Vasculitis encompasses a wide variety of diseases. Because diagnosis may be difficult, a careful evaluation is essential, including a detailed patient history, thorough physical examination, and appropriate laboratory studies. Diagnosis is based on clinicopathologic features that permit identification of the condition. Biopsies are often necessary to confirm a diagnosis. It is important to accurately categorize the vasculitic disorders, since prompt, aggressive therapy with potentially toxic drugs is necessary to avoid irreversible organ system dysfunction.

The diagnosis of necrotizing vasculitis rests on histology which shows parietal fibrinoid necrosis, vascular and perivascular infiltrates and intraluminal thrombosis in varying degrees of association. However, histopathological classifications of necrotizing vasculitis according to the size of the diseased vessels or the appearance of vascular infiltrates are not satisfactory, since microvasculitis may successively to simultaneously involve vessels of different calibers and result (generally in succession) in leukoclastic or lymphocytic vasculitis. For this reason we give preference to an aeti…

A 29-year-old male Haitian refugee had generalized lymphadenopathy, weight loss, and bilateral lung infiltrates diagnosed by transbronchial lung biopsy as tuberculosis. He had previously been labeled as having "pre-AIDS," which led to multiple suicide attempts. Four months later, cyanosis and gangrene of both lower extremities necessitated amputation, which revealed vasculitis.

A child with ulcerative colitis is described in whom many unusual complications of the disease occurred. Arthritis was present and preceded the onset of the colitis. The patient also developed vasculitis, hepatitis, pleuritis, pericarditis, cryoglobulinemia, and disseminated intra-vascular coagulopathy. A review of the literature on ulcerative colitis revealed the relative infrequent occurrence of many of these manifestations in the pediatric age group.

With the great interest in recent years in the use of immunosuppressive/antiinflammatory drugs for the treatment of severe systemic inflammatory disorders, it is surprising that, except in Wegener's granulomatosis, the use of methotrexate (MTX) in disseminated vasculitides has been neglected. We have treated three patients with MTX, each with a different form of severe, corticosteroid-resistant, life-threatening, disseminated vasculitis. One had a nonspecific vasculitis characterized by excruciating muscle pain, weakness, and atrophy; the second had rheumatoid vasculitis with extensive skin ul…

From own experiences the observations of 3 patients were reported who were immunosuppressively treated with purine antagonists (azathioprine and 6-mercaptopurine). Hereby in a severe allergic vasculitis in a 22-year-old patient and in a granulomatous arteritis during Wegener's granulomatosis in a 51-year-old patient remissions for more than 1 and 7 years, respectively, could be achieved under immunosuppressive long-term therapy. In the case of a periarteriitis nodosa death took place 2 months after administration of 6-mercaptopurine.

Six patients with biopsy-proven polyarteritis nodosa and chronic Australia antigenemia are described. Evidence was found in these patients for the presence of circulating immune complexes composed of Australia antigen and immunoglobulin. In addition, in two patients immunofluorescent studies localized Australia antigen, IgM, and complement in the blood vessel walls.

OBJECTIVE: Research on sleep patterns in individuals with vasculitis remains limited. This SleepVasc study aimed to 1) evaluate sleep disturbances and a range of sleep health dimensions in individuals with vasculitis, and 2) identify potential risk factors for sleep problems/disturbances/disorders.
METHODS: SleepVasc was conducted through a cross-sectional, web-based survey within the Vasculitis Patient-Powered Research Network. Participants completed these questionnaires: Epworth Sleepiness Scale, Functional Outcomes of Sleep Questionnaire-10, Multivariable Apnea Prediction Questionnaire, an…

IMPORTANCE: Hydralazine is a selectively used cardiovascular medication with case reports and case series demonstrating an association between the use of hydralazine and antineutrophil cytoplasmic antibody-associated vasculitis.
OBJECTIVE: To assess the risk of vasculitis associated with hydralazine use compared with use of an angiotensin-converting enzyme inhibitor (ACE) or angiotensin receptor blocker (ARB).
DESIGN, SETTING, AND PARTICIPANTS: This population-based, retrospective cohort study assessed adults 66 years or older from Ontario, Canada, who were newly prescribed hydralazine from …

PURPOSE OF REVIEW: Anti-neutrophil cytoplasmic antibody (ANCA)-associated vasculitis (AAV) is now recognized as a complex immune-mediated inflammatory disorder rather than a purely pauci-immune small-vessel vasculitis. Advances in understanding ANCA specificity, complement activation, neutrophil extracellular trap formation, genetic susceptibility, and environmental triggers have reshaped concepts of disease classification, prognosis, and therapy. A focused review of these developments is timely because they are directly informing current treatment strategies, disease stratification, and futur…
