The option value of innovative treatments in the context of chronic myeloid leukemia.

OBJECTIVE: To quantify in the context of chronic myeloid leukemia (CML) the additional value patients receive when innovative treatments enable them to survive until the advent of even more effective future treatments (ie, the "option value"). STUDY DESIGN: Observational study using data from the Surveillance, Epidemiology and End Results (SEER) cancer registry comprising all US patients with CML diagnosed between 2000 and 2008 (N = 9,760). METHODS: We quantified the option value of recent breakthroughs in CML treatment by first conducting retrospective survival analyses on SEER data to asse

Palliative Care in Obstetrics and Gynecology.

Palliative care is specialized care for people with life-limiting illness; it focuses on symptom management and quality of life and ensures that a patient's care is concordant with her goals and values. Unlike end-of-life care, palliative care can be offered concurrently with disease-directed therapies, including when the goal is cure. Obstetrics and gynecology patients for whom palliative care is most appropriate include women with gynecologic cancer and women with a fetus or neonate with a potentially life-limiting illness. Integration of palliative care for these patients offers both clinic

Family discord and proxy decision makers' end-of-life treatment decisions.

BACKGROUND: Because many Americans are decisionally incapacitated at the end of life, the last treatment decisions are often made by family proxies. Family discord concerning end-of-life (EOL) care may not only exacerbate a family crisis but also influence treatment decisions. We tested the extent to which family discord would be associated with preferences for types of end-of-life care, predicting that greater discord would be associated with weaker preferences for palliative care and stronger preferences for life-prolonging care. SETTING/SUBJECTS: Sixty-eight proxy decision makers for elder

Changes in quality of life after intensive care: comparison with normal data.

The Short Form 36 was used to compare critically ill patients' premorbid quality of life with normal values and investigate any changes following 6 months convalescence. One hundred and sixty-six survivors completed the Short Form 36 at discharge from intensive care. The answers given by survivors were significantly lower than normal for all dimensions. However, 21 patients who suffered from acute life-threatening conditions were identified and their overall scores were similar to normal values. After 6 months, 95 questionnaires were returned. Patients who had suffered acute pathologies report

Cost-effectiveness of an HIV prevention intervention for mentally ill adults.

Adults with severe mental illness are at high risk for human immunodeficiency virus (HIV) infection and transmission. Small-group interventions that focus on sexual communication, condom use skills, and motivation to practice safer sex have been shown to be effective at helping mentally ill persons reduce their risk for HIV. However, the cost-effectiveness of these interventions has not been established. We evaluated the cost-effectiveness of a 9-session small-group intervention for women with mental illness recruited from community mental health clinics in Milwaukee, Wisconsin. We used standa

Distanced perspectives: AIDS, anencephaly, and AHP.

US court decisions guaranteeing life-sustaining care to anencephalic infants have been viewed with disfavor, and sometimes disbelief, by some ethicists who do not believe in the necessity of life-sustaining support for those without cognitive abilities or an independently sustainable future. The distance between these two views--one legal and inclusive, the other medical and specific--seems unbridgeable. This paper reports on a program using multicriterion decision making to define and describe persons in a way which both acknowledges the differences perceived by many as well as those commonal

In the Superior Court of Fulton County, State of Georgia.

In sum, it is the decision of this Court that the hospital not deescalate Jane Doe's treatment or enforce any DNR order unless both parents agree to this final course of treatment. Scottish Rite, its physicians, staff, agents, and employees are enjoined from taking any action inconsistent with this order. This Court hopes that the ordeal suffered by all concerned in having to resort to the courts for direction will soon be alleviated by judicial precedent or legislative enactment establishing proper rules and procedures for all involved in a future dilemma such as this one. Childhood Socioeconomic Status and the Occurrence of Recent Negative Life Events as Predictors of Circulating and Stimulated Levels of Interleukin-6.

OBJECTIVES: Evidence supports an inverse association of childhood socioeconomic status (SES) with systemic inflammation in adulthood. However, it remains to be determined whether this association is dependent on exposure to stressful life experiences. METHODS: We predicted that the combination of a high number of recent negative life events and low childhood SES would be associated with the highest levels of both circulating interleukin (IL)-6 and lipopolysaccharide-stimulated production of IL-6. We tested this prediction among a community sample of 459 adults (47% male, mean [standard deviat

Quality of life in children with psoriasis: results from a monocentric study.

BACKGROUND: Psoriasis is a chronic inflammatory disease that can impair quality of life and psychological status not only in adult patients, but also in children. Few data about quality of life in children with psoriasis are available in literature. The aim of this study was to investigate the impact on the quality of life in children with psoriasis and the relationship with severity, localization, age at onset, duration and family history of psoriasis. METHODS: Health-related quality of life was assessed in 110 children and adolescents with psoriasis using a validated Italian version of the

A Matter of Taste? Quality of Life in Day-to-Day Living with ALS and a Feeding Tube.

Although people often refer to quality of life and there is a respectable research tradition to establish it, the meaning of the term is unclear. In this article we qualitatively study an intervention of which the quantitative effects are documented as indecisive. We do this in order to learn more about what the meaning of the term quality of life means when it is studied in daily life. With the help of these findings we reflect on the intricacies of objectifying and measuring quality of life using quantitative research designs. Our case is the feeding tube for patients suffering from ALS, a s

Kinetic analyses of changes in serum TSH receptor antibody values after total thyroidectomy in patients with Graves' disease.

We often recommend total thyroidectomy for patients with Graves' disease who wish to have a child in the near future in order to prevent fetal or neonatal hyperthyroidism, especially if the patients' serum thyrotropin receptor antibody (TRAb) values are high. The aim of this study was to analyze changes in serum TRAb values using a quantitative third-generation assay after total thyroidectomy and the half-lives of serum TRAb values to estimate the postoperative time needed to achieve the safe TRAb value for mothers. We retrospectively examined the records of 45 Graves' disease patients who und

Development and validation of a new questionnaire for the assessment of patients with diabetic foot disease: The Diabetic Foot Questionnaire (DiaFootQ).

BACKGROUND: The epidemiology data and global burden of diabetic foot disease underscores the need for effective prevention strategies, which requires an early diagnosis. Patient-reported outcome measures are instruments based on a simple format, which favours their application. Currently, there is an absence of instruments with a broad enough scope to capture the diverse aspects involved in diabetic foot disease. OBJECTIVES: To develop a questionnaire for the assessment of patients with diabetic foot disease and carry out an analysis of its validity and reliability. METHODS: The study was de

[Characteristics of Clinical Ethical Issues in Neurodegenerative Diseases and Shared Decision Making].

In neurological care, ethical issues often arise because of cognitive decline and prognostic uncertainty. These conflicts typically stem from competing values among the four principles of biomedical ethics. To address these issues, practicing shared decision-making (SDM)-which respects the patient's subjective quality of life (QOL) and personal values-is essential. Clinicians must recognize the ambiguity of "life-sustaining treatment" in this context and avoid hasty conclusions. Instead, they should strive to achieve the best possible consensus through continuous dialogue.
