[Severe laryngeal injury due to a nasogastric tube].

We report the case of a 71-year-old woman where nasogastric tube provoked a severe laryngeal injury, revealed by acute respiratory distress. Direct laryngoscopic findings included arytenoid edema, postcricoid ulceration and bilateral vocal cord paralysis in adduction. A tracheotomy was performed. Several months later, no improvement was noted because of a cricoid necrosis. Physiopathology, prophylaxis and management are discussed.

[Acute bilateral recurrent laryngeal nerve paralysis, a symptom of Shy-Drager syndrome].

The Shy-Drager syndrome causes symptoms of multiple nervous atrophy and orthostatic hypotension. This rare disease was diagnosed in a 40-year-old man after he developed an acute bilateral vocal fold paresis. The respiratory failure required an immediate tracheotomy. The Shy-Drager syndrome can include cranial nerve lesions. It should be taken into account in differential diagnosis in ENT diagnoses.

Submental intubation in maxillofacial fracture: a case report.

It can be challenging to create a safe airway in maxilla facial fracture and some skull surgeries. In this case study, the patient experienced jaw fractures that disturbed the dental occlusion and associated fracture of the base of the skull. Neither nasal nor oral intubation was possible based on the side effects of tracheotomy; therefore, submental intubation was applied successfully. The procedure and results are presented in the text.

[Subglottic pleomorphic adenoma].

Pleomorphic adenoma is the most common tumor of major salivary glands, but it is unusual into the larynx. The AA. expose a case of a 74-year-old man who consulted for dysphonia and diagnosed as having a subglottic pleomorphic adenoma. The patient needed an urgent tracheotomy owing to the dyspnoea. Removal of the tumor was made through direct laryngoscopy. Literature on pleomorphic laryngeal adenoma is reviewed.

Technique of management of subglottic stenosis.

There have been innumerable approaches to the management of subglottic stenosis, which bear testimony to the difficulty in obtaining predictably satisfactory results. Management includes techniques of endoscopic dilation, laser resection, laryngofissure and stenting, and an ingenious array of plastic reconstructions with or without postoperative stenting. On occasion, permanent intubation with a conventional distal tracheotomy or a silicone rubber T tube may be used.

[Laryngotracheal separation as treatment for severe bronchopulmonary aspiration].

We present the case of a patient with brain stem tumour and severe chronic aspiration. The bilateral dysfunction of lower cranial nerves and the severe gastroesophageal reflux contributed to the aspirations. Despite medical treatment and cuffed tracheotomy tube, she required almost constant hospitalization for a year and a half due to respiratory infections. Laryngotracheal separation dramatically reduced the infections and improved her quality of life.

Tracheal anomalies complicating ventilation of an infant with Apert syndrome.

Apert syndrome is a rare autosomal dominant disorder characterized by severe syndactyly of the feet and hands, craniofacial abnormalities, and craniosynostosis. A 7-month-old male infant with Apert syndrome who underwent direct bronchoscopy and tracheotomy during general anesthesia is presented. Early rigid bronchoscopy is important in these patients when there are problems with the airway, as they have a relatively high incidence of airway anomalies.

Mechanical Insufflation-Exsufflation: The Rest of the Story.

Mechanical insufflation-exsufflation has been reported to decrease pneumonia rates by about 90% for patients with Duchenne muscular dystrophy now living into their 40s and 50s without tracheotomy tubes. It greatly reduces respiratory complications and hospitalization rates to less than one per 10 patient-years for advanced spinal muscular atrophy type 1, through 25-30 years of age. It is most successful from the point at which small children become able to cooperate with it, generally from 3 to 5 years of age. However, since the 1950s, successful use to extubate and decannulate ventilator "unw…

Cervical tracheal resection with cricotracheal anastomosis: experience in adults with grade III-IV tracheal stenosis.

INTRODUCTION: Laryngotracheal stenosis is currently one of the most common complications associated with nasal and orotracheal intubation and tracheotomy. Once established, tracheal stenosis can be a complex and difficult problem to manage. PATIENTS AND METHODS: We retrospectively analysed 2004-2010 data for 12 male patients with postintubation cervical tracheal stenosis (grade III-IV) treated in the otolaryngology department, Mansoura University Hospitals. All patients had a tracheostomy at presentation, and all underwent tracheal resection with primary cricotracheal anastomosis and suprahyo…

[Improved Surgical Technique for Closing the Tracheocutaneous Fistula].

The tracheocutaneous fistula is a late complication after tracheotomy decannulation, which decreases the quality of life. Though several procedures to close tracheocutaneous fistula were reported, postoperative wound dehiscence was frequently noted. We developed new surgical technique to prevent this complication and report a case in which the technique was successfully applied. This surgical technique is useful simple method to be effective, and safe.

[Encephalitis localised in the brain stem and of indetermined etiology. 6 cases].

The authors report six cases of encephalitis localised in the brain stem. This disease occurred frequently during 1975, with the following symptoms and signs: fever, paralysis of several cranial nerves, temporary mental disturbance and C.S.F. changes. Several patients required tracheotomy and aritificial respiration. All patients recovered without any recurrences. Viral studies were negative. The possibility of a Listeria infection cannot be entirely eliminated.

[Pneumomediastinum - a case report].

UNLABELLED: Pneumomediastinum is a rare disorder which often occurs without symptoms. It is sometimes a result of an injury but most often it is spontaneous. Pneumothorax may be iatrogenic; one of the reasons might be tracheotomy. Pneumomediastinum in some cases may be the threat to life and health. In this rare cases surgical intervention is needed, however pneumomediastinum is usually spontaneously absorbed. AIM: The aim of this study was to present the differential diagnosis of pneumomediastinum based on the case of a 60-year-old woman with tracheostomy, diagnosed eight years ago with gene…

The acute effects of uvulopalatopharyngoplasty on breathing during sleep in sleep apnea patients.

Uvulopalatopharyngoplasty (UPPP) is frequently employed in the treatment of obstructive sleep apnea (OSA). Despite anecdotal reports of early post-UPPP deaths and the recommendation by some authors to perform a prophylactic tracheotomy for selected patients at the time of UPPP, there has been no systematic examination of breathing during sleep in the early post-UPPP period. In order to evaluate the early postoperative risk and the need for prophylactic tracheotomy in UPPP patients, we conducted polysomnograms (PSGs) on eight obese OSA patients on the second post-UPPP night and on another obese…
