[Depression, cancer and physician-assisted euthanasia].

A 73-year-old woman suffering from chronic recurrent depression and in the terminal phase of breast cancer requested euthanasia from her family doctor. Patients with a history of chronic depression have more difficulty proving that they have made a conscious choice to terminate their lives; however, depression does not necessarily alter the patient's ability to make decisions. In order to judge each case adequately, information from all those involved in the case (e.g. family, professionals) is important. It is vital that a SCEN ('Support and Counselling by Euthanasia in the Netherlands') doct

Symptom control in advanced cancer.

Most important is the physician's attitude, energy, and skill focused toward helping the patient through the difficulties in the last weeks of life. Many causes of nausea, dyspnea, and other troublesome symptoms can be effectively treated, and pharmacologic palliation is nearly always possible. A multidisciplinary team (ie, hospice) can help the physician see the whole picture and needs the physician's input to effectively manage physical, psychosocial, and spiritual components of the situation. The physician who attends to his own physical, psychosocial, and spiritual needs will have the stam

A longitudinal study of end-of-life preferences of terminally-ill people who live alone.

As a home death seems to be the perceived ideal, terminally ill people who live alone are at a disadvantage in terms of their place of care and death and little is known about their end-of-life preferences. This study aimed at eliciting patient preferences for their place of care and death longitudinally at two points during their illness trajectory, and reporting on the extent they were able to achieve their place of choice or congruence between preferred and actual place of death. A questionnaire was administered by a researcher in the patients' homes during two visits, 6-12 weeks apart depe

Learning from end-of-life injectable medication patient safety incidents in the community: a mixed-methods analysis.

BACKGROUND: Processes to implement injectable end-of-life symptom control medications in the community are complex and can have an adverse impact on patient safety. Recurring patient safety incident types and their contributory factors remain underrecognised, inhibiting system-wide learning. AIM: To understand injectable end-of-life symptom control medication incidents, their contributory factors, the impact on patients/families, and identify priority areas for improving safe, effective, and timely care. DESIGN AND SETTING: Mixed-methods analysis of nationally reported injectable medication

Revealing the hidden harms in end-of-life care: a mixed-methods characterisation of reported safety incidents involving injectable symptom control medication.

BACKGROUND: Many patients dying in the community are prescribed injectable medications and are vulnerable to unsafe care. Developing safer and effective healthcare systems requires learning from patient safety incidents, including those resulting in no harm or near misses, however, health systems typically, must prioritise learning from harmful incidents because of resource constraints and are at risk of missing key learning. AIM: To appraise the nature and outcomes of reported 'no harm' injectable end-of-life symptom control medication incidents, and understand the characteristics of those i

Rapid discharge checklists for people at the end of life who present to the emergency department.

People often express a preference for dying in a home setting rather than in hospital. In emergency departments (EDs), system pressures and the complexity of organising discharges mean that people who are approaching the end of life may be admitted to and die in hospital, even when discharging them to their preferred place may have been indicated and feasible. A rapid discharge checklist can support the process of safely and efficiently discharging patients at the end of life from the ED. It outlines essential steps in the discharge process, including confirming the suitability of discharge, l

Interventions to Promote Hope in People with Advanced Illness: Rapid Review.

Hope is a central element of well-being in people with advanced illness, shaping how individuals cope, sustain meaning, and remain engaged with care despite a limited prognosis. However, evidence on how to promote hope remains fragmented, limiting its integration into palliative care practice. The aim of this rapid review was to identify and synthesize the available evidence on interventions aimed at promoting hope in people with advanced illness, with particular attention to their components and reported effectiveness. The review was conducted following Cochrane recommendations for rapid revi

Assisted dying and countertransference: unconscious forces in life-and-death decisions.

Assisted dying debates overlook the powerful unconscious forces that shape end-of-life decision-making. These dynamics influence personal, clinical and societal judgements and may be contributing to the rapid international expansion of assisted dying practices. Strengthening safeguards requires acknowledging these forces and integrating structured psychological assessment, clinician support and reflective practice to reduce unconscious bias and enhance the reliability, transparency and ethical integrity of decisions.

Intersectionality-Informed HIV Cure-Related Research at the End of Life: A Call to Action.

Introduction: End-of-life (EOL) HIV cure-related research offers a unique opportunity to advance scientific discovery while honoring the values, dignity, and legacy of people with HIV. However, participation remains demographically skewed, mirroring long-standing inequities in who is informed, invited, and supported to take part. Synthesizing eight years of experience, published literature reviews, and community engagement from the University of California San Diego's Last Gift program, we propose strategies to embed justice, equity, diversity, inclusion, and accessibility (JEDIA) throughout t

Considering Patient, Family, and Provider Goals and Expectations in a Rapidly Changing Clinical Context: A Framework for Antimicrobial Stewardship at the End of Life.

