Comparison of 10- versus 14-gauge angiocatheter for treatment of tension pneumothorax and tension-induced pulseless electrical activity with hemorrhagic shock: Bigger is still better.

BACKGROUND: Little is known regarding the effect of hemorrhagic shock on the diagnosis and treatment of tension pneumothorax (tPTX). Recently, the Tactical Combat Casualty Care guidelines included the 10-gauge angiocatheter (10-g AC) as an acceptable alternative to the 14-g AC. This study sought to compare these two devices for decompression of tPTX and rescue from tension-induced pulseless electric activity (tPEA) in the setting of a concomitant 30% estimated blood volume hemorrhage. METHODS: Following a controlled hemorrhage, carbon dioxide was insufflated into the chest to induce either tP

[Pneumotorax and cannabis].

AIM: After having read the articles by Treasure and Beshay about the particular incidence of spontaneous pneumothorax (SP) in patients with daily consumption of Cannabis the authors took a strictly control of these subjects reviewing their personal experience in the treatment of this condition. In particular, the prevalence of SP has been evaluated through genomic assay of monocorial twins. The strong impact of the inflammatory phenomena that have always supported the emphysema burning is 10 time higher among worldwide population and patients with SP. METHODS: In order to determine the real i

Laparoscopy and transdiaphragmatic thoracoscopy in management of ruptured amebic liver abscess.

In this paper, we present a case of an amebic liver abscess in a 30-month-old child, which presented to the senior author with a right empyema thoracic. Diagnosis was made with ultrasound of the abdomen and chest and a computed tomography scan. A surgical intervention was done after initial management, including when the chest-drain insertion failed. Laparoscopy was performed. With three working ports in the abdomen, the abscess was deroofed and the pus removed. A 3-cm defect in the diaphragm was identified. An intercostal drain site was utilized to use sponge holders and suction catheters to

Tension pneumothorax and the "forbidden CXR".

A case is presented of unilateral tension pneumothorax associated with flail chest and pulmonary contusions in a spontaneously ventilating patient after a fall. The tension element was not suspected until chest x ray was available, nor was immediate needle thoracocentesis performed. No morbidity resulted as a consequence. This case highlights the difficulty in deciding whether or not tension pneumothorax is the predominant cause of respiratory distress in a patient with multiple chest injuries. It provides further evidence challenging some of the doctrine on how to treat suspected tension pneu

Chest wall thickness may limit adequate drainage of tension pneumothorax by needle thoracocentesis.

Tension pneumothorax in a large man was inadequately drained by needle thoracocentesis with a 4.5 cm cannula. Unsuccessful needle thoracocentesis of a clinical tension pneumothorax in a large patient should be followed immediately by chest drain insertion, without local anaesthetic, as dictated by clinical urgency. If the clinical situation is still not improved other diagnoses should be considered.

The needle-wire-dilator technique for the insertion of chest tubes in pediatric patients.

We evaluated the needle-wire-dilator (NWD) technique, using commercially available sets, for insertion of chest tubes in 24 pediatric patients who were admitted to our pediatric intensive care unit (PICU). Fourteen patients had pneumothoraces, three had hemothoraces, two had pneumonia with empyema, four had pleural transudate effusions, and one had chylothorax. The ages of the patients ranged from four months to 24 years, and the sizes of the inserted chest tubes ranged from 10F to 20F. All insertions were successful, and the time from invasion of the pleural space by the needle to completion

Midtrimester thoracoamniotic shunting for the treatment of fetal hydrops.

The management of fetal hydrops in the second trimester is changing as the underlying etiologies are better understood. We report a case which was diagnosed at 18 weeks gestation. There was no underlying anatomical abnormality. Fetal blood sampling confirmed a normal karyotype and there was no evidence of fetal infection. Bilateral thoracocentesis and paracentesis caused temporary improvement of the fetal condition but subsequent fluid reaccumulation was noted within the left pleural cavity. A pleuroamniotic shunt inserted at 22 weeks caused permanent resolution of the hydrops. The infant was

COVID-19 exacerbating ESRD induced pericardial effusion requiring pericardial window: A case report.

