The purpose of this article is to address the techniques of reconstruction of the forehead and the temple trying to add an "aesthetic" dimension, as the fourth dimension introduced by Burget for the reconstructive surgery of the nose. Achieve "aesthetic" reconstruction is to have constant attention to details. This is how to choose the right indication depending on the age and the etiology of the defect. This is to place incisions to perfection. And it is to be humble and open oneself to criticism, always seeking to improve the quality of its results.

Postpneumonectomy syndrome (PPS) is a rare syndrome whereby the airway and oesophagus become obstructed due to extreme rotation of the mediastinum after pneumonectomy. Our treatment of this condition involved mediastinal repositioning and placement of a saline filled tissue expander into the pleural space. This has not been reported in the plastic surgical literature and we describe technical difficulties and how they were overcome.

We describe the first reported use of an expanded thoracoepigastric myocutaneous flap in the closure of cloacal exstrophy. This approach offers several distinct advantages. Primary scalp wound closure using intra-operative scalp expansion in a craniosynostosis operation is described. Since this method enables easy scalp expansion, it is considered to be a useful adjunctive technique which should be taken into account when closing scalp wounds in craniosynostosis surgery, where the risk exists of cranial expansion-induced compression and deformity of the remodelled bone flaps after bone fixation.

A new method of treating syndactyly is described. The fingers are separated by graduated steps in a special apparatus using skeletal fixation of the phalanges with wires. Skin grafting is then carried out. Scarring and deformity are avoided. The operation can be used for very young children and our patients were aged from 5 months to 30 years. The results of 60 operations are reviewed: 56 were good, 3 satisfactory and 1 unsatisfactory. The advantages of the procedure are discussed.

Ocular atrophy in prime infancy leads to lack of orbital growth. Current treatment of plastic surgery in severe deformities is unsatisfactory, whereas tissue expanding techniques using intraorbital balloon are effective on orbital cavity growth. The objective of this article was to explore the effect of paraorbital soft-tissue expansion before orbital osteotomies and medial translocation by combined intracranial-extracranial approach. Tissue expansion was implanted in the zygomatic and temporal region 3 weeks before traditional operation in 2 cases of severe orbital hypertelorism. Soft-tissue expansion has undergone wide application in reconstructive surgery. The technique has been used about the head and neck, for breast reconstruction, and for reconstruction in the extremities. Scalp reconstruction, total nasal reconstruction, and the movement of flaps of bearded skin using soft-tissue expansion offer selected patients improved results as compared with what could be achieved by using other methods.
