Pretreatment with R(+)-verapamil significantly reduces mortality and cytokine expression in murine model of septic shock.

It is well known that cytokines play an important role in the pathogenesis of sepsis and septic shock. There is evidence indicating that the membrane transporter, P-glycoprotein (P-gp), may be involved in the release of cytokines, such as IL-2, IL-4 or IFN-gamma. The aim of this study was to assess the influence of P-gp inhibitor, R(+)-verapamil, on cytokine expression in serum and tissues as well as survival rate of mice with LPS-induced septic shock. These effects were compared with the response to treatment with pentoxifylline, lisofylline, and prednisolone administered alone or after pretr…

Treatment of pregnant women with a betamimetic and verapamil increases the micronuclei frequency in umbilical cord blood lymphocytes.

In prevention of preterm labor, betamimetics are used in gynecological practice mostly combined with antiarrhythmic verapamil because of their therapeutic cardiovascular side effects. The aim of this study was to investigate the influence of a betamimetic (ritodrine hydrochloride, fenoterol or hexoprenaline) and verapamil (administered to mothers) on the frequency of micronuclei (MN) in umbilical cord blood lymphocytes of neonates, using cytokinesis-block micronucleus test. The analyzed sample included 23 babies whose mothers received the therapy and 30 control babies whose mothers received no…

Shortening of the ventricular fibrillatory intervals after administration of verapamil in a patient with Brugada syndrome and vasospastic angina.

A 43-year-old man presented with electrocardiographic findings consistent with Brugada syndrome. Though the baseline coronary angiogram was normal, intracoronary infusion of ergonovine maleate caused complete occlusion of the left anterior descending and a 99% occlusion of the proximal right coronary artery, each relieved by intracoronary isosorbide dinitrate. Double extrastimuli delivered at the right ventricular outflow tract induced ventricular fibrillation terminated by a 200-J shock. Verapamil, 10 mg IV, increased ST-segment elevation and programmed stimulation repeated after the drug ind…

[The effect of verapamil on changes in the Ca2+ content of the intracellular structures of organs during acute myocardial ischemia].

In experiments on rats with acute myocardial infarction induced by occlusion of the left coronary artery in conscious animals the protective effect of verapamil on the heart, brain, liver, and kidneys in this disease was studied. The effectiveness of the drug was judged by the changes in the 45Ca2+ content in the intracellular structures (cytosol, mitochondria, endoplasmic reticulum) of the organs under study. A single injection of 200 micrograms/kg verapamil in acute myocardial infarction reduces considerably the content of 45Ca2+ in the intracellular structures of these organs and thus preve…

Individual and combined effects of verapamil or trandolapril on attenuating hypertensive glomerulopathic changes in the stroke-prone rat.

Previous studies have demonstrated differences in the progression to glomerulosclerosis with the use of calcium channel blockers (CCB). The results with angiotensin-converting enzyme (ACE) inhibitors are more consistent. Moreover, only two studies have examined the combined effects of these drug classes on the development of glomerulosclerosis. The aim of the study presented here was to test the hypothesis that nonhypotensive doses of the combination (VT) of a nondihydropyridine CCB, verapamil (V), and an ACE-inhibitor, trandolapril (T), will slow the development of glomerulosclerosis better t…

Comparison of the electrophysiologic effects of oral sustained-release and intravenous verapamil in patients with paroxysmal supraventricular tachycardia.

The electrophysiologic effects of intravenous verapamil (0.15 mg/kg) and oral sustained-release verapamil (verapamil-SR) (240 mg once daily for 7 days) were studied in 17 patients with paroxysmal supraventricular tachycardia (SVT). Ten patients had atrioventricular (AV) nodal reentrant tachycardia and 7 had AV reciprocating tachycardia involving an accessory AV pathway. Both preparations significantly prolonged anterograde effective refractory period of the AV node and depressed the retrograde AV nodal conduction system. The sinus cycle length, and atrial and ventricular effective refractory p…

Elevation of cytosolic free calcium in cultured ciliary epithelial cells by histamine: effects of verapamil and staurosporine.

Treatment with histamine (10(-4) M) of cultured non-pigmented human ciliary epithelial cells led to a biphasic elevation of free intracellular calcium mediated by H1-receptors. The initial transient increase was due to Ca(2+)-release from intracellular calcium stores and could be blocked with a high concentration of verapamil (10(-4) M). The subsequent sustained elevation of cytoplasmic calcium caused by an influx of extracellular calcium was reduced by staurosporine (10(-7) M). We conclude that the sustained increase of cytoplasmic calcium by histamine may be partially mediated by activation …

Changes in heart rate by verapamil during carotid sinus stimulation in patients with hyperparathyroidism, pre- and postoperatively.

It has been investigated whether calcium- and verapamil-dependent sensitivities of carotis baroreceptors also exist in man. To answer this question, we pre- and postoperatively measured changes in heart rate during carotid sinus stimulation before and after intravenous administration of 5 mg verapamil in 23 patients with primary hyperparathyroidism. Findings during hypercalcemia were as expected: a more pronounced reduction of heart rate at comparatively low calcium levels. During normocalcemia, we found an opposite effect: a more pronounced reduction at relatively high calcium levels, which w…

Conversion of paroxysmal supraventricular tachycardia in a child with a slow verapamil infusion: case report and literature review.

