[CORRECTION OF METABOLIC DISTURBANCES IN PATIENTS WITH PULMONARY TUBERCULOSIS AND CONCOMITANT DISEASES].

Metabolic disturbances were corrected using the oral specialized formula Nutrien-phthisio (ZAO "Company Nutritec", Russia) in 53 patients with pulmonary tuberculosis and concomitant diseases. In the whole group, tuberculosis was first detected in 21 patients; 32 had a chronic process. Chemotherapy was discontinued in all the patients due to the intolerance phenomena caused by comorbidity (erosive gastritis, gastroduodenal peptic ulcer, hepatitis B and C, chronic pyelonephritis) in 24 patients, by adverse reactions in 19, and by a combination of both factors in 10. The criteria for objectively

[Peptic ulcer is a risk factor in the surgical treatment of pulmonary tuberculosis].

Peptic ulcer of the stomach and duodenum is the most common (10-18%) concomitant gastrointestinal pathology in phthisiosurgical patients. Treatment of these patients presents a challenge due to a drastic reduction in the efficiency of oral antibacterial therapy resulted from poor tolerability of chemical drugs and to a risk for surgical complications particularly in the early period after lung surgery. In 1996-2002, a total of 495 patients with tuberculosis at various sites were endoscopically studied; peptic ulcer in different phases was detected in 30.9% of the patients. There has been a con

Spectrum of opportunistic infections among AIDS patients in Tamil Nadu, India.

A retrospective case note review of 100 AIDS patients attending a large Indian centre was performed. Of these 100 patients, 94% gave a history of heterosexual HIV transmission, 68% were male. The majority of females were aged 21 to 30 years. The most common mode of presentation was tuberculosis (61%), both pulmonary (46%) and extrapulmonary (15%). Oral candidiasis extending on to the oesophagus was the second most predominant opportunistic infection. This study also highlights the difficulty in detecting AIDS cases in India owing to difficulties in taking a sexual history and lack of laborator

Field evaluation of candidate baits for oral delivery of BCG vaccine to European badgers, Meles meles.

The control of tuberculosis (TB) in cattle in the UK and Ireland is compromised by transmission of Mycobacterium bovis to cattle from the European badger (Meles meles), which acts as a wildlife reservoir. Vaccination of badgers could potentially contribute to TB control but the only licensed vaccine is injectable BadgerBCG which requires the live-capture of badgers. Current research is aimed at developing an oral TB vaccine (where vaccine is contained within bait) that is intended to be more cost-effective to deploy over large areas. In order to identify a lead product, candidate baits identif

New antitubercular pretomanid analogues as potent payloads in polymeric micelles: Leveraging zebrafish assays to accelerate lead optimisation.

Third-generation derivatives of the tuberculosis drug pretomanid featuring acetylene-bridged aryl-heteroaryl or heterotriaryl side chains were designed as novel agents for nanoparticle-based delivery, seeking better efficacy and safety. While all nitro compounds retained excellent activity against Mycobacterium tuberculosis, several examples in the latter class excelled as having potency superior to the original lead (7) but equivalent to or reduced lipophilicity, implying enhanced lipophilic efficiency. Initial studies suggested a similar mode of action against the related fish pathogen, M. m

The need for vigilance: melioidosis mimicking tuberculosis.

BACKGROUND: Melioidosis, caused by Burkholderia pseudomallei, is often misdiagnosed due to its clinical similarity with tuberculosis and broad-spectrum clinical manifestations. The infection is particularly prevalent in endemic areas, and a high index of suspicion is required for timely diagnosis and treatment. CASE SUMMARY: We report a 50-year-old male construction worker from central India initially diagnosed with presumptive smear-negative tubercular lymphadenitis. Despite anti-tuberculosis treatment, his condition worsened, prompting further investigation. Culture and biochemical tests id

Role of Interferon-Gamma Release Assay for the Diagnosis and Clinical Follow up in Ocular Tuberculosis.

PURPOSE: To assess the performance of interferon-gamma release assay (IGRA) associated with tuberculosis skin test (TST) for ocular tuberculosis (OTB) diagnosis and therapeutic decision making. METHOD: One hundred and ninety-one patients with ocular inflammation were prospectively followed-up. Patients with clinical signs highly suspected of OTB, TST≥10 mm, and/or IGRA≥0.35 IU/mL received antitubercular therapy (ATT). Sensitivity (Se), specificity (Sp), and area under the curve (AUC) were assessed. RESULTS: Seventy-two (37.7%) patients received ATT for presumed OTB. Combining TST and IGRA ha

[Aseptic meningoencephalitis in primary Gougerot-Sjögren's syndrome].

