Systematic Review on Vestibular Symptoms in Patients With Enlarged Vestibular Aqueducts.

OBJECTIVE: Enlarged vestibular aqueduct (EVA) is a congenital condition that can lead to various outcomes in pediatric patients including hearing loss and vestibular dysfunction. Our goal was to critically appraise the literature on the proportion of patients with EVA who report vestibular dysfunction, determine relevant risk factors for the development of these symptoms, and describe vestibular tests and interventions used to improve outcomes. METHODS: A systematic review was performed in accordance with the PRISMA guidelines. We queried the EMBASE, Ovid Medline, and Cochrane Library databas

Benign recurrent vestibulopathy: MRI and vestibular tests results in a series of 128 cases.

PURPOSE: Benign recurrent vestibulopathy is a recent entity, close to probable Meniere's disease and vestibular-migraine. So far, no study has systematically investigated the presence of endolymphatic hydrops of the lateral semicircular canal in benign recurrent vestibulopathy using magnetic resonance imaging. The aim of this study was to determine magnetic resonance imaging data and vestibular test results in patients with benign recurrent vestibulopathy. METHODS: 128 patients with benign recurrent vestibulopathy included since 2010 were retrospectively analyzed. Patients had magnetic resona

Opsoclonus Following Downbeat Nystagmus in Absence of Visual Fixation in Multiple System Atrophy: Modulation and Mechanisms.

We report atypical opsoclonus in a patient with multiple system atrophy and propose a mechanism based on the patterns of modulation by visual, vestibular, and saccadic and vergence stimulation. Firstly, the 6-Hz opsoclonus mostly in the vertical plane occurred only after the development of downbeat nystagmus in darkness without visual fixation. Even after a substantial build-up, visual suppression of the opsoclonus was immediate and complete. Furthermore, the latency for re-emergence of opsoclonus in darkness was greater when the duration of preceding visual fixation was longer. Secondly, the

Greater functional activation during galvanic vestibular stimulation is associated with improved postural stability: a GVS-fMRI study.

PURPOSE: Previous literature has shown that standing stability relies on the vestibular system; however, the neural correlates underlying standing stability remains unclear. This study aimed to investigate the neural correlates of standing stability using functional magnetic resonance imaging (fMRI) following galvanic vestibular stimulation (GVS). MATERIALS AND METHODS: Forty-five healthy right-handed healthy volunteers were included. Postural stability was measured using the modified Clinical Test of Sensory Interaction and Balance (mCTSIB), which measures swaying speed and area on hard and

Sexual dimorphism in vestibular function and dysfunction.

It has been recognized for some time that females appear to be overrepresented in the incidence of many vestibular disorders, and recent epidemiological studies further support this idea. While it is possible that this is due to a reporting bias, another possibility is that there are actual differences in the incidence of vestibular dysfunction between males and females. If this is true, it could be due to a sexual dimorphism in vestibular function and therefore dysfunction, possibly related to the hormonal differences between females and males, although the higher incidence of vestibular dysf

Is abnormal vestibulomotor responses related to idiopathic scoliosis onset or severity?

Results from several studies have suggested that brainstem dysfunction occurs more often in adolescent with idiopathic scoliosis compared to healthy individuals. The vestibular nuclei occupy a prominent position in the brainstem. Because the lateral vestibulospinal tract controls axial muscles, alteration in the brainstem during body growth (i.e., preadolescent and adolescent period) may translate in abnormal trunk activation and thus cause permanent spinal deformities. We conceive that vestibular dysfunction may be observed only in AIS patients with severe spine deviation. Consequently, adole

Subjective visual vertical in patients with idiopatic scoliosis.

Idiopathic scoliosis (IS) is characterized by a three-dimensional deviation of the vertebral column and its etiopathogenesis is unknown. Various factors are associated with idiopathic scoliosis, among these a prominent role has been attributed to integration of vestibular information with graviception for perception of space. Subjective visual vertical (SVV) is a sensitive sign of verticality perception. The aim of this study was to determine if SVV in adolescents with IS is different from healthy controls. Examination of SVV was performed using the bucket method. Binocular measurements of SVV

Signal processing for a vestibular neurostimulator.

An implanted vestibular neurostimulator has been developed based on commercial cochlear implant technology. It has been implanted chronically in Rhesus monkeys and the physiology of electrical stimulation of the vestibular periphery has been studied. We are currently proposing a human feasibility study of implantation of the device for the treatment of incapacitating Meniere's disease. Because no animal model of Meniere's disease exists, signal processing for such a device must be based on prior observations of human subjects who have suffered Meniere's attacks while their eye-movements could

Endolymphatic sac shunt, labyrinthectomy, and vestibular nerve section in Meniere's disease.

Medical treatment for Meniere's disease is effective in controlling vertigo for approximately 85% of patients. However, when disabling vertigo continues, surgical therapy is indicated. Several surgical approaches are performed to control the symptoms of peripheral vestibular disorders refractory to medical measures, each procedure having many technical variations. Surgery is usually reserved for patients with disabling vertigo. Here, the authors discuss surgical options for vertigo control in Meniere's disease and review the literature on outcomes of these management options. The authors discu

Vestibular syndrome: a change in internal spatial representation.

