Preserved cutaneous silent periods in severe entrapment neuropathies.

Noxious digital nerve stimulation during isometric contraction of hand muscles leads to transient suppression of the electromyographic activity, the so-called cutaneous silent period (CSP), which is mostly due to a spinal reflex mediated by A-delta fibers. We investigated two patients with carpal tunnel syndrome (CTS) and two patients with ulnar entrapment at the elbow (UNE), in whom routine sensory conduction studies failed to document afferent fiber continuity across the lesion site. In three patients, motor nerve conduction studies and electromyography failed to demonstrate intact efferent

Sensory nerve conduction velocities of median, ulnar and radial nerves in patients with vibration syndrome.

OBJECTIVE: The present study aimed to clarify the range of involvement for hand-arm vibration syndrome (VS) in the median, ulnar and radial nerves of the hand. METHODS: Sensory nerve conduction velocities (SCVs) for 3 nerves in the hands and arms were examined for 34 patients with VS and 23 age-matched controls. Neuropathy types were classified by possible carpal tunnel syndrome (CTS), Guyon's syndrome and digital neuropathy in three nerves. RESULTS: SCV in the median nerve (middle finger, wrist-elbow) and ulnar nerve (little finger, wrist-elbow), and amplitudes of the median nerve (wrist-pr

Exclusive electrophysiological motor involvement in carpal tunnel syndrome.

OBJECTIVE: To define the frequency of exclusive electrophysiological motor involvement in carpal tunnel syndrome (CTS). METHODS: We reviewed the electrophysiological studies of 2727 consecutive hands with typical symptoms and signs of CTS and at least one abnormal test of the following: median distal motor latency (DML), digit two sensory conduction velocity (D2-SCV), segmental D2-SCV from wrist to palm, median-ulnar sensory latency difference from ring finger stimulation. RESULTS: Thirty-one hands (1.2%) had prolonged median DML ( > 4.4 ms) with normal SCV ( > 48 m/s). In 17 of 31 hands, se

The electrodiagnosis of the carpal tunnel syndrome.

The carpal tunnel syndrome is one of the most commonly discussed subjects among the public, as well as the medical community, these days. Even though this is a rather simple nerve compression, which can be easily corrected, the carpal tunnel syndrome can induce further complications, sometimes leading to total disability of the patient. Early, proper diagnosis and treatment is very vital. Unfortunately, the exact pathomechanism and proper diagnosis is yet to be seen. As electromyographers, the authors would like to re-visit this debatable carpal tunnel syndrome emphasizing the electrodiagnosis

[Reintervention after endoscopic surgery of the carpal tunnel syndrome].

BACKGROUND: The carpal tunnel syndrome is one of the most frequent peripheric nerve compression syndromes. Almost 15 years ago, the endoscopic surgery of the carpal tunnel was introduced, in order to reduce postoperative pain and to shorten the length of the postoperative inactivity period, especially work absence. In the literature the rate of complications due to the endoscopic carpal tunnel release surgery is reported differently in respect to number and severity. This paper summarizes all reinterventions after endoscopic surgery in a single specialized hand surgery department, which prefer

Functional Results after Intramuscular Ulnar Nerve Anterior Transposition for Young Adults Patients.

Background: This study investigated the functional outcomes of intramuscular ulnar nerve transposition (IMUNT) in young adults with cubital tunnel syndrome (CuTS). Methods: This retrospective study enrolled 37 military soldiers on active duty diagnosed with and treated for CuTS to determine the compression sites, complication rate, and postoperative results. Patient outcomes were analyzed using the Disability of the Arm, Shoulder, and Hand (DASH) questionnaire and the Bishop-Kleinman rating scales. Results: Patient outcomes were analyzed after a mean follow-up duration of 26.1 (22-29) months f

Carpal Tunnel Syndrome and Ulnar Nerve Entrapment Are Associated with Impaired Psychological Health in Adults as Appraised by Their Increased Use of Psychotropic Medication.

We aimed to study psychological health, as approximated by the use of psychotropic drugs, in a population diagnosed and surgically treated for carpal tunnel syndrome (CTS) or ulnar nerve entrapment (UNE), or both, also considering the demographic and socioeconomic factors of the individuals. Linking data from five large national registers, use of psychotropics (at least one dispensation during the first year after the surgery or the baseline date) was examined in around 5.8 million people 25-80 years old residing in Sweden 2010. Among these individuals, 9728 (0.17%), 890 (0.02%) and 149 (0.00%

Bilateral thoracic outlet syndrome: An uncommon presentation of a rare condition in children.

We report an adolescent girl who had left-sided neurogenic thoracic outlet syndrome (TOS) due to impingement of the scalenus anterior muscle with bilateral changes on nerve conduction studies and responded well to surgical decompression. A 13-year-old Caucasian girl presented with intermittent pain, swelling, erythema, tingling and numbness of the palmar aspect of her left hand. Nerve conduction studies revealed bilateral ulnar sensory and motor conduction abnormalities, suggesting early compressive neuropathy in the asymptomatic arm as well. She underwent surgical exploration when it was note

Treatment of recurrent compressive neuropathy of peripheral nerves in the upper extremity with an autologous vein insulator.

