Management of coronary artery fistulae. Patient selection and results of transcatheter closure.

OBJECTIVES: We report short-term findings in 33 patients after transcatheter closure (TCC) of coronary artery fistulae (CAF) and compare our results with those reported in the recent transcatheter and surgical literature. BACKGROUND: Transcatheter closure of CAF has been advocated as a minimally invasive alternative to surgery. METHODS: We reviewed all patients presenting with significant CAF between January 1988 and August 2000. Those with additional complex cardiac disease requiring surgical management were excluded. RESULTS: Of 39 patients considered for TCC, occlusion devices were place

A device-related fistula between coronary artery and left atrial appendage following left atrial appendage closure: Case presentation.

INTRODUCTION: Left atrial appendage (LAA) closure (LAAC) is considered a viable alternative to anticoagulation therapy for stroke prevention in nonvalvular atrial fibrillation, we report a case with a less common shunt resulting from a device-related coronary artery-appendage fistula (CAAF) following LAAC. METHODS AND RESULTS: A 67-year-old male with a history of LAAC was referred to our emergency room with recurrent chest pain and palpitations and was diagnosed with ischemic angina pectoris. Subsequent coronary angiography (CAG) revealed 70% in-stent restenosis and an abnormal shunt of contr

Elusive pulmonary venous confluence-to-hepatic vein connection leading to heart failure.

A persistent omphalomesenteric vein (POMV) maintaining a link between the pulmonary venous system and the infracardiac hepatic venous system can lead to significant left-to-right shunting. To the best of the authors' knowledge, POMV causing significant left-to-right shunting has been unreported previously, especially in association with type B interrupted aortic arch. This report describes this association in a child who had chronic heart failure even after successful repair of interrupted aortic arch and ventricular septal defect. Subsequent percutaneous device closure of the POMV using an Am

Outcomes of single versus double Perclose suture device use after transfemoral transcatheter procedures using large-bore arterial access: A meta-analysis and systematic review.

BACKGROUND: In large-bore transfemoral procedures such as transcatheter aortic valve replacement and endovascular aneurysm repair, the comparative effectiveness of single (SP) versus the default double Perclose (DP) strategies for arterial closure remains uncertain. METHODS: We performed a systematic literature search in PubMed, Scopus, Web of Science, and the Cochrane Library from January 2014 to April 2026 to identify studies comparing SP and DP devices for large-bore transfemoral arterial access closure. Outcomes included device/technical success, need for unplanned surgical or catheter-ba

Impending aortic aneurysm rupture - a case report and review of the warning signs.

BACKGROUND: Bed rest duration following deployment of a vascular closure device after transfemoral left-sided cardiac catheterization is not standardized. Despite research supporting reduced bed rest, many hospitals require prolonged bed rest. Delayed ambulation is associated with back pain, urine retention, difficulty eating, and longer stay. OBJECTIVE: To study length of stay, safety, and opportunity cost savings of reduced bed rest at a large urban hospital. METHODS: A single-site 12-week study of 1-hour bed rest after transfemoral cardiac catheterizations using vascular closure devices. …

Direct Carotid Puncture Mechanical Thrombectomy in Medium Vessel Occlusion (MEVO) Stroke Using Obtura Closure Device for Hemostasis.

BackgroundMechanical thrombectomy (MT) is the gold standard treatment for acute ischemic stroke (AIS) patients with large vessel occlusion. Direct cervical carotid puncture is a recognized approach in unfavorable aortic arch anatomy. However, manual compression/ surgical closure of the access site can be challenging if the International Normalized Ratio (INR) is high and the patient is taking anticoagulation.Case SummaryWe present a case of Stanford type A (STA) aortic dissection (status post Bentall procedure) presenting with acute ischemic stroke (AIS) with left M2-MCA thrombotic occlusion.

Percutaneous repair of paravalvular prosthetic regurgitation: acute and 30-day outcomes in 115 patients.

BACKGROUND: Percutaneous repair has emerged as a potential therapy for patients with prosthetic paravalvular regurgitation. However, there is a relative paucity of data on the feasibility and outcome of this procedure. METHODS AND RESULTS: All patients in whom percutaneous paravalvular regurgitation closure was attempted at our hospital were identified and included. Under echocardiographic and fluoroscopic guidance, patients underwent implantation of ≥1 an Amplatzer Septal Occluder, Duct Occluder, Muscular Ventricular Septal Defect Occluder, or Vascular Plug II. Device-Related Thrombus After Left Atrial Appendage Closure: Incidence, Predictors, and Outcomes.

BACKGROUND: In patients with atrial fibrillation, left atrial appendage closure with the Watchman device prevents thromboembolism from the left atrial appendage; however, thrombus may form on the left atrial face of the device, and then potentially embolize. Herein, we studied the incidence, predictors, and clinical outcome of device-related thrombus (DRT) using a large series of clinical trial cohorts of patients undergoing Watchman implantation. METHODS: We studied the device arms of 4 prospective Food and Drug Administration trials: PROTECT-AF (Watchman Left Atrial Appendage System for Emb

Congenital aortocaval fistula combined with patent ductus arteriosus in an infant: a case report.

Congenital aortocaval fistula (ACF) is a rare cardiac malformation. While it can occur in combination with patent ductus arteriosus (PDA), this has not been reported. In this case, a 1-year-old infant had a heart murmur found in a routine physical examination, and PDA was revealed by transthoracic echocardiography and abdominal ACF was detected by three-dimensional coronary artery computed tomography. Percutaneous interventional therapy, used for ACF and PDA, was performed to occlude the malformation. The patient presented good health without any discomfort at a 1-year follow-up. The percutane

Mitral Paravalvular Leak Closure Using a Percutaneous and Nonfluoroscopic Approach.

OBJECTIVE: Mitral paravalvular leak (PVL) is a serious complication after mechanical mitral valve replacement. We describe a step-by-step percutaneous and nonfluoroscopic (PAN) technique for mitral PVL closure guided exclusively by transesophageal echocardiography (TEE) to eliminate radiation exposure. KEY STEPS: First, preprocedural anatomical assessment and device sizing use cardiac computed tomography and TEE. Next, under TEE guidance, transseptal puncture is performed, followed by steerable sheath positioning in the left atrium. Then, a guidewire is advanced across the defect, and a deliv

Neonatal Transcatheter Closure of a Large Pulmonary Arteriovenous Fistula with a KONAR-MF™ VSD Occluder.

Pulmonary arteriovenous fistulas (PAVFs) are rare congenital anomalies characterized by abnormal vascular connections that bypass the capillary bed, causing hypoxemia, cyanosis, and systemic embolic risks. Early diagnosis and intervention are critical to prevent complications, particularly in neonates with severe right-to-left shunts. This report describes the first documented case of neonatal transcatheter closure of a large PAVF using a KONAR-MF™ VSD Occluder (MFO). A 1-day-old neonate with a prenatally diagnosed PAVF presented with oxygen saturation of 82%. Angiographic and echocardiographi
