis of the radius. Cytology and culture of the fluid revealed a sterile, eosinophilic tenosynovitis. Tenoscopic exploration confirmed the presence of a cap…

Tendon friction rubs in systemic sclerosis: a possible explanation--an ultrasound and magnetic resonance imaging study.

OBJECTIVE: To assess the tendon and joint involvement at wrists and ankles of patients suffering from diffuse SSc and to identify the morphological substrate of tendon friction rubs (TFRs). METHODS: Fifteen consecutive patients suffering from diffuse SSc were included. All patients had two musculoskeletal US (MSUS) examinations of the wrists and ankles. MRI was performed at the most affected joints as detected by MSUS and in all sites in which TFRs were present. RESULTS: No clinically overt arthritis or tenosynovitis was detected in the wrists and/or ankles prior to MSUS. Synovitis, tenosyno…

The use of ultrasound to predict flexor tendinopathy after anterior locking plating of the distal radius.

This study compares ultrasound to clinical and radiographic measurements for assessing tendon pathology associated with distal radial anterior locking plates. A total of 46 patients undergoing removal of a distal radial plate had a preoperative clinical examination, radiographs and ultrasound evaluation to detect evidence of tendon irritation. Gross changes to the tendon were assessed during plate removal. In total, 32 patients demonstrated clinical abnormality. Soong 2 position was noted in 13 patients. Ultrasound revealed tenosynovitis in nine patients, tendon fibrillation or thickening in f…

What is the additional value of MRI of the foot to the hand in undifferentiated arthritis to predict rheumatoid arthritis development?

BACKGROUND: MRI-detected subclinical joint inflammation in the hand joints of patients with undifferentiated arthritis (UA) predicts progression to rheumatoid arthritis (RA). It is unknown if adding MRI of the foot increases predictive accuracy compared to the hand alone. METHODS: 1.5-T contrast-enhanced MRI of the unilateral foot (MTP-1-5) and hand (MCP-2-5 and wrist) was performed in 123 patients presenting with UA (not fulfilling the 2010 RA criteria) and scored for bone marrow edema (BME), synovitis and tenosynovitis. Symptom-free controls (n = 193) served as a reference for defining an a…