The investigators present the case of a 12-month-old female with Down's syndrome and an endocardial cushion defect who presented acutely ill in paroxysmal supraventricular tachycardia (PSVT). Unsuccessful vagal maneuvers were followed by a slow intravenous infusion of verapamil, during which the rhythm converted. The discussion which follows highlights the many issues to be addressed in the acute management of pediatric PSVT including the varied presentations, underlying etiologies, differential diagnosis, electrocardiographic findings, electrophysiologic mechanisms, and prognosis. The literat…

Chlorpromazine, quinacrine, and verapamil as donor pretreatment in liver preservation, tested in the isolated perfused rat liver.

Rats were pretreated with a single iv dose of chlorpromazine (CPZ) 3 mg/kg, verapamil 1 mg/kg, or quinacrine 2 mg/kg. Livers were taken out and perfused with University of Wisconsin (UW) preservation solution and stored on ice for 48 h in the UW solution before reperfusion with erythrocyte-free and colloid-free Krebs-Hanseleit buffer at 38 degrees C in a nonrecirculating perfusion system for 2 h. CPZ- and quinacrine-pretreated livers produced significantly more bile than control livers and also released significantly less alanine aminotransferase into the perfusate at 30, 60, and 120 min of re…

Interference of pentobarbital and verapamil with the reactivity of middle cerebral artery of cat exposed to experimental subarachnoid hemorrhage.

1. Pentobarbital and verapamil produced concentration-dependent vasodilation in segments of middle cerebral artery of either control or exposed to experimental subarachnoid hemorrhage (SAH) cats precontracted with 5-hydroxytryptamine (5-HT, 10(-5) M) or K+ (75 mM), the potency to evoke relaxation being greater for verapamil than pentobarbital. 2. Pentobarbital (10(-4) M and 10(-3) M) or verapamil (10(-6) M) inhibited the contractions induced by 10(-5) M 5-HT or 75 mM K+ in both kinds of vessels. 3. Pentobarbital (10(-3) M) and verapamil (10(-6) M) reduced the concentration-dependent contractio…

Effect of diltiazem, verapamil and dantrolene on the contractility of isolated malignant hyperpyrexia-susceptible human skeletal muscle.

1. Diltiazem (10 mumol/L) and verapamil (10 mumol/L) inhibited the hypercontractility induced by 3% halothane and 2 mmol/L caffeine in malignant hyperpyrexia susceptible (MHS) muscle. Diltiazem also inhibited 80 mmol/L KCl contractures. 2. Like the skeletal muscle relaxant dantrolene sodium (6 mumol/L), diltiazem not only prevented but reversed the abnormal contractures induced by halothane and caffeine. 3. The effect on caffeine contractures of diltiazem and dantrolene in combination was additive. 4. The ability of diltiazem and verapamil to inhibit the hypercontractility of MHS muscle sugges…

Analysis of the in vitro pharmacologic response of renal pelvis and detrusor smooth muscle to thiphenamil, oxybutynin and verapamil.

A new computerized methodology was used to acquire and analyze the relative pharmacologic sensitivity of the renal pelvis and urinary bladder of the rabbit to pharmacologic stimulation. Comparison was made of the effects of thiphenamil, verapamil and oxybutynin on the spontaneous contractions of isolated detrusor and renal pelvis. Data collected by computer were evaluated in terms of amplitude, frequency, and tension change to varying concentration of pharmacologic stimulation. Data were analyzed using a frequency/time domain algorithm developed specifically to evaluate the contribution of the…

Use of oral verapamil in long-term treatment of neonatal, paroxysmal supraventricular tachycardia. A pharmacokinetic study.

Efficacy and pharmacokinetics of verapamil were studied in two neonates affected by supraventricular paroxysmal tachycardia, under maintenance treatment with the drug. Verapamil proved to be fully effective in suppressing arrhythmic episodes at the daily doses of 1.5 mg/kg four times a day in case 1 and of 2 mg/kg in case 2. The results of plasma half-life of the drug, calculated in a dose interval, were 3.14 hr and 2.10 hr, respectively. In patient 2, doses less than 0.95 mg/kg four times a day did not produce detectable drug plasma levels, while a further stepwise increase of dose up to 2 mg…

Treatment of paroxysmal supraventricular tachycardia in infancy with digitalis, adenosine-5'-triphosphate, and verapamil: a comparative study.

The treatment of paroxysmal supraventricular tachycardia (PSVT) in infancy with digitalis, adenosine triphosphate (ATP) and verapamil is reported. Treatment was successful in about 90% of the patients treated with ATP and verapamil and in 61--71% of the patients treated with digitalis (Lanatoside C). Verapamil terminated the tachycardia within 2 minutes of administration in most instances and ATP in less than 1 minute. Digitalis, however, took as long as 2 hours; it was therefore excluded as the drug of first choice in emergencies, and is better suited for treating patients with poor hemodynam…

Continuous Intra-arterial Infusion of Verapamil for Severe Vasospasm Treatment After Subarachnoid Hemorrhage: A Case Report.

Delayed cerebral ischemia can occur after subarachnoid hemorrhage (SAH) and is associated with the development of symptomatic vasospasm. Despite medical treatments for vasospasm, it can be refractory. Endovascular approaches, including bolus injection or continuous infusion of verapamil via in situ placement of a microcatheter, can serve as rescue therapy. A 39-year-old male patient with a history of HIV-1 infection was brought to the Emergency Department (ED) after being found unconscious at home. He presented with a Glasgow Coma Scale score of 9 (E2V1M6), left hemiparesis, and isocoric, phot…
