[Peritonsillar phlegmons and abscesses. Retrospective study of 100 cases].

Our main goal was performing a retrospective study about peritonsillar cellulitis and abscess to prove the efficiency of treatment. So that we have collected 100 clinical histories of admitted patients in our ENT Service between 1990 and 2001 with such pathology and analysed a serie of variables and the treatment given in each case. It is one of the most common infectious diseases of the head and neck region. If aspiration positive, it require an incision and drainage to improve the symptoms (throat pain, fever, dysphagia and trismus). We have made no immediate tonsillectomies. All the patient…

Masticatory muscle tendon-aponeurosis hyperplasia that was initially misdiagnosed for polymyositis: a case report and review of the literature.

BACKGROUND: Masticatory muscle tendon-aponeurosis hyperplasia (MMTAH) is a relatively newly identified clinical condition that manifests as trismus with a square-shaped mandible. Herein, we report a case of MMATH that was initially misdiagnosed for polymyositis due to trismus and simultaneous lower limb pain, with literature review. CASE PRESENTATION: A 30-year-old woman had a history of lower limb pain after exertion for 2 years. Initial physical examination had been performed at the Department of General Medicine in our hospital. There was also redness in the hands and fingers. Although pol…

The Advantage of Using Video Laryngoscope in Puncture and Incisional Drainage of Peritonsillar Abscess: A Case Report.

UNLABELLED: This case report demonstrates the use of a video laryngoscope to aid in the incision and drainage of a peritonsillar abscess in a 30-year-old male. This technique, which has not been widely discussed in the literature, provided enhanced visualization, overcoming challenges like poor access due to trismus and poor lighting. The video laryngoscope improved safety, offered clearer guidance, and provided valuable teaching opportunities, particularly for less experienced physicians and residents. This case contributes to the literature by illustrating how the video laryngoscope can func…

Pathogenesis and therapeutic intervention of oral submucous fibrosis.

Oral submucous fibrosis (OSF) is a chronic, progressive, potentially malignant condition affecting the oral cavity and frequently involving the upper part of the aerodigestive tract including the oropharynx and the upper part of the esophagus. It is characterized by juxtaepithelial inflammatory reaction and progressive fibrosis of lamina propria, leading to stiffening of the oral mucosa eventually causing trismus. This condition is associated with significant morbidity and high risk of malignancy. Over the years, several drugs and combinations have been tried for the treatment of submucous fib…

[A case of malignant hyperthermia: anesthesia and recovery].

We report the case of a 29-years-old woman with Menière's syndrome undergoing surgery to drain the endolymphatic sack. After anesthetic induction trismus developed but tubes were inserted without difficulty. Tachycardia began 15 min after start of balanced anesthesia with isoflurane, as arterial oxygen saturation and CO2 pressure decreased and temperature increased. These symptoms responded favorably to immediate treatment with dantrolene sodium, which was continued for 30 h in the ward for postoperative intensive care. Blood chemistry reports showed high enzyme levels that peaked at 24 h. We …

Difficult laryngoscopy/intubation: the child with mandibular hypoplasia.

The child with mandibular hypoplasia (Treacher Collins syndrome, Pierre Robin sequence, hemifacial microsomia, etc) presents the otolaryngologist and anesthesiologist with considerable problems when direct laryngoscopy and/or endotracheal intubation is attempted. In addition to the small mandible, several other features of these patients contribute to the difficult laryngoscopy: macroglossia, glossoptosis, trismus related to temporomandibular joint abnormalities, and prominent maxilla or maxillary incisors. Most of the techniques that have been described for laryngoscopy/intubation in problem …

The use of corticosteroids in the lateral sinus augmentation surgical procedure: A systematic review and meta-analysis.

BACKGROUND: The lateral maxillary sinus augmentation (MSA) procedure has good predictability in terms of the success of bone regeneration wi … there was statistically significant effect of corticosteroids on swelling, pain, wound dehiscence, trismus, and hematoma. The complication rates postoperatively were comparable between the two study groups, however slight differences existed in the incidence of active suppuration (1.7% [95% CI 0.7-3.9] Cort vs. 3.2% [2.2-4.5] No-Cort), wound dehiscence ( …

Pancreatic adenocarcinoma metastasis to the oral cavity: A rare case report and literature review.

