An anomalous course of the left brachiocephalic vein beneath the aortic arch was identified in 7 patients undergoing surgical repair of congenital cardiac malformations. Six of these patients had the morphologic features of tetralogy of Fallot including severe obstruction to the right ventricular outflow tract and a right aortic arch. A review of the literature reveals this to be an uncommon anomaly. When it is present, however, it is frequently associated with a ventricular septal defect, obstruction of the right ventricular outflow, and aortic arch anomalies. This anomaly can be documented b

BACKGROUND: The Ross aortic valve replacement with a modified Konno-type enlargement Ross-Konno procedure of the aortic annulus and subannular region allows an autograft aortic valve replacement for children with significant annular and subannular hypoplasia. The potential for growth and the proven durability of the autograft make the Ross-Konno procedure an ideal aortic valve replacement for this subgroup with multilevel left ventricular outflow tract obstruction. We reviewed our institutional midterm experience to assess autograft and homograft hemodynamics, and management after a Ross-Konno

Tetralogy of Fallot (TOF) with unilateral absence of pulmonary artery and the anomalous coronary artery is a rare combination. Detailed preoperative evaluation of coronary artery anatomy is must to prevent injury to the major vessels crossing right ventricular outflow tract. We report a rare association of single coronary artery with left circumflex artery crossing right ventricular outflow tract close to the pulmonary annulus in tetralogy of Fallot with absent left pulmonary artery in 11-year-old girl. Though there is a great diversity of coronary anomalies in tetralogy of Fallot, the prepulm

This patient was a septuagenarian female with a past medical history of hypertrophic cardiomyopathy with systolic anterior motion and moderate mitral regurgitation. Preprocedural transoesophageal echocardiography did not show any abnormal papillary muscle. An elective alcohol septal ablation was performed. During alcohol septal ablation at the catheterization laboratory, the patient developed acute cardiogenic shock with pulmonary oedema that required intubation. Transoesophageal echocardiography showed worsening obstruction of the left ventricular outflow tract due to swelling of the septum w

Left ventricle myotomy and myectomy was carried out in nine patients with obstructive hypertrophic cardiomyopathy solely because of a previously documented episode of cardiac arrest. Before cardiac arrest, each patient had either no or only minimal functional limitation and therefore would not have met the usual criteria for operation, namely, severe symptoms unresponsive to medical therapy. Of the nine patients, one died in the perioperative period, and one died suddenly and unexpectedly 9 months postoperatively. The remaining seven patients have survived 9 months to 5.5 years after operation

BACKGROUND: Anomalies of the mitral apparatus have been shown to contribute to left ventricular outflow obstruction in patients with hypertrophic cardiomyopathy (HCM). We report our 5-year single-center experience with a shallow myectomy procedure associated with transaortic mitral valve repair in a cohort of HCM patients.
METHODS: We studied 83 consecutive patients who underwent surgical treatment of symptomatic left ventricular outflow obstruction. In all study patients, a transaortic shallow septal myectomy was performed. Fibrous or muscular structures connecting the papillary muscles to t

BACKGROUND: Obstruction to left ventricular outflow can be seen after surgical repair of congenital heart disease in which a ventricular septal defect (VSD) is closed by means of a baffle to the systemic great artery arising from the right ventricle (Rastelli operation, intraventricular repair, for conotruncal anomaly). We investigated the hypothesis that obligatory volumetric changes that occur after this operation lead to acute alterations in ventricular geometry and VSD size, resulting in subsequent subaortic stenosis in patients who were thought before operation to have a large, nonrestric

OBJECTIVE: To describe the concept of comprehensive left ventricular outflow tract reconstruction based on eliminating all obstructive mechanisms.
MATERIAL AND METHODS: The authors analyzed own experience of left ventricular outflow tract reconstruction in patients with hypertrophic cardiomyopathy (over 400 surgeries).
RESULTS: Complex left ventricular outflow tract reconstruction involves resection of interventricular septum and correction of papillary muscles, chordae tendineae and accessory muscle bundles. The described approach demonstrates high efficacy and safety and can be considered

Heart murmurs are caused by turbulent blood flow or by vibration of cardiac structures. Turbulent blood flow may originate from structural heart disease or from physiological phenomena. The aims of this study were to establish the cause of heart murmurs in apparently healthy adult cats and to determine whether a heart murmur is a reliable indicator of heart disease. In this retrospective study, we reviewed the medical records of cats in which a heart murmur was detected during physical examination by one of the authors in the period January 2008 to December 2009. Cats younger than 6 months and

BACKGROUND: Some patients with failing surgical aortic or mitral valves are anatomically unsuitable for typical valve-in-valve procedures due to threatened coronary artery or left ventricular outflow tract obstruction, respectively.
OBJECTIVES: The authors assessed the clinical and benchtop efficacy of the novel CLEVE (CLEveland Valve Electrosurgery) leaflet modification technique in patients with the previous concerns.
METHODS: Eight patients with degenerated aortic valve replacement (AVR) and 6 patients with mitral valve replacement (MVR) at high risk for obstruction of left main coronary

