The case was 83-year-old man who had complete situs inversus, and was pointed out to have peripheral adenocarcinoma with the size of 1.8 cm at the left upper lobe( S3). Because of severe emphysema and other multiple comorbidities, left S3 segmentectomy with hilar lymph node sampling was performed using video-assisted thoracic surgery (VATS). Preoperatively, the simulation of operation was performed using the 3 dimension computed tomography images of pulmonary arteriovenous and bronchus (3DCTAB). Postoperative course was uneventful. 3DCTAB was thought to be useful in understanding the anatomica

Ectopic cervical thymoma (ECT) is a rare tumor. We present a case of 56-year-old woman with an ECT in the anterior neck that was correctly diagnosed preoperatively. The patient had no symptoms of myasthenia gravis or other immune disorders, and the tumor was not invading any adjacent structures. We performed tumor resection and thymectomy through a transcervical approach using video-assisted thoracoscopic surgery with a multi-access single port. To our knowledge, this is a novel combined technique for the resection of an ECT.

BACKGROUND: Presently, the clinical effect of radical video-assisted thoracic surgery (VATS) is equal to that of traditional open chest surgery and has received worldwide recognition of its advantages: less trauma and faster recovery. In the attempt to further the advancement of minimally invasive surgery the operative techniques of uniportal VATS and non-intubated anesthesia have been developed. These techniques have been widely applied in hospitals around the world, however there are few reports on the combined use of these techniques: non-intubated uniportal VATS.
METHODS: We report a case

The right aortic arch (RAA) forms a vascular ring, encircling both the esophagus and trachea. We herein report a case of thoracic esophageal cancer with RAA successfully resected using video-assisted thoracoscopic surgery (VATS). A 64-year-old man who presented with a complaint of abdominal pain, was admitted to our hospital. Further examinations revealed gall stones and multiple superficial esophageal carcinomas. Three-dimensional computed tomographic images showed RAA and aortic diverticulum. The trachea and esophagus were completely encircled by the RAA, the aortic diverticulum and the pulm

Adults who have undergone surgical repair of congenital diaphragmatic hernia have a prolonged illness. They usually have severe adhesions around the intrathoracic hernial sac; therefore, the adhesion itself as well as misidentification of the hernial defect can make surgical repair difficult, even in open surgery. Here, we present the successful video-assisted thoracoscopic surgical repairs of Bochdalek and Morgagni hernias in patients with severe adhesions of the hernial sac (peritoneum) to the parietal pleura lying over the thoracic wall and diaphragm. An 18-year-old woman with a Bochdalek h

BACKGROUND: Video-assisted thoracoscopic surgery (VATS) has become the preferred minimally invasive approach for thoracic surgical procedures, with potential advantages over traditional thoracotomy. Perioperative and long-term outcomes between uni-port (U-VATS) versus multi-port (M-VATS) techniques remains under investigation.
OBJECTIVES: To compare U-VATS and M-VATS in terms of operative outcomes, complications, and oncological parameters.
DESIGN: Retrospective cohort study.
SETTING: Single tertiary referral center, King Saud University Medical City (KSUMC), Riyadh, Saudi Arabia.

PURPOSE: Thoracic paravertebral block (TPVB) provides effective analgesia for video-assisted thoracoscopic surgery (VATS). However, even when pain is well-controlled by TPVB, female patients remain at high risk for postoperative nausea and vomiting (PONV) and sleep disturbances because of non-analgesic factors, such as hormonal and central emetic pathways. We investigated whether adding a two-stage diphenhydramine regimen to TPVB-based multimodal analgesia improves early recovery quality.
METHODS: In this prospective, randomized, double-blind, placebo-controlled trial, 80 women (ASA I-III) sc

OBJECTIVE: Lung cancer remains the leading cause of cancer-related mortality worldwide. Although minimally invasive surgery continues to advance, postoperative complications (such as pleural effusion and pulmonary air leaks) remain significant challenges, prolonging the duration of chest tube placement and increasing patient burden. This study aimed to systematically evaluate the safety and clinical efficacy of porcine fibrin sealant (PFS, a biological tissue adhesive), in VATS lobectomy for lung cancer.
METHODS: We conducted a single-center, retrospective observational cohort study including

OBJECTIVE: Video-assisted thoracoscopic surgery (VATS) is associated with significant postoperative pain. Multimodal analgesia, including single-shot paravertebral blocks, is widely used but provides limited analgesic duration. Intercostal cryoanalgesia, which offers prolonged pain relief, presents a promising adjunctive option. This study aimed to assess the analgesic benefit of intercostal cryoanalgesia in VATS lung cancer surgery. The primary outcome was thoracic pain during cough 24 hours postsurgery, measured via a verbal numerical rating scale.
METHODS: In a randomized, double-blind, co

Scapular fractures are rare injuries, whereas rib fractures are more common. Scapular fractures are frequently associated with rib fractures, which may lead to significant respiratory compromise. While surgical fixation has demonstrated benefits for both scapular and rib fractures, simultaneous fixation using a single surgical approach is not widely practiced. A 54-year-old male sustained a right scapular body fracture (AO 14B1) and multiple right rib fractures following a crush injury. Due to severe respiratory pain and prolonged mechanical ventilation, surgical fixation was performed on post

INTRODUCTION: Bronchogenic cysts, arising from an aberrant bronchial primordium inclusion during the fetal period, are typically located in the mediastinum but can develop in ectopic regions. While generally asymptomatic, these cysts may become symptomatic due to infection or, rarely, hemorrhage. This report details a case of a hemorrhagic bronchogenic cyst in the supradiaphragmatic region, successfully resected using video-assisted thoracic surgery (VATS) with a confronting upside-down monitor setting.

Video-assisted thoracic surgery esophagectomy (VATS-E) may increase the risk of postoperative nausea and vomiting (PONV) because it uses a high dosage of anesthesia through a long operative duration. However, no study has examined the risk factors for PONV after VATS-E. Therefore, we investigated the risk factors for PONV to support the appropriate risk management of PONV after VATS-E. This prospective cohort study included 155 patients who underwent VATS-E at the Showa University Hospital between April 1st, 2020 and November 30th, 2022. The primary outcome was the incidence of PONV within 24

BACKGROUND: Surgeon and anesthetist share the airway in a simpler way in the resection and reconstruction phase of tracheal surgery in tubeless spontaneous-ventilation video-assisted thoracoscopic surgery (SV-VATS). Tubeless SV-VATS means stable spontaneous ventilation in the resection and reconstruction phase to anesthesiologist, and unobstructed surgical field to surgeon. What's the ideal airway management strategy during "Visual Field tubeless" SV-VATS for tracheal surgery is still an open question in the field.
METHODS: We retrospectively reviewed 33 patients without sleeve and carina res
