Pediatric Orthopaedic Transfers to a Level I Trauma Center: After-Hours Burden and Resident Preparedness.

BACKGROUND: Level I pediatric trauma centers manage transferred orthopaedic injuries; however, interfacility transfer (IFT) patterns and educational implications for residents remain poorly characterized. METHODS: We performed a retrospective cohort study of pediatric ground/air emergency medical service transport IFTs to a level I pediatric trauma center for orthopaedic injuries from 2016 to 2021. Variables included demographics, International Classification of Diseases 10th Revision (ICD-10)-coded injuries, referring facility (emergency departments [EDs] and urgent care centers [UCCs]), ar…

Penetrating Abdominal Injuries: A 10-Year Retrospective Analysis of 49 Patients at a German Trauma Center.

BACKGROUND Penetrating abdominal injuries are rare in central Europe; therefore, most trauma surgeons have little experience in the treatment of these injuries. The optimal management of penetrating abdominal injuries remains debated. While immediate surgery is standard for unstable patients or those with peritonitis, bowel evisceration, or gunshot wounds, the treatment of stable patients is still controversial. This study aimed to assess the reliability of preoperative diagnostics compared to intraoperative findings. MATERIAL AND METHODS Forty-nine patients with penetrating abdominal injuries

Prevalence of contrast extravasation and embolization in blunt liver and spleen injuries at a single pediatric trauma center.

PURPOSE OF THE STUDY: In 2019, the APSA guidelines for the management of solid organ injuries in children recommended limiting embolization to patients with evidence of ongoing hemorrhage. We sought to identify patients who presented to our institution with contrast extravasation (CE) on initial imaging, determine the proportion who required embolization, and describe their associated outcomes. METHODS: A retrospective chart review of all patients with blunt liver and/or spleen injuries (BLSI) treated at our institution between 2014 and 2023 was performed. RESULTS: We identified 219 patients

Utilization patterns of low-titer group O whole blood in a level 1 trauma center.

BACKGROUND: Whole blood (WB) is an easy-to-use resuscitation fluid. As more low-titer group O WB has become available at our hospital, it is being used in an increasingly diverse range of situations, both pre-hospital and in-hospital. STUDY DESIGN AND METHODS: We conducted a retrospective analysis of all WB units issued by our transfusion service during calendar year 2024. Data were obtained from the transfusion service database and linked to patient medical records to characterize indications, location of transfusion, and patient outcomes. Results are descriptive. RESULTS: A total of 1915 W

Predictive Factors of Cranioplasty Infection: Results From a Contemporary Level I Trauma Center Cohort.

Cranioplasty restores skull integrity after decompressive craniectomy but carries a risk of surgical site infection that may necessitate implant removal and prolonged antimicrobial therapy. We performed a retrospective study of adult patients who underwent cranioplasty between 2014 and 2022 at a single level 1 trauma center to identify predictors of postoperative infection. Demographic, clinical, and operative variables were abstracted from the electronic medical record. The primary outcome was surgical site infection. Associations were assessed using univariate testing, multivariable logistic

Long-term results of surgical management high-energy scapula fractures at a major trauma center.

BACKGROUND: Scapular fractures are rare injuries typically resulting from high-energy trauma and often occur in polytrauma patients. Historically managed nonoperatively, recent trends in surgical intervention have evolved, particularly in major trauma centers (MTCs) where expertise and case volume have increased. However, there remains limited literature on the surgical management and long-term outcomes of these injuries. METHODS: This retrospective cohort study reviewed all patients who underwent surgical fixation for scapular fractures over a 10-year period at a single UK MTC. Patient demog…

Pediatric Upper Extremity Firearm-related Injuries: A Level I Pediatric Trauma Center Experience.

INTRODUCTION: Firearm injuries have become increasingly more common in the pediatric population; however, there is a paucity of literature examining the management of these pediatric firearm-related injuries (FRI) specifically as they affect the upper extremity. This study identifies demographic and environmental risk factors in pediatric upper extremity FRIs and evaluates the severity of injury, concomitant injuries, and rates of surgical intervention in pediatric patients treated at a Level I pediatric trauma center over 20 years. METHODS: We completed a retrospective analysis on 540 patien

Impact of interhospital transport on outcome in traumatic epidural hematoma: experiences of a level-1 trauma center.

BACKGROUND: Epidural hematomas (EDH) are associated with a high rate of mortality and morbidity. Good clinical outcome depends on initial Glasgow Coma Scale (GCS), pupillary abnormalities, hematoma volume, age and time to surgery. The latter is mostly influenced by distance to the next level-1-trauma center. OBJECTIVE: The aim of this study was to evaluate the surgical care and the influence of a potential interhospital transport of patients with acute EDH. MATERIAL & METHODS: A retrospective analysis of data from 2009 to 2020 was carried out. All patients who underwent surgical evacuation o

Operative neurosurgery for traumatic subdural hematoma: association between trauma center variation and patient outcomes.

OBJECTIVE: Traumatic subdural hematoma (SDH) is a common form of traumatic brain injury (TBI) that often represents a neurosurgical emergency. Surgical evacuation is recommended for SDH with midline shift (MLS) > 5 mm, regardless of the presenting Glasgow Coma Scale (GCS) score; however, real-world practice is unknown. The objective of this study was to test the hypothesis that significant variation exists in the tendency for operative neurosurgical intervention for traumatic SDH among trauma centers (TCs) and that this variation is associated with patient outcomes. METHODS: The authors perfo…

A 10-year review of penetrating subclavian artery injury at a major trauma center in South Africa.

OBJECTIVE: Penetrating subclavian artery injuries are uncommon but notoriously difficult to manage. Heterogeneity in clinical features, hemodynamic stability, and injury patterns make timely diagnosis challenging. Open surgical repair is technically demanding owing to the vessel's location and carries high morbidity and mortality. Advancements in imaging, treatment modalities, and perioperative care alter the spectrum and management of such injuries and warrant ongoing evaluation. METHODS: We performed a retrospective review of all cases over a 10-year period. Consecutive patients with penetr

Early use of professional interpreters improves trauma outcomes: Results of a single-center retrospective study.

Patients with limited English proficiency (LEP) experience reduced pain assessment and treatment, less comprehensive physical exams, and fewer explanations of the next steps in care. These disparities persist in hospitals with staffed professional interpreters, raising questions about interpreter access and the impact on outcomes. A retrospective review of 1133 trauma activations at a single center Level 1 Trauma Center in 2021-2022 was conducted. Demographic, injury, and outcome data were drawn from the institutional trauma registry, and patient-preferred language was pulled from EMR data. Ea

Neurological symptoms in pediatric abusive head trauma: Multi-center data for clinical forensic medicine.

Central nervous system-associated lesions can frequently be found in abusive head trauma (AHT) cases. Since there are frequently no visible signs of injury on the body surface, the diagnosis of AHT can be challenging. In particular, if the affected child shows only isolated neurological symptoms, these are often misinterpreted as a minor illness. Using a retrospective study design at three university hospitals, 72 medico-legal cases of "shaken baby syndrome" - a common variant of AHT - were analyzed. A comparison between confession cases (n=15) and non-confession cases was used in order to red