Antibiotic administration is often a part of end-of-life (EOL) care, including among patients who are not critically ill. Guideline-issuing bodies recommend that antimicrobial stewardship providers (ASPs) provide support to prescribers making decisions about whether or not to treat infections in this population. Relatively little is known about the rationale for antimicrobial prescribing during the EOL period in noncritical care settings, although patient and family preferences are often an influencing factor. The effectiveness of antimicrobials in improving quantity or quality of life in this

End-of-life dreams and visions in a patient with delirium: A Brazilian case report and narrative review.

OBJECTIVES: End-of-life dreams and visions (ELDVs) and delirium frequently occur near death but differ in core features. Clinical differentiation becomes challenging when they co-occur. This case report illustrates the interplay between ELDVs and delirium, examines the limits of current diagnostic criteria in mixed cases, and outlines a nuanced approach to distinction. METHODS: We report the case of an elderly Brazilian woman with metastatic cancer who exhibited both ELDVs and delirium. Mental status was serially assessed using the Confusion Assessment Method (CAM). ELDV accounts were prospec

Embodied decisions unfolding over time: a meta-ethnography systematic review of people with cancer's reasons for delaying or declining end-of-life care.

BACKGROUND: Barriers to accessing hospice and palliative care have been well studied. An important yet less researched area is why people approaching the end-of-life decline a referral when they are offered services. This review focused on synthesising literature on patients in the last months of life due to a cancer diagnosis who have declined a referral to end-of-life care. METHODS: Six academic databases were systematically searched for qualitative literature published between 2007 and 2021. Two researchers independently reviewed and critically appraised the studies. Using meta-ethnographi

Caring for Appalachians With Intravenous Drug Use-Associated Infective Endocarditis at End of Life: A Phenomenological Secondary Data Analysis.

The purpose of this study was to describe the experiences of nurses caring for patients with intravenous drug use-associated infective endocarditis at the end of life in Appalachia. This study was a secondary analysis of 9 phenomenological unstructured interviews conducted by the author with nurses as part of a study exploring their lived experiences caring for this population. Data were analyzed using van Manen's 6-step approach. The analysis revealed 5 themes: (1) care at the end of life as a resolve to care for lost causes, (2) care at the end of life as an act of courageous communication,

A first voice perspective of people experiencing homelessness on preferences for the end-of-life and end-of-life care during the COVID-19 pandemic.

OBJECTIVE: People experiencing homelessness often encounter progressive illness(es) earlier and are at increased risk of mortality compared to the housed population. There are limited resources available to serve this population at the end-of-life (EOL). The purpose of this study was to gain insight into preferences for the EOL and end-of-life care for people experiencing homelessness. Utilizing an interpretive phenomenology methodology and the theoretical lens of critical social theory, we present results from 3 participants interviewed from August to October 2020, with current or previous ex

Predictive Factors for the Development of Dyspnea Within 7 Days After Admission Among Terminally Ill Cancer Patients.

BACKGROUND: Predictive factors for the development of dyspnea have not been reported among terminally ill cancer patients. OBJECTIVE: This current study aimed to identify the predictive factors attributed to the development of dyspnea within 7 days after admission among patients with cancer. METHODS: This was a secondary analysis of a multicenter prospective observational study on the dying process among patients admitted in inpatient hospices/palliative care units. Patients were divided into 2 groups: those who developed dyspnea (development group) and those who did not (non-development gro

Preferences of Cognitively Impaired Patients and Patients Living with Dementia: A Systematic Review of Quantitative Patient Preference Studies.

BACKGROUND: Treatment decisions based on guidelines rather than patients' preferences determine adherence to and compliance with treatment, which, in turn, could improve health-related outcomes. OBJECTIVES: To summarize the stated treatment and care preferences of people with dementia (PwD). METHODS: A systematic review was conducted to assess the stated preferences of PwD. The inclusion criterion was the use of quantitative methods to elicit stated preferences, enabling a ranking of preferences. RESULTS: Eleven studies revealed preferences for diagnostics, treatment decisions, patient-rela

Deactivation of Implantable Cardioverter-Defibrillators in Heart Failure: A Systematic Review.

Implantable cardioverter-defibrillator aids in the prevention of cardiac arrest by delivering an electrical shock in the presence of life-threatening ventricular arrhythmias. Although implantable cardioverter-defibrillators are essential to sustain life in patients with end-stage heart failure, it is important to consider the option for prompt deactivation of implantable cardioverter-defibrillators to prevent inappropriate electrical shocks at the end of life where death is inevitable. In this systematic review, available literature was reviewed, using six electronic databases, to identify pro
