Total introital obliteration as a consequence of lichen sclerosus: a rare cause of urinary retention.

INTRODUCTION AND HYPOTHESIS: Lichen sclerosus is a chronic inflammatory dermatitis, with a predilection for the anogenital area. In later stages, lichen sclerosus may develop into widespread scarring, and occasionally leading to severe introital stenosis and urinary retention. Our video is aimed at presenting a case of surgical management of lichen sclerosus-related introital stenosis determining urinary retention. METHODS: An 82-year-old woman was evaluated for almost complete urinary retention, with concomitant continuous enuretic urinary leakage all day and night and recurrent urinary trac…

A Patient with SIADH, Urinary Retention, Constipation, and Bell's Palsy following a Tick Bite.

Introduction. Lyme disease is the most common vector borne disease in the USA caused by the bacterium Borrelia burgdorferi. If untreated, Lyme disease can cause a variety of secondary symptoms often difficult to interpret. Some of the rare manifestations of Lyme disease include SIADH-like syndrome, enteroparesis, and urinary retention. Case Report. A 69-year-old male presented with anorexia and constipation and was found to have hyponatremia. Several days after admission, Bell's palsy developed and he experienced urinary retention requiring catheterization. Lyme disease was confirmed on serolo…

Molecular mechanisms of voiding dysfunction in a novel mouse model of acute urinary retention.

Acute urinary retention (AUR) is a common urological emergency and affects a significant patient population. The inability to eliminate urine may lead to permanent damage to the bladder's structure and functioning. However, we know little about the underlying molecular sequelae to the urine retention. To closely mirror the potential high pressures that patients with AUR could experience, we catheterized anesthetized female mice via the urethra and filled the bladder by pumping saline (25 µL/min) into the bladder lumen to 50 cm or 80 cm water pressure. A water column with designated height (50 …

Incidence of and risk factors for postoperative urinary retention in men after carotid endarterectomy.

OBJECTIVE: The goal of this study was to determine the incidence of postoperative urinary retention (POUR) in men after carotid endarterectomy (CEA) and to identify preventable risk factors for the development of this complication. METHODS: All male patients who underwent CEA from 2014 to June 2018 were identified. Exclusions included CEA with concomitant cardiac surgery, baseline dialysis, and indwelling or straight catheterization. POUR was the primary end point, defined as inability to void requiring catheterization within 24 hours postoperatively or after removal of a preoperatively place…

Into the void: a review of postoperative urinary retention after minimally invasive gynecologic surgery.

PURPOSE OF REVIEW: Postoperative urinary retention (POUR) is a common occurrence after minimally invasive gynecologic surgery (MIGS). This review discusses the definition, incidence, evaluation, diagnosis, and treatment of POUR after MIGS. RECENT FINDINGS: Reported incidence of POUR after MIGS ranges from 0.5 to 21%. Active void trials or awaiting spontaneous void after surgery are both viable options for diagnosing POUR, but active void trials appear to be more accurate, quicker, and preferred by patients. The use of enhanced recovery after surgery pathways and minimizing postoperative opioi…

Clean Intermittent Self-Catheterization as a Treatment Modality for Urinary Retention: Perceptions of Urologists.

PURPOSE: Clean intermittent self-catheterization (CISC) is now considered the gold standard for the management of urinary retention. In the literature, several articles on patients' perspectives on CISC and adherence to this technique have been published. No studies have yet explored the points of view of professional caregivers, such as nurses and doctors. The aim of this study was to explore the opinions of urologists about CISC and to evaluate the need for dedicated nurses specialized in CISC through a self-administered questionnaire. METHODS: A questionnaire was developed to explore the o…

Undiagnosed nephrogenic diabetes insipidus as a cause of acute urinary retention in a young soldier.

We present a case of undiagnosed nephrogenic diabetes insipidus as a cause of acute urinary retention in a 21-year-old male soldier. Soldiers live in close quarters, and have a regimented lifestyle that may not allow for frequent voiding; therefore, undiagnosed nephrogenic diabetes insipidus may result in acute urinary retention.