BACKGROUND: Left ventricular outflow tract obstruction may be unmasked after a successful aortic valve replacement (AVR) for severe aortic stenosis in the setting of asymmetrical basal septal hypertrophy (ABSH). The quantitative assessment of the obstructive potential of ABSH adjacent to a severely stenotic valve can be challenging. We reviewed our experience with patients who underwent concomitant septal myectomy at the time of AVR for severe aortic stenosis.
METHODS: During the 10-year period ending January 2009, 3,523 patients underwent AVR for the primary indication of severe aortic steno

BACKGROUND: Population-based studies have provided insight into the natural history of adult hypertrophic cardiomyopathy, but comparable information for affected children is lacking.
METHODS AND RESULTS: All Australian children who presented with primary cardiomyopathy at 0 to 10 years of age between January 1, 1987, and December 31, 1996, were enrolled in a longitudinal cohort study. A single cardiologist reviewed serial cardiac investigations on each subject. A total of 80 subjects with hypertrophic cardiomyopathy were identified. An underlying syndromal, genetic, or metabolic condition was

The relationship of the conduit to the sternum is crucial in the Rastelli operation. Right-sided conduits are more greatly affected by sternal compression than left, since the position of the right ventricular infundibulum is more anterior. A 37-year-old woman developed right ventricular outflow tract obstruction, left ventricular outflow tract obstruction, and aortic valve regurgitation secondary to infective endocarditis 15 years after Rastelli repair for double-outlet right ventricle (SDL). We enlarged the ventricular septal defect, performed intraventricular rerouting and aortic valve repl

Severe mitral regurgitation and stenosis due to failed mitral annuloplasty ring can be managed with percutaneous mitral valve in ring in high surgical risk patients. A 66-year-old male underwent coronary artery bypass surgery and mitral valve ring annuloplasty 7 years previously. He started to have shortness of breath with minimal effort in the past 2 years. Transthoracic echocardiogram revealed a new severe mitral regurgitation and severe mitral stenosis. The patient was turned down from surgery due to high surgical risk. The transcatheter mitral valve in ring implantation was decided. In thi

OBJECTIVE: Many patients with symptomatic hypertrophic cardiomyopathy have minimal left ventricular outflow tract gradients, and there is uncertainty whether their limitation is due to diastolic dysfunction or labile outflow tract obstruction. The purpose of this study was to characterize the clinical presentation and outcome of septal myectomy in patients with hypertrophic cardiomyopathy and latent obstruction.
METHODS: Among 749 patients who underwent septal myectomy, 249 had latent obstruction with minimal (0-30 mm Hg) resting gradients preoperatively. All were symptomatic and had more sev

OBJECTIVE: Surgical experiences of the reoperative double ventricular outflow tract reconstruction long after the successful repair of conotruncal anomalies were reviewed.
METHODS: Ten adult patients with conotruncal anomalies (6 females, 22.9 ± 5.5 years old) underwent the reoperative double ventricular outflow tract reconstruction. Primary diagnosis was pulmonary atresia with ventricular septal defect in 6 patients, truncus arteriosus in 3, and double-outlet right ventricle in 1. The indication for the left ventricular outflow tract reconstruction was the left ventricular dilatation and dys

Combined cases of hypertrophic obstructive cardiomyopathy (HOCM) and pulmonary arterial hypertension (PAH) are rare and have a management dilemma. Although preload is crucial in the management of HOCM, anti-PAH agents dramatically change the preload, leading to improving or worsening heart failure in patients with HOCM. We had a 74-year-old woman with Sjogren-syndrome-associated PAH. Her heart failure worsened following the initiation of anti-PAH agents due to an incremental preload on the left ventricle, whereas HOCM clinically developed following the termination of anti-PAH agents and progre

Tetralogy of Fallot is the most common cyanotic disease sharing a common anatomic basis but with varied presentation. There are very few described cases of TOF with left sided obstructive lesions. We describe a left ventricular outflow tract obstruction in a 3 year old child in association with TOF. Also we review the available literature for the association of left ventricular obstructive lesions in association with the same.

BACKGROUND Apical hypertrophic cardiomyopathy (ApHCM) is an uncommon phenotype of hypertrophic cardiomyopathy (HCM) characterized by myocardial thickening predominantly affecting the left ventricular apex, typically without significant obstruction of the left ventricular outflow tract. This report describes a 53-year-old man with chronic hypertension and obstructive sleep apnea (OSA) in whom characteristic electrocardiographic (ECG) and cardiac magnetic resonance imaging (MRI) findings established ApHCM, highlighting practical implications for differential diagnosis and follow-up.
