Papillofoveal traction in macular hole formation: the role of optical coherence tomography.

OBJECTIVES: To determine the validity of the assumption that optical coherence tomographic scans of macular holes have a discrete linear sig … ed DLS. CONCLUSIONS: The DLS corresponds to the posterior vitreous face. Anteronasal papillofoveal traction may generate some macular holes.

[Vitreoretinal tomography and foveolar traction in macular hole development and macular pseudohole].

OBJECT: To clarify the morphologic features and foveolar traction in macular hole development and macular pseudohole. METHODS: The vitreoretinal tomography of idiopathic macular holes and macular pseudoholes was observed with optical coherence tomography (OCT). RESULTS: In stage 1 and 2, foveolar intraretinal splitting was evident. The posterior hyaloid membrane was detected in 7 of 10 eyes with stage 1 and 2 holes. Dome-shaped vitreoretinal separation was seen in 6 of 7 eyes in which the posterior hyaloid membranes were detected. A complete posterior vitreous detachment was seen around a st…

Epiretinal membrane and traction retinal detachment complicating laser-induced chorioretinal venous anastomosis.

PURPOSE: To report a patient with a sight-threatening complication of laser-induced chorioretinal venous anastomosis for treatment of central retinal vein occlusion. METHOD: A 69-year-old woman with a nonischemic central retinal vein occlusion underwent focal argon laser photocoagulation treatment to create a chorioretinal venous anastomosis. RESULTS: Massive preretinal fibrosis and traction retinal detachment developed over the area of argon laser photocoagulation, which required vitreous surgery. CONCLUSION: Laser-induced chorioretinal venous anastomosis to treat central retinal vein occl…

Traction reduction of acute and acute-on-chronic slipped capital femoral epiphysis.

Gentle reduction of acute or the acute portion of acute-on-chronic severe slipped capital femoral epiphyses (SCFE) is generally recommended. Reports of the use of longitudinal traction with medial rotation traction, although few, are uniform in describing a high rate of success with rare complications. Of 30 acute or acute-on-chronic SCFEs examined at the author's institution between 1970 and 1990, 13 underwent attempted traction reduction with longitudinal traction and medial rotation. Only five of these 13 hips had discernible reduction. One of these five hips developed aseptic necrosis; how…

Papillary muscle traction in mitral valve prolapse: quantitation by two-dimensional echocardiography.

Previous angiographic observations in patients with mitral valve prolapse have suggested that superior leaflet displacement results in abnormal superior tension on the papillary muscle tips that causes their superior traction or displacement. It has further been postulated that such tension can potentially affect the mechanical and electrophysiologic function of the left ventricle. The purpose of this study was to confirm and quantitate this phenomenon noninvasively by using two-dimensional echocardiography to determine whether superior displacement of the papillary muscle tips occurs and its …

Nd:YAG laser photodisruption of the vitreous traction in avulsed retinal vessel syndrome.

We applied Nd:YAG laser photodisruption to vitreous traction in three patients with avulsed retinal vessel syndrome. In every case the vitreous traction on the avulsed vessels was relieved and the vessel that had been avulsed in the vitreous cavity returned to the retinal surface postoperatively. There were no serious intraoperative and postoperative complications, nor was there any evidence of recurrence of vitreous hemorrhage after the treatment.

Closed traction lesions of the brachial plexus--an epidemic among young motorcyclists.

One-hundred-and-sixty patients with closed traction lesions of the brachial plexus sustained in motorcycle accidents were asked about their accident. One-hundred-and-six responded. Most were young men with limited experience of riding motorcycles. Riders with less than one year's experience were more likely to be injured in the absence of another vehicle than were more experienced riders. Two-thirds of those responding felt the present training requirements for motorcyclists to be inadequate.

Epiphyseal traction to correct acquired growth deformities. An animal and clinical investigation.

In a canine model with a transepiphyseal distal femoral fracture, varus deformity was corrected and the arrested limb was lengthened using transphyseal traction (turnbuckle). Correction was possible without removing the transphyseal bone bridge operatively. Subsequent growth was not adversely affected in most animals. Based on this study, two patients with significant growth deformities secondary to an enchondroma were treated by epiphyseal traction. Although correction was achieved initially, both lost the length previously gained because of premature removal of the external fixator and/or su…

Treatment of combined traction-rhegmatogenous retinal detachment in the morning glory syndrome.

