Physical predictors of infection in patients treated with clean intermittent catheterization: a prospective 7-year study.

OBJECTIVE: To study urinary tract complications in patients treated using long-term clean intermittent catheterization (CIC), and to evaluate the physical factors which might be implicated in these complications. PATIENTS AND METHODS: During 1995, 170 patients (84 men and 86 women, mean age 56.9 years) with a mean duration of use of CIC for 8.8 years were assessed for complications related to CIC. The same patients had participated in a thorough follow-up study during 1988-9 and were examined again using questionnaires and the analysis of urine samples. CIC was practised by two-thirds of the

Recurrent Ileal Conduit Obstruction Secondary to Parastomal Hernia Resulting in Bilateral Hydronephrosis and Pyelonephritis.

Parastomal hernia is a recognized long-term complication following ileal conduit urinary diversion, though associated urinary obstruction is not very common. We report the case of a 63-year-old woman with an extensive history of abdominal and urological surgery who presented with bowel obstruction symptoms, reduced conduit output, and acute kidney injury. Cross-sectional imaging demonstrated bilateral hydronephrosis and hydroureter along with bilateral pyelonephritis secondary to extrinsic compression of the ileal conduit by a parastomal hernia containing the cecum. Intraconduit catheterizatio

Interleukin-18 and Gelsolin Are Associated with Acute Kidney Disease after Cardiac Catheterization.

Patients undergoing cardiac catheterization are at high risk of post-procedure acute kidney injury (AKI) and may experience persistent renal damage after an initial insult, a state known as acute kidney disease (AKD). However, the association between AKD and urinary renal biomarkers has not yet been evaluated in this population. We enrolled 94 patients who underwent elective cardiac catheterization to investigate patterns of urinary renal biomarkers and their associations with post-procedure AKD. Serial urinary renal biomarker levels were measured during pre-procedure, early post-procedure (12

[Bacteriuria in patients with indwelling urinary catheters-when and how to treat? : Diagnosis, prevention and treatment of catheter-associated urinary tract infections].

Catheter-associated urinary tract infection (CAUTI) represents one of the most common healthcare-associated infections. In patients with indwelling urinary catheters, bacterial colonization frequently occurs in nearly all patients and increases with the duration of catheterization. A crucial clinical challenge is the differentiation between catheter-associated asymptomatic bacteriuria (CA-ABU) and symptomatic local or systemic catheter-associated urinary tract infection (L- or S‑CAUTI). While CA-ABU in general does not require treatment, CAUTIs should be treated appropriately. The pathogenesis

Significance of histological prostatitis in patients with urinary retention and underlying benign prostatic hyperplasia or adenocarcinoma of the prostate.

OBJECTIVE: To compare the clinical features of patients having urinary retention and benign prostatic hyperplasia (BPH) with those having adenocarcinoma of the prostate (ACP) and to evaluate the significance of histological prostatitis. PATIENTS AND METHODS: The clinical data and histopathology reports of patients with retention admitted to Tygerberg Hospital between September 1998 and June 2007 were evaluated. Statistical analysis was performed with Student's t-test, Mann-Whitney test and Fisher's exact test where appropriate and P < 0.05 was considered to indicate statistical significance.

Impact of urinary catheters including reusable catheters on quality of life and work productivity in persons with spinal cord lesion.

While preventing renal damage and symptomatic urinary tract infection are of paramount importance to persons with a spinal cord lesion (SCL), patient-reported outcomes (PROs) are receiving much more attention than ever before. We performed a cross-sectional internet survey of 282 adult members of SCL patient advocacy groups and investigated, for the first time, the impact of different types of urinary catheter, including a Japanese reusable silicone catheter and a reusable intermittent balloon catheter, on PROs of persons with SCL. Intermittent self-catheterization (ISC) did not surpass contin

Acute urinary retention rates following early removal or no placement in colon and rectal surgery: a single-center analysis.

BACKGROUND: The adequate duration of urinary drainage following colorectal surgery remains debated. The purpose of this study was to compare acute urinary retention (AUR) rates among various durations of urinary catheterization following colon and rectal surgery. METHODS: We conducted a retrospective analysis of patients undergoing elective colorectal resection enrolled in the Enhanced Recovery After Surgery (ERAS) protocol from 2018 to 2019. Patients were placed into four groups: no catheter placement (NC), catheter removed immediately after surgery (CRAS), removal less than 24 h (CR < 24), …

Impact of intermittent catheterization on the quality of life of multiple sclerosis patients.

