Use of a bioactive scaffold for the repair of bone defects in a novel reproducible vertebral body defect model.

Bone defects in vertebral bodies (VB) usually occur after the reduction of fractures or are caused by bone disease. Besides the treatment of original disease, repair of the bone defect can restore the structure of VB and improve stabilization of the spine to protect the spinal cord nerves. To aid studies of the efficacy of bioengineering techniques for repair of VB, we developed a rat model with a critical size bone defect in VB. Air-motivated burrs were used to create two sizes of bone defect (2 x 3 x 1.5 mm; 2 x 3 x 3 mm) in the anterior part of VB in 6-month-old Fischer 344 rats. Quantitati

A new distractable implant for vertebral body replacement: biomechanical testing of four implants for the thoracolumbar spine.

INTRODUCTION: Expandable titanium implants for vertebral body replacement in the thoracolumbar spine have been well established in the reconstruction of the anterior spinal column. Load transfer at the bone-implant interface remains a point of concern. The purpose of the study was to compare the performance in axial load transfer from the implant to the vertebral body in four different implants, all of them in clinical use to date. MATERIALS AND METHODS: We tested a second generation implant (Synex II) in comparison to three different expandable titanium cages: Synex I, Obelisc and X-Tenz. Tw…

Pullout strength of anterior spinal instrumentation: a product comparison of seven screws in calf vertebral bodies.

A lot of new implant devices for spine surgery are coming onto the market, in which vertebral screws play a fundamental role. The new screws … nificantly in pullout strength. Pullout strength correlated significantly with BMD (P = 0.0015) and vertebral body width/screw length (P < 0.001). The newly developed screws for spine surgery (USS II) show higher pullout strength when compared to established systems. Screw design had no significant influence on pullout force in vertebral body screws, but outer d …

Effects of fusion-bone stiffness on the mechanical behavior of the lumbar spine after vertebral body replacement.

BACKGROUND: Implants for vertebral body replacement are often inserted together with an additional stabilizing implant, e.g. an internal fixation device. During implantation bone grafts or milled bone is normally added to the anterior implant. Little is known about the stiffening effect of this fusion-bone mass on the mechanical behavior of the corresponding bone region, including the load distribution between the different parts. METHODS: A three-dimensional finite element model of the lumbar spine was created with a vertebral body replacement at L3, a paired internal fixation device between…

Osteoporotic spinal deformity: a biomechanical rationale for the clinical consequences and treatment of vertebral body compression fractures.

This review article develops a biomechanical rationale for the clinical consequences and treatment of osteoporotic vertebral body compression fracture. In patients with osteoporotic vertebral fractures and spinal deformity, altered spinal biomechanics and global spinal imbalance are important factors in the increased morbidity and mortality reported in this population. Severe spinal deformity impairs physical functioning, health, and quality of life. The spinal deformity itself, independent of pain, is a significant cause of disability. Spinal deformity is also an independent risk factor for h…

Reoperation and fusion after documented tether breakage following vertebral body tethering: a systematic review and meta-analysis.

PURPOSE: Radiographic tether breakage after vertebral body tethering (VBT) is common, but its clinical consequence remains uncertain. After a break is detected, the relevant question is not breakage incidence but the conditional probability of further surgery. We quantified patient-level reoperation and fusion-containing surgery after documented breakage. METHODS: PubMed/MEDLINE, Scopus, and Web of Science Core Collection were searched from inception through June 30, 2026. Two reviewers independently screened English-language full-text reports, extracted linked post-breakage outcomes, and adj…

Effects of intraoperative topical hemostatic agents on chest tube output and thoracic complications following vertebral body tethering.

PURPOSE: Vertebral body tethering (VBT) is an emerging treatment option for idiopathic scoliosis, but routinely requires intraoperative placement of a chest tube (CT). Topical thrombin and injectable collagen have routinely been used as a hemostatic agent, but data are lacking on its use in VBT. Thus, we aim to investigate the impact of the topical hemostatic use on CT output and duration, and the subsequent rate of clinically-significant hemothorax and effusion after VBT. METHODS: Retrospective review was conducted on patients who underwent VBT at a single institution between 2015 and 2024. …

Development of a novel machine learning model to automate vertebral column segmentation utilizing biplanar full-body imaging.

BACKGROUND CONTEXT: Degenerative scoliosis (DS) is a common spinal disorder among adults, characterized by lateral curvature of the spine. Recent advancements in biplanar full-body imaging, a low-dose and weight-bearing X-ray modality, facilitate safer and longitudinal imaging of DS patients. Quantifying spinal curvature serves as a valuable metric for assessing DS severity and informing surgical planning. However, manual annotation of vertebral structures in radiographic images is labor-intensive, necessitating specialized expertise and resulting in significant inter- and intraobserver variab