INTRODUCTION: Central nervous system involvement is rarely observed in primary Sjogren's syndrome (PSS) and aseptic meningoencephalitis occurs exceptionally. OBSERVATION: We report the case of a 25-year-old woman who presented with weakness of the four limbs, vision loss and concentration deficit. Neurological examination revealed a mental confusion, cerebellar involvement and spastic tetra paresis. Cerebrospinal fluid analysis revealed elevation of cell counts (162/mm3) and protein content (3 g/l). MRI showed increased T2 signals in the white matter. No evidence for an infectious etiology, e

Review of oral histoplasmosis in Malaysians.

We reviewed biopsy records for 37 cases of oral histoplasmosis for patient characteristics, clinical features, and histopathologic findings. These represented cases diagnosed in the Division of Stomatology, Institute for Medical Research, Kuala Lumpur between July 1967 and October 1994. All were male patients who ranged in age from 11 to 79 years (mean age, 56.7 years). There were 40.6% Malays, 37.8% Chinese, 18.9% Indians, and 2.7% other races. Five patients with mouth lesions as the initial presenting lesions were proven to be cases of disseminated histoplasmosis. In the remaining cases apar

Repeated pulmonary dosing of β-glucan-chitosan-PLGA nanoparticles controls Mycobacterium tuberculosis in mice.

To address limitations in tuberculosis (TB) therapy, we developed an inhalable, immunomodulating, biocompatible nanoparticle system (β-C-P) encapsulating rifampin that targets alveolar macrophage. The nanoparticle consists of a poly(lactic-co-glycolic acid) (PLGA) core, a chitosan coating, and a surface functionalized with 1,3-β-glucan for enhanced macrophage uptake and immunomodulation. We evaluated the safety, immunological effects, and efficacy of rifampin-loaded β-C-P nanoparticles delivered via oropharyngeal aspiration (OPA) in healthy mice and in a low-dose Mycobacterium tuberculosis (Mt

Exploring the role of Microbiome in Susceptibility, Treatment Response and Outcome among Tuberculosis Patients from Pakistan: study protocol for a prospective cohort study (Micro-STOP).

INTRODUCTION: Tuberculosis (TB) caused by Mycobacterium tuberculosis is a common infectious disease associated with significant morbidity and mortality, especially in low-income and middle-income countries. Successful treatment of the disease requires prolonged intake (6-8 months) of multiple antibiotics with potentially detrimental consequences on the composition and functional potential of the human microbiome. The protocol described in the current study aims to identify microbiome (oral and gut) signatures associated with TB pathogenesis, treatment response and outcome in humans. METHODS A

Utility of clinically-directed selective screening to diagnose HIV infection in hospitalized children in Bombay, India.

The increasing prevalence of HIV infection in urban India together with limited financial resources necessitates judicious HIV testing. This prospective study was undertaken to determine the utility of selective screening for HIV infection based on five clinical risk factors reported in African children. The study was conducted at the Departments of Paediatrics and Microbiology, LTMG Hospital, Bombay, India between September 1998 and 2000. The children were enrolled after taking informed consent from their parents. The HIV seroprevalence rate was determined in children (aged 1 month to 12 year

Antimycobacterial activity of rifaximin (L/105) in experimental tuberculosis in the guinea pig.

The antimycobacterial activity of rifaximin was studied in the guinea pig, after oral administration. The experiment confirmed the scanty absorption of rifaximin in the gastrointestinal tract, as the two dosages used (60 and 30 mg/kg) did not modify the tubercular illness after four months of therapy. Furthermore, sensitivity to rifaximin of the callenging Mycobacterium tuberculosis isolated at the end of the study from the spleen, lung, liver and lymph nodes of the guinea pigs that received rifaximin did not change.

Intranasal delivery of streptomycin sulfate (STRS) loaded solid lipid nanoparticles to brain and blood.

Factors like unreliable and poor oral absorption, including an active Pgp-efflux point towards a compromised oral bioavailability (BA) of streptomycin sulfate (STRS). Latter instigates its parenteral use (i.m.) only. Furthermore, its chronic use leads to serious side effects like nephrotoxicity and ototoxicity. In the present study, we propose to develop streptomycin sulfate (STRS) loaded solid lipid nanoparticles (STRS-SLNs) for non-invasive intranasal (IN) delivery. STRS-SLNs were prepared using patented nanocolloidal aqueous dispersion technique (Indian Patent application 3093/DEL/2012). Sm

Disseminated tuberculosis masquerading as a presentation of systemic lupus erythematosus.

Tuberculosis (TB) infection is the endemic in Asia-Pacific region. Miliary TB is a disseminated form which may present similarly as autoimmune conditions. Here we describe a 17-year-old girl who had miliary TB with manifestations mimicking new-onset systemic lupus erythematosus (SLE) including oral ulcers, serositis, cytopenia, proteinuria and raised autoantibody titers. Complex associations between SLE and TB are highlighted. High index of clinical suspicion for TB infection is needed upon presentations resembling immune diseases like SLE.
