Human Immunodeficiency Virus (HIV) Care Continuum Outcomes After Transition to Adult Care Among a Prospective Cohort of Youth With HIV in Atlanta, Georgia.

BACKGROUND: Healthcare transition from pediatric to adult-oriented clinical settings is often viewed as a high-risk time for care disengagement. However, there is a paucity of prospective, longitudinal research documenting human immunodeficiency virus (HIV) care outcomes after healthcare transition. METHODS: We conducted a prospective, observational cohort study of healthcare transition among youth enrolled at an HIV care center in Atlanta, Georgia. Pediatric clinic patients (average age, 24 years) were enrolled up to 3 months before the expected transition and were followed up to determine l

Stakeholder experiences and expectations of transition of care services to reduce medication-related hospital readmission for cardiology patients.

BACKGROUND: Transition of care (ToC) for patients with cardiovascular diseases is a complex, high-risk period often leading to medication-related harm (MRH) and hospital readmissions. While specialised programs exist, a gap in individualised medication management services exist for specific cardiology patient populations. Pharmacist-led, interdisciplinary ToC services have demonstrated a positive impact on patient outcomes. The REducing hospital re-admission for high-risk CARDiology patients (RECARD) program aims to address this by co-designing, implementing, and evaluating a new pharmacist-le

Closing adult vaccination gaps through a pharmacist-directed transitions of care service in a safety-net hospital.

BACKGROUND: Adult vaccination gaps remain common, especially among populations with health and socioeconomic disparities. Although pharmacist-led vaccination initiatives have been studied in community settings, less is known about incorporating vaccine assessment and intervention in inpatient settings. This study evaluated the impact of a pharmacist-led inpatient vaccination initiative nested within a transitions of care (PharmTOC) consult service. OBJECTIVES: The primary objective was to assess the impact of an inpatient pharmacist-directed immunization initiative. Secondary objectives were

Core components of a specialty-wide model of care for transition to adult healthcare services: a systematic review of guidelines.

BACKGROUND: Young people with complex healthcare needs diagnosed in childhood require transition to adult services. Without a structured process for transition, the transfer of care is a challenging event for young people during a vulnerable period of their lives. Loss to follow-up can lead to long-term health complications. We conducted a specialty-wide systematic guideline review of clinical practice guidelines on transition care to assist health organisations to develop a model of care for transition services that can apply across medical specialties by synthesising best-practice internatio

Safe Handover or Lost in Transition?-A Retrospective Evaluation of Pharmacist Referrals to Primary Care Following Hospital Discharge.

The medication-related problems identified by clinical pharmacists during hospitalisation may be better addressed after patient discharge and recovery from acute illness. This approach enables follow-up and may reduce the risks associated with multiple medication changes. However, inadequate information transfer at discharge is a recognised patient safety risk. Referral-based communication between clinical pharmacists and general practitioners (GPs) is a novel procedure in southern Sweden. This retrospective observational study aimed to describe the patient population, medication-related probl

Models of care for the management of alcohol use disorder in general hospital settings and transition to the community: a scoping review.

BACKGROUND: Early detection of alcohol use disorder (AUD) amongst people admitted to general hospitals offers an opportunity for early intervention and accesses to evidence-based care. However, current operational models for the management of AUD in these settings are poorly defined, limiting the ability to assess their impact on individual patient outcomes, treatment effectiveness, or healthcare system efficiency. This scoping review aims to identify and characterize existing models of AUD management within general hospitals. METHODS: A scoping review approach was adopted, including relevant

Wearable Devices in Cardiovascular Care: A Narrative Review of the Transition Toward Predictive, Preventive, Personalized, and Participatory Medicine.

Cardiovascular diseases remain the leading cause of global mortality, yet conventional diagnostics are episodic and clinic-centered, missing the dynamic physiological events that unfold between encounters. Wearable devices offer continuous, real-world monitoring and, together with artificial intelligence, digital biomarkers, and telecardiology, increasingly support a shift toward predictive, preventive, personalized, and participatory (P4) cardiovascular care. This narrative review synthesizes the contemporary evidence base for wearable cardiovascular technology and organizes it around the fou

Ageing, health system implications, and a changing epidemic: organizational transitions and stigma dynamics in Philippine HIV care.

