A critical analysis of outcome for children sustaining cardiac arrest after blunt trauma.

PURPOSE: Injury is the leading cause of cardiac arrest in children older than 1 year. Previous findings suggest that children who require cardiopulmonary resuscitation (CPR) administered by paramedics for any reason rarely survive to hospital discharge. The authors evaluated the outcome of children sustaining cardiac arrest after blunt trauma in a Regional Pediatric Trauma Center. METHODS: Children (age < 16) who underwent CPR in the field or in the emergency department (ED) after blunt trauma were identified from the trauma registry of a regional pediatric trauma center over a 3-year period

[Management of multiple trauma in the emergency room].

The management of multiple trauma patient in the emergency room is paradoxical because the treatment must be performed as soon as possible, but the precise diagnosis using imaging is time consuming. Multiple trauma might be classified into 3 classes. Patient in class 1 is severely injured with serious neurological, respiratory and/or hemodynamic distress. Imaging procedures only consist on chest x-ray, abdominal ultrasonography and echocardiography if needed, while saving treatments are immediately required such as tracheal intubation and mechanical ventilation in case of severe brain trauma o

The functional outcome of lower-extremity fractures with vascular injury.

Salvage of lower-extremity Gustilo type IIIC fractures is difficult, time-consuming for the patients and physicians, and not universally successful because of poor functional outcomes. Even if successful with limb salvage, the functional result may be unsatisfactory because of mutilating injuries to muscle and nerve, bone loss, and the presence of chronic infection. From July 1991 until July 1994, revascularizations of open IIIC fractures were attempted for wounds with Mangled Extremity Severity Score (MESS) < or = 10. The functional results were evaluated at 2 years after injury. Thirty-six l

A Randomized Controlled Trial of Surgical Rib Fixation in Polytrauma Patients With Flail Chest.

BACKGROUND: Flail chest (FC) is known to account for high mortality and morbidity and is typically treated with conservative care. Operative fixation of FC has been advocated as an alternative treatment choice. This prospective randomized controlled trial aims to compare surgical and nonsurgical management of FC in patients with severe polytrauma. METHODS: Severe polytrauma patients with FC admitted between January 2015 and July 2017 to our trauma center were investigated. The enrolled patients were randomly assigned to the surgical or nonsurgical group. Basic characteristics of injury and cl

Survey of trauma patients requiring maxillofacial intervention, ages 56 to 91 years, with length of stay analysis.

PURPOSE: The purpose of this study was to analyze trauma patients, ages 55 and older, sustaining multiple injuries including maxillofacial trauma. Factors influencing length of intensive care unit stay (ICUS) and length of total hospital stay were delineated and examined to determine if specific causes of increased length of stay could be elucidated, and, once known, if these causes could translate into recommendations tailored to the oral and maxillofacial surgery trauma practice. PATIENTS AND METHODS: One hundred ninety-six patients, 55 years of age or older, who received either consultatio

Trauma in CT: The Role of Severe Injury on Satisfaction of Search Revised.

PURPOSE: The satisfaction-of-search (SOS) effect occurs when an abnormality on an image is missed because another is found. The aim of this experiment was to test whether severe distracting fractures control the magnitude of SOS on other fractures when both appear in a single CT image. METHODS: The institutional review board approved this study. The experimental (SOS) condition included 35 cervical spine CT cases, all of which contained severe cervical spine injuries. For each of these cases, a similar case was found that had no injuries. Image modification software was developed to add simul