The treatment of entrapment neuropathy in the upper extremity with surgical decompression has generally provided good results. Recurrence of symptoms, however, is not uncommon and its management is both challenging and difficult. Nineteen patients with recurrent carpal tunnel and cubital tunnel syndrome were treated with the vein wrapping technique using the autogenous saphenous vein. The average number of surgeries before vein wrapping was 3.3. The mean patient age was 53 years (range, 28-75 years) and the mean follow-up period was 43 months (range, 24-78 months). All patients reported reduct

[True neurological thoracic outlet syndrome].

The thoracic outlet syndrome (TOS) encompasses various clinical entities affecting the neurovascular bundle crossing the thoracic outlet. Unfortunately, this term often proves to be confusing because many of these entities have little in common beyond their known or presumed lesion site. Neurogenic TOS (true TOS) is caused by compression of the lower trunk in the brachial plexus, the cervical ribs or fibrous band. This syndrome is extremely rare. We consider that this neurological form of TOS is a clearly defined neurological syndrome. We report 10 patients with true TOS. All were females. Sta

Clinical value of 3D-FIESTA-C sequence in evaluating the arcuate ligament of the elbow joint: a retrospective study.

BACKGROUND: Cubital tunnel syndrome (CuTS) is a commonly seen form of compressive neuropathy. This study aimed to assess the clinical value of 3D-FIESTA-C for evaluating the arcuate ligament in patients with CuTS. METHODS: Between October 2018 and May 2019, 22 inpatients diagnosed with CuTS were enrolled (15 males, 7 females; mean age 60.91 ± 7.38 years). All patients underwent preoperative 3D-FIESTA-C scanning of the elbow joint and subsequently received cubital tunnel release. The accuracy, sensitivity, specificity, positive predictive value, negative predictive value, and kappa value of the

Tardy ulnar nerve palsy caused by cubitus varus deformity.

Clinical features and X-rays of thirty-four cases with cubitus varus deformity were analysed in order to explain the occurrence of tardy ulnar nerve palsy caused by cubitus varus deformity. Cubitus varus cases with tardy ulnar nerve palsy, compared to cases without it, were older at the first visit to the clinic for cubitus varus deformity. There were no other differences between the two groups in clinical features. Computed tomography showed that the olecranon moved to the ulnar side against the olecranon fossa. Operative findings showed that the medial head of triceps brachii shifted mediall

The hand complications of Colles' fractures.

Two hundred and thirty-five patients with displaced Colles' fractures were followed to union, and 209 patients to six months, specifically searching for hand pathology. The incidence of carpal tunnel syndrome was seventeen per cent at three months and twelve per cent at six months. The patients with the syndrome were significantly older and their fractures showed significantly greater residual dorsal angulation. Sixteen patients developed Dupuytren's disease between three and six months, when the incidence was eleven per cent. These patients were significantly older and had average fractures a

Carpal tunnel syndrome: pathophysiology and clinical neurophysiology.

Carpal tunnel syndrome (CTS) is a constellation of symptoms associated with compression of the median nerve at the wrist. The pathophysiology of CTS is not fully understood but mechanical aspects of injury within the carpal tunnel are most likely. The issues of ischemia, mechanical trauma, ectopic impulse generation, demyelination, tendonitis, elevated carpal tunnel pressure, mechanical factors, small and large fiber involvement and the variability of symptoms are presented. Documentation of neurophysiologic abnormalities in the median nerve is helpful to establish the diagnosis for CTS. There

The Correlation Between Smartphone Use and Compressive Ulnar Neuropathy at the Elbow: A Retrospective Study.

Background: Cubital Tunnel Syndrome (CuTS) is the second-most common compressive neuropathy of the upper limb, traditionally associated with prolonged elbow flexion, trauma, or anatomical constraints. With the widespread adoption of smartphones, sustained upper-limb postures have emerged as potential novel risk factors for ulnar nerve compression. This retrospective study aimed to investigate the potential correlation between smartphone use patterns and the development of CuTS. Methods: A retrospective observational study was conducted on 100 subjects recruited between 2021 and 2024, including

The role of cutaneous afferents in position sense, kinaesthesia, and motor function of the hand.

In reconstructive surgery and rehabilitation work, for decades it has been a basic problem for the author to discover which of the peripheral receptor systems provide the primary information on position, motion and degree of load (proprioception). At one time, joint receptors were accepted as the dominant source and more recently musculocutaneous receptors, whereas little attention has been given to cutaneous factors. A number of clinical observations, however, have indicated the importance of cutaneous afferents and this prompted the present study. There are no means of isolating the joint re

Ring finger sensorial conduction studies in grading carpal tunnel syndrome.

BACKGROUND: Comparing the distal sensory latencies of median and ulnar nerve over wrist-to-ring finger (RF) segment is a sensitive conduction parameter in the diagnosis of carpal tunnel syndrome (CTS). However to the best of our knowledge, there is not any research questioning whether the RF studies are useful in grading the CTS or not. OBJECTIVES: To determine whether the hands with moderate degree CTS and elicitable median nerve sensory responses over second finger and unelicitable responses over RF represent a more severe electrophysiological grade than the hands with elicitable responses