A patient suffering from metastatic pancreatobiliary adenocarcinoma was referred to our hospital's stomatology department after a rapidly progressing swelling in the right mandibular angle, trismus and preauricular pain of 3-month duration. A mandible CT scan showed extensive bone rarefaction and a thickening of the contiguous soft tissues that involved the medial masseter and pterygoid muscle. The morphological findings and immunohistochemical profile were compatible with bone metastasis from pancreatic adenocarcinoma. Therefore, palliative treatment was proposed to the patient. Pancreatic ca…

Serotonin syndrome with escitolapram and concomitant use of cocaine: a case report.

Serotonin syndrome is a potentially life-threatening condition caused by excessive serotonergic activity in the central nervous system. It is characterized by mental status changes (eg, confusion, agitation, lethargy, coma), autonomic instability (eg, hyperthermia, tachycardia, diaphoresis, nausea, vomiting, diarrhea, dilated pupils), and neuromuscular hyperactivity (eg, myoclonus, hyperreflexia, rigidity, trismus). Serotonin syndrome classically occurs in patients receiving two or more serotonergic drugs, but it can occur with monotherapy. We report a case of a 20-year-old man who developed s…

Severe life threatening maxillofacial infection in pregnancy presented as Ludwig's angina.

BACKGROUND: Ludwig's angina is a rapidly spreading cellulitis that may produce upper airway obstruction often leading to death. There is very little published information regarding this condition in the pregnant patient. CASE: A 24-year old black female was admitted at 26 weeks gestation with tooth pain, submandibular swelling, severe trismus, and dysphagea, consistent with Ludwig's angina. Her treatment included emergent tracheostomy, incision and drainage of associated spaces, teeth extraction, and antibiotic therapy. CONCLUSIONS: During a life threatening infectious situation such as the …

[Malignant hyperthermia. Current aspects of dantrolene treatment of 2 cases of hyperthermia maligna incipiens].

During induction with volatile anaesthetic agents and succinylcholine (suxamethonium) both children showed a singular clinical symptom: trismus. In the first case(1) narcosis had been interrupted followed by clinically controlled recovery. One week later, after treatment with oral dantrolen (Dantamacrin), anaesthesia with triggerfree agents was performed without complications. In the second case (2) anaesthesia has been continued by using triggerfree agents. Both children showed significant increases of Creatin-Kinase-activities over 5000 U/l after the interrupted narcosis (1) and postoperativ…

[A case of cephalic tetanus presenting with opisthotonus].

We report a case of cephalic tetanus presenting with opisthotonus. A 49-year-old man was admitted because of repeated convulsions. The patient was a garbage truck driver known to be alcoholic, who fell down to suffer an injury of the left face two days before the onset of convulsion. Intravenous administration of diazepam and phenytoin partially relieved the convulsions. Anti-tetanus human immuno-globulin was also administered despite absence of typical clinical sign. Six hours later, however, the patient became unable to open the mouth, i.e. lockjaw developed, and the diagnosis of tetanus was

Extended therapy for acute dystonic reactions.

Dystonic reactions to neuroleptic and antiemetic medications are commonly seen in the outpatient setting. Despite initially successful treatment in the emergency department, symptoms may later recur. We have recently seen several cases of recurrent dystonic reactions; four representative cases are reported. Three of the patients experienced no further dystonic reactions when treated appropriately as outpatients. The fourth patient was lost to follow up. Based on this experience in the pharmacology of the drugs involved in producing and treating dystonic reactions, it is recommended that patien

Management of a cheek fistula associated with false ankylosis.

A 14-year-old boy presented with a Grade II left cheek fistula and inability to open the mouth following gangrenous stomatitis in early childhood. The treatment of the patient by multistaged reconstruction of the cheek defect is discussed. A large turnover flap was used for the lining, a technique which obviates the need for a second lining flap, and thus eliminates the disfigurement of a donor site. Also, it uses the healthy skin that would otherwise have been discarded in the creation of mucosal flaps.