PURPOSE: Lower urinary tract dysfunction is common in multiple sclerosis (MS). The purpose of this study was to prospectively evaluate the impact of intermittent catheterization (IC) on the quality of life of patients affected by MS. METHODS: Between 2007 and 2009, we admitted 23 patients to teach them the technique of IC. Their quality of life was evaluated before and more than 6 months after the beginning of learning the technique, when the urinary situation was stable. Two questionnaires were used: one specific for urinary disorders (QUALIVEEN(®)) and one general (SF-36(®)). RESULTS: Twen

The effects of early removal of indwelling urinary catheter after radical hysterectomy.

Radical hysterectomy has long been a primary mode of therapy for selected gynecologic malignancies. The lower urinary tract is an area associated with complications following this procedure. Lack of satisfactory reflex micturition and urinary retention, diminished bladder sensation, infection, and fistula formation are common adverse sequelae. Prolonged indwelling catheterization is a cornerstone of postoperative management after radical hysterectomy. An alternative regimen consisting of early postoperative catheter removal, with a strict voiding schedule, and intermittent self-catheterization

Foley Catheter and Islamic Rulings on Purification: A Clinical-Fiqh Review and Patient-Community Survey.

BACKGROUND & OBJECTIVE: Foley and suprapubic catheters are commonly used for postoperative urinary drainage and intake-output monitoring. For practicing Muslims, continuous urinary drainage raises important concerns regarding ritual purification (wuḍū), prayer validity, and recitation of the Qur'an. Our objective was to review Islamic jurisprudential (fiqh) rulings related to catheterized patients and to assess awareness, concerns, and acceptance of these rulings among patients and the wider community, with the aim of providing practical clinical guidance. METHODOLOGY: This hospital based mix

Voiding dysfunction.

Voiding dysfunction in women results from either a failure of the bladder to contract appropriately or a failure of the urethra or bladder neck to relax during attempts at micturition. Complaints of urinary frequency and urgency most often accompany voiding dysfunction. Associated symptoms, which are not always present, include postvoid fullness, poor flow, hesitancy, and complete urinary retention. Obstructive uroflowmetry and elevated residual urine are suggestive screening tests; however, complete multichannel urodynamic evaluation is necessary to arrive at a specific diagnosis. Once a diag

Cystitis Caused by Citrobacter amalonaticus in an Older Outpatient in the Absence of Catheterization.

Citrobacter amalonaticus (C. amalonaticus) is a rarely isolated member of the Citrobacter genus and an uncommon cause of human infection. Although Citrobacter species are recognized uropathogens, the clinical significance of C. amalonaticus in urinary tract infections (UTIs) remains poorly characterized. We report a case of cystitis caused by C. amalonaticus in an 87-year-old woman who visited our outpatient clinic owing to urinary symptoms. She had previously received medication for an overactive bladder and had persistent postvoid residual urine. Urine culturing yielded only C. amalonaticus,

Using Temporary Prostatic Stents to Eliminate Bacterial Colonization in Men with Chronic Indwelling Catheters: A Pilot Study.

Background Chronic urinary catheterization is commonly associated with chronic bacterial colonization and high rates of symptomatic infection that increase morbidity and mortality. This study describes the results of replacing chronic catheters with temporary prostatic stents (TPS) to reduce bacterial colonization rates. Methods Twenty-two chronically catheterized adult male patients were enrolled. Upon removal, the indwelling urinary catheter (IUC) was cultured to identify the presence and type of bacterial flora. The IUC was replaced with a TPS. All patients had five consecutive TPS placed o…

Calcified Foley Catheter and Vesicovaginal Fistula: A Case Report.

Calcification of indwelling urinary catheters is a known complication of prolonged urinary catheterization. It is caused by the formation of a bacterial biofilm, which promotes the accumulation of calcium deposits around the balloon. These deposits can lead to recurrent urinary tract infections, bladder stones, and, more rarely, bladder wall damage that can progress in the long term to a vesicovaginal fistula. We report the case of a 56-year-old woman living in a rural area who presented with persistent vaginal urinary incontinence. The patient reported having a urinary catheter that had been …