The vestibular system contributes to a wide range of functions from reflexes to spatial representation. This paper reviews behavioral, perceptive, and cognitive data that highlight the role of changes in internal spatial representation on the vestibular syndrome. Firstly, we review how visual vertical perception and postural orientation depend on multiple reference frames and multisensory integration and how reference frames are selected according to the status of the peripheral vestibular system (i.e., unilateral or bilateral hyporeflexia), the environmental constraints (i.e., sensory cues),

Vestibulo-ocular reflex.

During joint inflammation, various reactive oxygen species (ROS) are present in the surrounding tissue and joint fluid. In the laboratory, hydrogen peroxide (H2O2) is typically used to simulate inflammatory conditions, and media containing proteins and hyaluronic acid (HA) are employed to simulate joint synovial fluid. Electrochemical interactions between H2O2 and HA in the presence of a CoCrMo surface are expected, since HA molecules contain redox-active moieties. We hypothesized that any redox reactions of these moieties with ROS will mitigate the oxidizing effect of H2O2 on the CoCrMo surfa

[Evaluation of the effect of anti-motion sickness pharmaceuticals on adaptation to the model conditions of long-term rotation].

A reduced prescription of preparation P-4 (one close 1,5-2 hrs. before, the other in 2 hrs. after the test had begun) proved to have a high preventive effect in human subjects during 4-hrs. exposure in a slow-rotating room. The preparation mitigated the main motion sickness symptoms and accelerated adaptation.

Assessing vestibular function: which tests, when?

Vertigo and dizziness are common complaints encountered in clinical practice. The patient's history and a thorough otoneurological evaluation are essential for identifying the specific pathology behind the patient's complaints. If the patient reports an illusion of movement (vertigo), this most likely indicates an imbalance within the vestibular system. A sensation of rotatory movement together with a spontaneous nystagmus suggests a lesion involving the semicircular canals, while an illusion of linear movement indicates a disturbance of the otoliths. Nystagmus of central origin or caused by a

Spaceflight influences on ocular counterrolling and other neurovestibular reactions.

Exposure to extended periods of weightlessness in orbital flight has profound effects on the neurovestibular system and influences head and eye movements, postural control, and spatial orientation. The associated space motion sickness is among the earliest of the signs of adaptation to this new environment. This report both reviews the prominent neurovestibular phenomena associated with going into space and returning to earth and relates the issues to vestibular compensation and rehabilitation. New results from the Spacelab SLS-2 mission are included, showing significant reductions in postflig

Topography and reciprocal activity of cerebellar Purkinje cells in the uvula-nodulus modulated by vestibular stimulation.

In the rabbit uvula-nodulus, vestibular and optokinetic information is mapped onto parasagittal zones by climbing fibers. These zones are related functionally to different pairs of vertical semicircular canals, otolithic inputs and horizontal optokinetic inputs. Vestibular stimulation restricted to one of these zones modulates climbing fiber responses (CFRs). Within each of these zones, simple spikes (SSs) are modulated reciprocally with CFRs. In rabbits anesthetized with chloralose-urethan, we have used vestibular and optokinetic stimulation to evoke CFRs within a parasagittal zone while reco

Influence of vestibular organ stimulation on stomach movement and respiration.

Some previous investigators have reported controversial results in the stomach movement in response to labyrinthine stimulation (1). The purpose of the present study was to investigate the influence of vestibular stimulation on the stomach movement and respiration in anesthetized cats, and to investigate the differences between increased and decreased responses of the stomach movement. This study used an electrophysiological technique: a bipolar stainless-steel stimulating electrode with a tip diameter of 20 microns introduced chronically into the anterior and/or lateral semicircular canal (2)

Vestibular dysfunction in noise induced hearing loss.

It is known that loud tones can damage the cochlea, but vestibular involvement has been given less attention. However, patients with noise induced hearing loss (NIHL) sometimes have balance disorders as well as some have symptoms resembling those of Meniere's disease. In this study, we examined the relationship between loud sounds and vestibular problems. Thirty-six NIHL patients were divided into two groups according to the presence (vertigo group) or absence (non-vertigo group) of vestibular complaints. Electrocochleograms were recorded from all subjects after pure tone audiometry. A higher

Visual hallucinations evoked by caloric vestibular stimulation in normal humans.

This study describes a hitherto unknown phenomenon in healthy humans: visual hallucinations evoked by caloric vestibular stimulation. The hallucinations are of elementary type. The following quantitative and qualitative characteristics of the hallucinatory images were measured: latency, duration, location in the visual field, relation to the provoked vertigo, colour of the hallucinatory image and the background, number, size, shape, distance from the eyes, and motion. Two mechanisms are proposed to explain the genesis of these hallucinations. One mechanism is direct: change in the spontaneous