BACKGROUND: The global HIV epidemic is undergoing a demographic transition as expanded access to antiretroviral therapy (ART) has increased life expectancy among people living with HIV (PLWH). As a result, the population of older adults living with HIV (OPLWH) is rapidly growing, introducing new clinical, psychosocial, and public health challenges. In the Philippines, one of the fastest-growing HIV epidemics in the Asia-Pacific region, this shift is occurring alongside rising incidence, creating a dual burden of ongoing transmission and emerging survivorship needs. METHODS: This study employe

Social Work Support for Anticipatory Grief in Rural Palliative Care Transitions: A Narrative Review for South Africa.

Family caregivers play a central role in palliative care, especially during hospital-to-home transitions, when rural families may assume complex care with limited preparation and uneven professional support. Anticipatory grief, the grief experienced before a loved one's death, can intensify emotional, practical, social and financial strain. A SANRA-informed, agenda-setting narrative review integrated literature on palliative care, caregiver burden, anticipatory grief, transitional care, rural healthcare and social work to develop a research and practice agenda for rural South African public he…

Transitioning to telemental health: Sociodemographic predictors of integrated behavioral health care utilization pre- and peri-COVID.

INTRODUCTION: The transition to telehealth as a result of the COVID-19 pandemic brought the potential to ameliorate or exacerbate disparities in access to behavioral health care. This study examined patient-specific sociodemographic predictors of integrated behavioral health (IBH) utilization prior to and during COVID to identify variations in care access. METHOD: Data from three primary care clinics across two medical centers (N = 819 patients), and multivariate general linear modeling, were used to test direct associations between age, sex, primary language, and race/ethnicity and use of IB

Transition of patients with Duchenne muscular dystrophy from paediatric to adult care: An international Delphi consensus study.

BACKGROUND: Duchenne muscular dystrophy (DMD) is a rare neuromuscular disorder characterized by a progressive decline in muscle function, leading to loss of ambulation, respiratory and cardiac failure, and ultimately death. Improvements in DMD management have increased patient life expectancy; therefore, there is a growing requirement for patients to transfer from paediatric to adult care services. There is also a need for clear recommendations to guide this process. AIM: To establish international consensus guidelines regarding best practices for transitioning patients with DMD from paediatr

Socioeconomic Inequalities in the Transition to Nursing Home Care: Longitudinal Evidence from a Population Based Study in Switzerland.

Lower socioeconomic status (SES) is linked to earlier nursing home admission, but it is unclear if this remains true after accounting for health needs. This study examines SES-related inequalities in nursing home admission, controlling for health needs in an area of Switzerland with universal home-based care for older adults. Data were from the Lausanne Cohort 65+ study, including 3650 participants. SES was measured by education level, occupational class, and receiving means-tested benefits (a proxy for low income). Health needs were assessed with a set of physical and mental health variables.

We Rely on Relationships: Homeless Service Providers' Experiences in Coordinating Care Transitions During COVID-19.

PURPOSE OF STUDY: Care coordination occurring across multiple sectors of care, such as when professionals in health or social service organizations collaborate to transition patients from hospitals to community-based settings like homeless shelters, happens regularly in practice. While health services research is full of studies on the experiences of case management and care coordination professionals within health care settings, few studies highlight the perspective of nonclinical homeless service providers (HSPs) in coordinating care transitions. PRIMARY PRACTICE SETTING: This study explore

Evaluating Treatment Burden in Patients with Complex Needs Receiving a Transition of Care Intervention: A Rapid Qualitative Analysis.

Many patients, especially those with long-term conditions, face significant challenges in managing their health. Burden of treatment is the effort required for self-managing health. This burden is often intensified by social determinants of health, such as limited access to care and financial instability. Burden of treatment is understudied in socially and medically complex patients, particularly in the critical period of transitioning home after hospital discharge. To address this gap, this study analyzed data from telephone interviews with urban primary care patients who had been recently ho

Transition from Hospital to Home-Based Care for Older Adults in Southwestern Uganda: Informal Caregiver's Experiences.

BACKGROUND: The transition from hospital-based to home-based care is a complex and challenging process. In most developing countries, patients transfer directly from hospital to home immediately after stabilizing from acute illness. After discharge, all the patient's care needs are taken over by informal caregivers whose experiences and practices may directly or indirectly impact the quality of patient care as well as the outcomes. This study aimed at exploring the experiences of informal caregivers during the hospital to home transition process of older adults. METHODS: We conducted a qualit
