The immunological significance of cellular infiltrates in chronic rejection of human kidney transplants.

Fifteen transplanted human kidneys with clinical and histological signs of chronic rejection were examined microscopically for cellular infi … ed human kidneys. Cleaved lymphocytes were the most frequent of the infiltrating cells found in the transplants. However, their role is not yet clear.

Next-generation microbiome therapeutics: psychobiotics and fecal microbiota transplants for mental health treatment.

The human gastrointestinal tract (GIT) harbors a diverse microbial community, collectively referred to as gut microbiota, which plays an essential role in maintaining host physiology, metabolism, immune function, and neurobehavioral processes. Mounting evidence highlights the importance of the bidirectional Gut-brain (GB) axis, through which gut microbes influence neural signalling, stress response, and emotional regulation. Dysbiosis of this axis has been associated with psychiatric, neurodevelopmental, and neurodegenerative disorders, such as anxiety, autism spectrum disorder (ASD), major de

Vitrification and Nanowarming: Redefining the Limits of Allograft Preservation for Partial Heart Transplants.

Over 48,000 organ transplants were performed in 2024, but thousands of organs were discarded due to exceeding cold ischemic times (CIT), which can be as short as 4 hours for hearts. Therefore, current cold storage methods lead to significant loss of transplantation opportunities. Vitrification, a cryopreservation technique that prevents ice formation by rapidly cooling tissues into a glass-like state, offers a promising solution for long-term organ banking. Nanowarming, which uses iron oxide nanoparticles to rewarm tissues uniformly and rapidly, addresses limitations of traditional rewarming m…

Laparoscopic Surgery in Recipients of Intra-abdominal Solid Organ Transplants: A Systematic Review.

OBJECTIVES: Laparoscopy is a minimally invasive surgical approach that can reduce morbidity compared with open surgery in appropriately selected patients. However, patients with a history of intra-abdominal solid organ transplantation may have postoperative adhesions and altered anatomy, leading to more complex and variable anatomy. It remains unclear whether prior transplantation increases the risk of complications during laparoscopy. This study aimed to evaluate the incidence of complications in patients undergoing laparoscopy following intra-abdominal solid organ transplantation. MATERIALS

Combined Multiorgan Heart and Kidney Transplants With Single Donor Allografts: Simultaneous Versus Staged?

BACKGROUND: Multiorgan heart and kidney transplants (HKTx) performed for patients with end-stage heart failure and chronic kidney disease have increased in recent years. However, no established protocols exist on whether a heart and kidney from the same donor should be transplanted in the same operation versus 1-2 days apart. METHODS: Using SRTR data 1993-2023, we compared same-donor HKTx recipients with both transplants performed on the same day ("simultaneous") to recipients with kidney transplants performed within 1 day of the heart transplant ("staged"). We examined differences in weighte…

Transplants foster B cell alloimmunity by relaying extracellular vesicles to follicular dendritic cells.

B cells play fundamental roles in transplant rejection. However, how allogeneic (allo)-antigens (Ags) are transported from allografts to follicular dendritic cells (FDCs) in lymphoid tissues for development of B cell responses remains unknown. We demonstrated that graft allo-Ags are relayed to FDCs via small extracellular vesicles (sEVs), which activate complement via immunoglobulin M (IgM) bound to vesicle phospholipids. Complement-opsonized allo-sEVs bind splenic marginal-zone B cells that shuttle the vesicles to FDCs, which retain and recycle the allo-sEVs so they are recognized by B cells.

Follow up and safety of use of hepatitis C virus discordant liver transplants.

BACKGROUND: Hepatitis C virus (HCV)-discordant liver transplants (LT), nonviremic and viremic, were first shown at our center as safe and efficacious for HCV-negative recipients in the short-term. This review evaluates HCV-discordant LT long-term outcomes. METHODS: All HCV-discordant deceased donor LT from 03/2016-06/2023 were reviewed in this retrospective, single-center study. RESULTS: 194 HCV-discordant LTs (96 (49.5 %) nucleic acid test (NAT)-negative and 98 (50.5 %) NAT-positive) were evaluated with a median follow-up of 53 months. Baseline liver biopsies of 139 (71.6 %) allografts, rep…

In pursuit of an HIV cure: from stem cell transplants to gene therapies.

Since 2009, seven people living with human immunodeficiency virus (PLHIV) have been declared cured of HIV after receiving allogeneic hematopoietic stem cell transplants (alloHSCTs) to treat hematologic malignancies. In this sense, cure signifies the absence of viral DNA/RNA and undetectable viral loads without the use of antiretroviral therapy (ART). Five of these transplants utilized mutated C-C motif chemokine receptor type 5 (CCR5Δ32/Δ32) stem cells. Much has been learned from these and past cases, and although effective, bone marrow transplants cannot be easily or safely translated to cure…

Reasons for COVID-19 Vaccine Hesitancy Among Patients Listed for Solid Organ Transplants.