INTRODUCTION: Pericardial effusion can lead to cardiac tamponade requiring immediate management. Pericardial windows are an effective measure to surgically drain recurrent effusions. PRESENTATION OF CASE: We present a case of a 53-year-old male with recurrent pericardial effusion secondary to ESRD and exacerbated by COVID-19 infection, describing the successful management of an uncommon clinical scenario. A sub-xiphoid pericardial window and right sided thoracostomy was performed. DISCUSSION: There are limited case reports highlighting recurrent pericardial effusion management, secondary to …

Two rare complications in a single patient of lung cancer: Radiation-induced spontaneous esophageal perforation and aortic rupture and their successful management.

Adjuvant radiotherapy is indicated in few operated cases of lung cancer for effective local control. Oesophageal perforation and aortic rupture are rare and lethal complications of postoperative adjuvant radiotherapy. Both of these complications happened in a 64 year male patient with squamous cell carcinoma of lung. Radiation induced oesophageal perforation occurred immediately after completion of radiotherapy. Endoscopic Self Expanding Radio Therapy (SEMS) was placed, thoracic cavity was debrided and window thoracostomy was performed. After few months of this episode, he had torrential bleed…

Western Trauma Association critical decisions in trauma: Traumatic hemothorax.

ABSTRACT: Traumatic hemothorax is a common consequence of thoracic injury, with management strategies ranging from observation to tube thoracostomy and operative intervention. This Western Trauma Association algorithm provides a practical framework for the evaluation and management of patients with traumatic hemothorax. Recommendations were developed through review of the available literature, including randomized and observational studies, supplemented by expert consensus where high-quality evidence is limited. The proposed pathway incorporates patient physiology, hemothorax size, adequacy of…

Iatrogenic Pneumothorax in a Cat Following Placement of a Continuous Glucose Sensor.

OBJECTIVE: To describe the development and management of an iatrogenic pneumothorax in a cat after placement of a continuous glucose monitor sensor. CASE SUMMARY: A 13-year-old neutered male domestic shorthair cat was presented to the emergency department for signs related to unregulated diabetes mellitus. A continuous glucose monitor was placed to facilitate glycemic monitoring during hospitalization. After placement of the sensor, the cat developed a unilateral iatrogenic pneumothorax that required placement of a thoracostomy tube for intermittent evacuation. The cat's pneumothorax resolved…

A Rare Case of Catamenial Pneumothorax and a Review of the Current Literature.

A 34-year-old female smoker, with a history of pelvic endometriosis, presented with initial symptoms of shortness of breath and a choking sensation. She was found to have a right pneumothorax on chest x-ray. Over the next eight months, she ultimately underwent three tube thoracostomies, two video-assisted thoracoscopic surgeries (VATS), wedge resection, and repeated pleurodesis due to pneumothorax recurrence. She was seen multiple times post-surgically with the focus of treatment being smoking cessation rather than contraceptive therapy, despite an early follow-up visit noting that the initial

Right-sided luxation of the heart caused by blunt traumatic pericardial rupture.

Cardiac luxation is a rare condition in cases of blunt thoracic trauma, yet it is quite fatal. We present a case of a 28-year-old man, admitted to the emergency room after a motorcycle accident in a hemodynamically unstable condition and radiographic presentation of multiple rib fractures, bilateral pneumothorax, pneumomediastinum, and significant dislocation of the heart to the right. After performing emergency bilateral tube thoracostomy and achieving hemodynamic stability, a CT scan was performed and the patient was diagnosed with pericardial rupture with right-sided luxation of the heart. …

A very rare fatal complication: bilateral reexpansion pulmonary edema after treatment of single side total pneumothothorax.

Reexpansion pulmonary edema is a rare but fatal clinical condition that develops during the treatment of pneumothorax, pleural effusion and collapsed lung after atelectasis. A 31-year-old male patient was referred to our clinic with the complaint of stinging back pain that started 3 days ago and dyspnea developed during the last 24 hours. Physical examination and radiologic examinations revealed total pneumothorax in the right hemithorax. After tube thoracostomy, his general condition deteriorated and bilateral reexpension edema developed in the lungs. The patient was admitted to the intensive…
