Type IV hiatal hernias are characterized by herniation of the stomach along with associated viscera such as the spleen, colon, small bowel, and pancreas through the esophageal hiatus. They are relatively rare, representing only about 5%-7% of all hernias, and can be associated with severe complications. We report a 71-year-old veteran wrestler who presented to our department with a type IV paraesophageal hernia containing a gastric volvulus and treated successfully with emergency operation.

This case report describes a case of a high-speed injury in a patient with preexisting spinal segmentation abnormality, leading to anterior corner avulsion fracture of T1 and oesophageal perforation. As well as being indicative of anterior ligamentous injury, bony fragments associated with hyperextension injuries may injure adjacent viscera or vessels, and clinicians should consider the possibility of clinically occult visceral or vascular damage if such a fracture is demonstrated. Conservative management of both injuries was successful in this case.

From May to June 1992, Apodemus agrarius and Apodemus speciosus were captured in Hunchun, Jilin Province. Four strains of Rickettsia tsutsugamushi were isolated from viscera of rats and trombiculid mites. At the same time, the antibody against Rickettsia tsutsugamushi was assayed in the sera of the local people and the wild rats. The positive rates were 15.2% and 16.4%, respectively. The above results showed that a natural focus of scrub typhus exists in Hunchun area.

Infants with congenital diaphragmatic hernia have significantly increased chest circumferences. This implies that intrathoracic volumes are increased as well. Forces produced by the herniated abdominal viscera seem to provide the chief impetus for this change. Other factors may also contribute, for thoracic enlargement is asymmetric and not always ipsilateral to the hernia. The contribution of an enlarged chest to respiratory insufficiency, persistence of the fetal circulation, and hyperinflation is not fully understood, but may have relevance in evaluating new approaches to therapy.

Gross and histopathologic investigations were carried out on the viscera, the brain, and the spinal cord of sheep spontaneously infected with listeriosis. It was found that with the protracted course of the disease the gross lesions were found in the various sections of the spinal cord too. Complete duplication of the uterus and cervix with a unilaterally imperforate vagina must be suspected when a unilateral pelvic mass terminates in a purpuric bulge of the lateral vaginal wall in a young woman with severely progressive dysmenorrhea. Intravenous pyelography will reveal renal agenesis ipsilateral to the imperforate vagina. The prompt and accurate diagnosis of this unusual anomaly should lead to transvaginal drainage of the retained menstrual fluids prior to irreversible damage of the pelvic viscera from chronic cryptomenorrhea.

Six children aged between 10 months and 13 years with allergic vasculitis are described. The features they had in common were necrotic skin lesions, involvement of various viscera and signs of hypersensitivity reactions. Histological changes of allergic vasculitis were present in the small vessels of the skin. This condition has features in common with both polyarteritis nodosa and with anaphylactoid purpura. Severe cases that do not respond to corticosteroids may respond to immuno-suppression with cyclophosphamide.

Status of metacercarial infections of Paragonimus westermani was observed in freshwater crabs, which were purchased at 3 markets in its peak season of 1990. All of 85 crabs were Eriocheir japonicus. No other species of Eriocheir were found. When crushed muscle and viscera was examined individually, the infection rate was 11.8%; and mean number of metacercariae was 2.1 per infected crab. Unless adequately cooked, freshwater crabs are still potential sources of human paragonimiasis.

We report two cases of duodenal atresia associated with corporal symmetry changes. The one of them presented a "situs inversus total" and the other one a Ivermak syndrome. The association between duodenal atresia and Ivermak syndrome has a very low incidence. We have not found references with respect to the association between duodenal atresia and "situs inversus totalis". The embryo-genetic hypothesis that could relate both anomalies are discussed. Perhaps are not caused by chance.
