A case of acute urinary retention secondary to a giant bladder diverticulum in a 23-year-old patient is described. Its etiopathogenesis is briefly discussed, highlighting its congenital origin which, in our view, must be based on the histological examination of the specimen.

The macroscopic and microscopic features of the urothelial response of the human urinary bladder to urethral catherisation are described. The catheter reaction is characterised by a predominantly eosinophilic inflammatory response producing, macroscopically, a papillary mucosal appearance termed polypoid cystitis. The severity of the epithelial inflammatory response correlates significantly with the duration of catheterisation. Urothelial dysplasia confined to the catheter reaction site was noted in 6% of cases. The possible implications of these findings are discussed.

Verrucous carcinoma is a variant of well differentiated squamous cell carcinoma that rarely affects the bladder. The bladder localization of this carcinoma is usually associated with urinary schistosomiasis. In this work we report on a rare case of verrucous carcinoma of the bladder not associated with urinary schistosomiasis. To complete this study, analysis of DNA was carried out on the histologic sections of the tumour.

A case of primary signet ring cell carcinoma of the urinary bladder is reported. A 52-year-old man was admitted with the complaint of gross hematuria. Cystoscopy showed non-papillary tumor on the right lateral wall. Abdominal CT, Ga scintigraphy, upper gastrointestinal series and barium enema, revealed no signs of a tumor other than the bladder carcinoma. The routine hematologic and chemistry profiles showed no abnormalities except for the serum level of carcinoembryonic antigen elevated to 210 ng/ml. Over a two years period, the model AS 800 artificial urinary sphincter was implanted in 10 children (mean 14 years old) with incontinence due to a neurogenic bladder (7 spina bifida; 3 sacral agenesis). Goal of continence was achieved in 100% of 7 boys and 3 girls with a follow up between 6-32 months (mean 17 months). Ten patients with primary, secondary and concurrent Tis were treated by intravesically instilled BCG-solution. A complete tumor remission could be obtained in 6 patients. Local toxicity was the reason for discontinuing the treatment in 2 patients. One patient developed a granulomatous prostatitis during the treatment. Although significant side effects did occur we think that topical BCG therapy is an effective treatment modality in Tis of the urinary bladder.

A 30 year old woman presented with symptoms of bladder stone. At operation, in addition to a big bladder stone, a fistula from the bladder to an ovarian dermoid tumor, was found. The cause of the fistula formation remains obscure.

A rare vesical carcinoma, arising in a 47-year-old male, is described. The tumor developed from the left ureteral orifice and posterior urethra, showing a nonpapillary appearance with a wide stalk. By light microscopy, the carcinoma was found to be mainly composed of signet ring cells and associated with small foci of transitional cell carcinoma. The previously reported cases of signet ring cell carcinoma of the urinary bladder were reviewed with a special reference to the manifold potentiality of the urothelium.

An analysis of the effects of natural (vitamin A) and synthetic retinoids on experimental carcinogenesis of the urinary bladder indicates that inhibitory activity is carcinogen- or carcinogen-class-specific. These findings suggest that there may be more than one pathway for the neo-differentiation of urothelial cells to frank carcinoma and that these pathways may be related to both carcinogen class and dietary vitamin A status.

Perforation of the urinary bladder associated with long-term indwelling catheter drainage is a rare and lethal iatrogenic disorder. Moreover, bladder perforation can occur in a variety of surgical settings. We report here several unusual situations: one in which a fibroid uterus probably played a role in pressure necrosis of a bladder with an indwelling catheter, one in which carcinoma of the prostate and faulty catheter drainage was present, and one in which pelvic radiation therapy was followed by bladder perforation. This is the case of an Indian woman who was hospitalized after a diagnosis of chronic inversion of the uterus with a vesicovaginal fistula. She suffered from urinary incontinence for 40 years, the condition having developed following a difficult labor. Ultimately, she proved to have a complete inversion of the bladder through a vesicovaginal fistula.

Variable epithelial hyperplasia was observed in urinary bladder of nine capuchin monkeys (Cebus apella) when examined at cystotomy 94 to 164 weeks after infection with Schistosoma haematobium. These hosts were followed for 24 to 136 weeks postcystotomy to determine the status of bladder lesions in relation to duration of infection and to ascertain whether lesion samples removed at cystotomy reestablished themselves in autologous and heterologous transfers. There was involution of urothelial hyperplasia in eight of nine animals and no evidence for establishment of transplanted bladder lesions.