Acute urinary retention in an adolescent as the presenting symptom of lichen sclerosus et atrophicus.

BACKGROUND: A rare case of acute urinary retention caused by labial fusion in an adolescent is described and the possible causes are discussed. CASE: A 17-year-old girl, not sexually active, presented to our emergency service for acute urinary retention. Genital examination revealed labia minora fusion from the clitoris to the vaginal fourchette; urethra, and clitoris were not visualized. Sexual abuse and trauma were excluded. The labia minora were manually separated in the operating room revealing a normal vagina and urethral meatus. Skin biopsies taken from the fused labia minora revealed L…

Urinary retention caused by a urethral cystine stone in a 10-month-old infant.

Urinary retention resulting from urolithiasis in early infancy is an extremely rare event. Cystine stone formation in those younger than 1 year is also a rare condition. We report a case of urinary retention caused by a urethral cystine stone in a 10-month-old male infant. Obstructive urolithiasis should be considered one of the reasons of decreased urine output in infants.

Acute Urinary Retention in a 47-month-old Girl Caused by the Giant Fecaloma.

We present a case of a 47-month-old female suffering from acute urinary bladder neck obstruction and bilateral hydronephrosis secondary to a fecaloma. Fecaloma is defined as an accumulation of inspissated feces in the colon or rectum giving the appearance of an abdominal mass. A fecaloma can be developed by diverse causes and the causes of the fecaloma in this case were septum reformation after the Duhamel procedure and long-term constipation. Chronic constipation is very common at outpatient clinic. However, acute urinary retention and voiding difficulty caused by fecaloma in the giant Duhame…

National incidence and outcomes of postoperative urinary retention in the Surgical Care Improvement Project.

BACKGROUND: The national incidence of postoperative urinary retention (POUR), its risk factors, and associated outcomes are not well understood. METHODS: We identified patients undergoing one of the Surgical Care Improvement Project surgeries using the National Inpatient Sample. By using International Classification of Diseases, 9th revision, Clinical Modification codes (ICD-9-CM), we identified POUR and the outcomes urinary tract infection, noninfectious catheter-related complications, length of stay, and posthospitalization care. Multivariable analysis identified predictors of POUR and its …

Prolonged postpartum urinary retention: the importance of asking the right questions at the right time.

The pathophysiology of postpartum urinary retention (PUR) is still unclear. The prevalence rate varies with the definition used and the literature discusses various risk factors. Bladder catheterization is the treatment of choice, but the best way to do it is not ascertained. Long-term sequelae seem to be rare. There are no widely accepted preventive measures. Our 2 cases illustrate that it is important to prevent PUR not only by avoiding the known risk factors, but also by additionally asking for signs of voiding dysfunction postpartum.

Management of postoperative urinary retention: a randomized trial of in-out versus overnight catheterization.

BACKGROUND: There has been no consensus on the best catheterization strategy for the management of postoperative urinary retention. A prospective randomized trial was undertaken to establish the best practice guidelines for the management of postoperative urinary retention. The authors also evaluated the contemporary incidence of urinary retention following different categories of general surgery and examined risk factors associated with its occurrence. METHODS: All patients who underwent elective inpatient surgery between January 2002 and June 2003 were recruited into the study. Patients who…

Acute urinary retention in the elderly: an unusual presentation of appendicitis with a high perforation risk.

BACKGROUND: Urological manifestations are rare in acute appendicitis. Although acute urinary retention (AUR) is more commonly found in elderly patients, any previous cases reported have been under the age of 30. OBJECTIVE: The aim of this study was to investigate the incidence, clinical presentation, and results of adult appendicitis patients presenting with AUR. AUR is defined as the sudden inability to urinate with obstructive symptoms. METHODS: A retrospective case note review of 480 adult patients (>/=18 years) with appendicitis, diagnosed at the Veterans General Hospital-Taipei over a 3…