BACKGROUND: Patients listed for solid organ transplants (LSOTP) are at high risk for severe COVID-19 outcomes. Despite national guidelines recommending COVID-19 vaccination for LSOTP, vaccine hesitancy and underuse are reported in this population; however, reasons for this finding have not been examined thoroughly. METHODS: This single-center retrospective survey analysis aimed to characterize reasons for COVID-19 vaccine hesitancy among 110 heart, liver, and kidney patients LSOTP who had not received all the recommended vaccine doses at the time of the study. Survey questions also investigat…

Spatial Transcriptomic Signatures of Early Acute T Cell-mediated Rejection in Kidney Transplants.

BACKGROUND: Kidney transplantation significantly improves the quality of life for those with end-stage renal failure, yet allograft rejection resulting from immune cell interactions remains a persistent challenge. Although T cell-directed immunosuppressive drugs effectively contain graft rejection in most patients, a notable proportion still experiences acute T cell-mediated rejection (TCMR). Despite an emphasis on suppressing T cell-mediated immune responses, successful control over TCMR is not always achieved, suggesting the potential involvement of factors beyond T cells. METHODS: Biopsy s

Creating a Risk Score for Prolonged Length of Stay Following Pediatric Lung Transplants.

BACKGROUND: Pediatric lung transplant (LT) outcomes correlate with prolonged length of stay (pLOS), yet a paucity of research exists predicting pLOS in this population. We aim to identify factors associated with pLOS following pediatric LT, and to create a risk score identifying individual patients at increased risk of pLOS. METHODS: Using the OPTN database, we analyzed 733 pediatric patients who received an LT between January 2000-July 2022. We used LASSO regularization to identify factors predicting pLOS. A risk score was calculated using odds ratios for each variable in the model. RESULTS

3D Imaging of Striatal Transplants in a Small Animal Model of Huntington's Disease.

High-resolution imaging in small animal models of neurologic disease is a technical challenge. In a pilot project, we have explored a non-destructive synchrotron imaging technique for the 3D visualization of intracerebral tissue transplants in a well-established small animal model of Huntington's disease. Four adult female Sprague Dawley rats each received injections of 0.12 M quinolinic acid (QA) into two target positions in the left striatum, thus creating unilateral left-sided striatal lesions similar to those frequently seen in patients suffering from Huntington's disease. One week after l…

Evaluation of pulmonary abnormalities in recipients of hematopoietic cell transplants and cellular therapies.

Hematopoietic cell transplant (HCT) and chimeric antigen receptor T-cell (CAR-T) therapy recipients are susceptible to multiple pulmonary complications that are caused by infectious and noninfectious processes. Numerous variables can be associated with specific pulmonary diseases including time from transplantation, presence of graft versus host disease (GVHD), underlying disease, and prolonged neutropenia and lymphocytopenia. Most pulmonary complications are infectious in origin, with bacterial pneumonia remaining the most common pulmonary infection, particularly before neutrophil engraftment

CTLA4-Ig mediated immunosuppression favors immunotolerance and restores graft in mouse airway transplants.

CTLA4-Ig is a potent costimulatory blocker that inhibits T cell activation during alloimmune inflammation and increases graft survival and function. CTLA4-Ig-mediated immunosuppression has been demonstrated to support transplant function in various clinical trials and preclinical settings, but its effects on the balance between regulatory T cells (Tregs) and effector T cells (Teffs), as well as complement activation, are less well investigated. In the present study, we proposed to investigate the effects of CTLA4-Ig mediated immunosuppression on the phase of immunotolerance and the subsequent

Personalized tissue-engineered arteries as vascular graft transplants: A safety study in sheep.

Patients with cardiovascular disease often need replacement or bypass of a diseased blood vessel. With disadvantages of both autologous blood vessels and synthetic grafts, tissue engineering is emerging as a promising alternative of advanced therapy medicinal products for individualized blood vessels. By reconditioning of a decellularized blood vessel with the recipient's own peripheral blood, we have been able to prevent rejection without using immunosuppressants and prime grafts for efficient recellularization in vivo. Recently, decellularized veins reconditioned with autologous peripheral b

Outcomes after anti-thymocyte globulin vs Basiliximab induction before deceased donor kidney transplants.

BACKGROUND: Deceased donor kidney transplants represent an important source of renal replacement for the 100 000 patients initiating hemodialysis annually. We compared the association of induction therapy, anti-thymocyte globulin [rabbit] (rATG) or basiliximab, with posttransplant rejection, graft and patient survival. METHODS: Using the United Network for Organ Sharing (UNOS) database, we identified patients that received deceased donor kidney transplants. The outcomes analyzed were 6- month rejection, 1-year rejection, patient survival and graft survival. Multivariate logistic regression mo…

Outcome of Liver Transplants Using Donors After Cardiac Death With Normothermic Regional Perfusion.

BACKGROUND AND AIMS: The incorporation of normothermic regional perfusion (NRP) to donors after cardiac death (DCD) allows the recovery of liver grafts without the deleterious effects on graft survival the super-rapid technique may cause. The aim of the present report is to determine if the use of NRP in Maastricht type III DCD donors achieves short- and medium-term results comparable to donors after brain death (DBD). PATIENTS AND METHODS: This is an observational cohort study including 117 liver transplants executed between November 2016 and April 2021, divided into NRP (n = 39) and DBD (n …