A 14-year-old boy had a near total retinal detachment associated with the morning glory optic disc anomaly. A tiny slit-like retinal break with adjacent vitreous traction was found within the coloboma, providing communication between the vitreous cavity and the subretinal space. Vitrectomy, fluid-gas exchange, and postoperative confluent Krypton laser photocoagulation around the optic disc resulted in a successful reattachment of the retina. The retina has remained attached for 14 months.

Is vitreoretinal traction the cause of 'Combined Hamartoma of the Retina and Retinal Pigment Epithelium' (CHRRPE)? A case series and proposal for 'Traction-Induced Retinal and Retinal Pigment Epithelium Lesion' (TRIPEL).

PURPOSE: Combined hamartoma of the retina and retinal pigment epithelium (CHRRPE) is traditionally described as arising from hamartomatous proliferation. We hypothesize that posterior hyaloid membrane (PHM) traction is the primary driver with secondary retinal/RPE changes, supported by imaging and surgical outcomes. METHODS: Dual-center retrospective series of 64 eyes with 'CHRRPE' and OCT imaging. RESULTS: Mean age was 16.2±16.9 years. The PHM was adherent and thickened on OCT in 97%, with broad (>1500µm) attachment in 76%. Traction was present in all eyes (mini-peaks 69%, maxi-peaks in 73%…

Flow-dependent traction in high- and low-flow normal trabecular meshwork cells.

In the conventional aqueous outflow pathway, high-flow (HF) regions are compliant and low-flow (LF) regions are stiff and relatively shear-insensitive. We investigated whether HF- and LF-derived human trabecular meshwork/juxtacanalicular tissue (TM/JCT) cells differ in how shear flow due to aqueous humor outflow is transduced into cell-matrix biomechanics. Normal human HF and LF TM/JCT cells from one donor were cultured on type-I-collagen-coated polyacrylamide gels (∼4.7 kPa) embedded with FluoSpheres for 3D traction force microscopy. A recirculating system (computational fluid dynamics-calibr…

Surgical management of myopic traction maculopathy; expert perspectives from the myopia society.

Myopic traction maculopathy (MTM) occurs in approximately 9-34 % of highly myopic eyes and requires surgical treatment to prevent irreversible vision loss due to progression to macular hole (MH) or retinal detachment (RD). While treatment of MTM, which presents with a variety of morphological patterns, shares the common goal of managing the causative tractional tissues on the retina, it is essential to tailor the surgical plan according to the locations of each lesion. Here, we classify MTM into four core lesions: retinoschisis, foveal RD, lamellar MH (LMH), and full-thickness MH (FTMH), and p…

Autotransplantation of retained canines unable to undergo orthodontic traction. A clinical report.

AIM: The canine is the tooth that, together with the third molar, presents the highest risk of inclusion: 85% of these teeth are located in the palate and 15% in the buccal area. In most treatment plans, retained canines are driven to extrusion through orthodontic treatment. However, in some cases, for technical reasons or patient choice, it is not possible to perform this consolidated approach. The present clinical report evaluates 10 ectopic canines transplanted in both adults and younger patients with up to 60 months of follow-up. CASE DESCRIPTION: Two clinical cases are described in detai

Association between retinal morphological characteristics and visual function in myopic traction maculopathy.

OBJECTIVE: To quantify retinal morphological characteristics in patients with myopic traction maculopathy (MTM), including myopic foveoschisis (MF), using optical coherence tomography angiography (OCTA) and investigate the correlation of MTM with visual function. METHODS: This cross-sectional study included 19 eyes with myopic foveoschisis (MF) and foveal detachment (FD), 37 eyes with MF without FD, and 24 myopic eyes as control. OCTA was used to assess macular vessel density (VD) in the superficial capillary plexus (SCP), deep capillary plexus (DCP), foveal avascular zone (FAZ) area, FAZ per…

PROGRESSION OF MYOPIC MACULOPATHY IN PATIENTS WITH MYOPIC TRACTION MACULOPATHY AFTER VITRECTOMY.

PURPOSE: To compare the progression of myopic maculopathy with or without vitrectomy in patients with myopic traction maculopathy (MTM). METHODS: Seventy-seven eyes with MTM were classified into either the observation group (n = 38) or the vitrectomy group (n = 39). Progression of myopic maculopathy was assessed with fundus photography using infrared images. Progression within stage was evaluated as an increase in the area of atrophic lesions on infrared images using ImageJ software. The rate of progression was compared using the paired t -test. RESULTS: The mean follow-up period was 60.0 ± …
