Mortality and cardiovascular events in patients treated with homocysteine-lowering B vitamins after coronary angiography: a randomized controlled trial.

CONTEXT: Observational studies have reported associations between circulating total homocysteine concentration and risk of cardiovascular disease. Oral administration of folic acid and vitamin B(12) can lower plasma total homocysteine levels. OBJECTIVE: To assess the effect of treatment with folic acid and vitamin B(12) and the effect of treatment with vitamin B(6) as secondary prevention in patients with coronary artery disease or aortic valve stenosis. DESIGN, SETTING, AND PARTICIPANTS: Randomized, double-blind controlled trial conducted in the 2 university hospitals in western Norway in 1

Efficacy of multivitamin supplementation containing vitamins B6 and B12 and folic acid as adjunctive treatment with a cholinesterase inhibitor in Alzheimer's disease: a 26-week, randomized, double-blind, placebo-controlled study in Taiwanese patients.

BACKGROUND: Elevated serum homocysteine levels have been associated with the development of Alzheimer's dementia (AD). The combined use of a mecobalamin capsule preparation, which contains vitamin B12 0.5 mg with an active methyl base, and an over-the-counter nutritional supplement that contains folic acid 1 mg and pyridoxine hyperchloride 5 mg may be effective as a homocysteine-lowering vitamin regimen. OBJECTIVE: The aim of this study was to determine whether oral multivitamin supplementation containing vitamins B6 and B12 and folic acid would improve cognitive function and reduce serum hom

When will we eliminate folic acid-preventable spina bifida?

Mandatory vitamin B12 fortification of enriched grain products is long overdue in the United States and Canada. Fortification would help provide the 2.4 mug of synthetic vitamin B12 that the US Institute of Medicine recommends for all persons 50 years and older. The findings of Ray and colleagues in this issue suggest that B12 may also help to prevent neural tube defects. If recommendations for B12 fortification were followed, it is possible that cases of spina bifida and anencephaly would be prevented. Two hundred twenty thousand children each year acquire folic acid-preventable spina bifida

[Effect of the supplementation of vitamins B12, B6 and folic acid on homocysteine and plasmatic lipids in patients with hyperlipoproteinemic secondary type IV].

The cases of hyperlipoproteinemic secondary type IV are manifested by elevation of triglycerides, with normal or high cholesterol and lightly high homocysteine. The effect of vitamins B12, B6 and folic acid, on homocysteine and lipids, in 24 male patients, 35-68 years, with hiperlipoproteinemia secondary type IV with myocardial isquemic, and without previous treatment of hipolipemiant, was investigated. The patients were supplemented with therapeutic doses tablets of vitamin B12, 500 (microg/day); B6, (600 mg/day) and folic acid (20 mg/ day), during 120 days. Homocysteine, triglycerides, total

Dietary deprivation of B-vitamins reflected in murine splenocyte proliferation in vitro.

The effect of murine dietary deprivation of specific B-vitamins on in vitro splenocyte proliferative response in short-term tissue culture was examined in serum-free, protein-free, minimal medium. Using different media formulations, incorporation of [3H]thymidine was used to monitor the relative growth capacity of phytohemagglutinin-stimulated splenocytes from mice exposed to different diets. The results demonstrate that the growth deficit observed in vitro in the absence of specific vitamin(s) in the medium correlates only to those specific vitamin(s) eliminated from the diet. Dietary repleti

Riboflavin therapy in complex I deficiency: Two new cases of leukoencephalopathy and a systematic literature review.

OBJECTIVE: Complex I (CI) deficiency, the most common biochemical defect in pediatric mitochondrial diseases, presents with diverse phenotypes, including cardiomyopathy, myopathy, Leigh syndrome, and mitochondrial leukoencephalopathy (ML). No curative therapies exist. Riboflavin, a precursor of CI cofactors FMN and FAD, is a potential treatment, but evidence is heterogeneous and formal guidelines are lacking. METHODS: We retrospectively analyzed two patients with genetically confirmed CI deficiency due to NDUFS1 and NDUFV2 variants, treated with high-dose riboflavin with long-term clinical, b

Exclusive Breastfeeding Is Not Ensuring an Adequate Vitamin B Status in Premature Infants with Very Low Birth Weight.

Background: Exclusive breastfeeding for the first 6 months of corrected age (CA) is recommended for premature infants with very low birth weight (<1500 g) (VLBW). B vitamins are essential for normal development and growth, including DNA methylation, and we investigated whether exclusive breastfeeding for the first 6 months provides an adequate cobalamin, folate, vitamin B6, and vitamin B2 status compared to additional or exclusive formula feeding from term to 12 months CA. Methods: In recruited infants born prematurely with VLBW, levels of vitamin B12 (cobalamin), folate, vitamin B6 (pyridoxal

Methoxistasis: integrating the roles of homocysteine and folic acid in cardiovascular pathobiology.

Over the last four decades, abnormalities in the methionine-homocysteine cycle and associated folate metabolism have garnered great interest due to the reported link between hyperhomocysteinemia and human pathology, especially atherothrombotic cardiovascular disease. However, clinical trials of B-vitamin supplementation including high doses of folic acid have not demonstrated any benefit in preventing or treating cardiovascular disease. In addition to the fact that these clinical trials may have been shorter in duration than appropriate for modulating chronic disease states, it is likely that

Nicolau syndrome induced by intramuscular vitamin K in a premature newborn.

INTRODUCTION: Nicolau syndrome (NS), also known as livedo-like dermatitis or embolia cutis medicamentosa, is a very rare complication of intramuscular and intraarticular injection of various drugs. CASE REPORT: We herein report a case of NS developing approximately 2 h after the intramuscular administration of vitamin K1 in an extremely low birth weight premature newborn. To our knowledge, this patient is the youngest case suffering from such a livedoid skin necrosis and the first case of neonatal NS developing due to intramuscular administration of vitamin K1.

A critical quantum chemical and experimental study of the potentiality of direct labeling of the CN group with [(99m)Tc(CO)(3)](+) or [(186/188)Re(CO)(3)](+) in CN containing biomolecules.

INTRODUCTION: It was determined recently that [(99m)Tc(OH(2))(2)(X(-))(CO(3))(3)] could strongly bind to the CN group, allowing direct labeling of CN in vitamin B(12) despite the presence of a benzimidazole group. The aim of this paper was to perform a critical study of this potentiality, coupling quantum chemical calculations to experimental evidence. METHODS: Computational methods: Within the density functional theory calculations, the 6-31+G basis set (C, H, O, N atoms) and the LANL2DZ basis set (Tc,Re) were used. Stability calculations of the [RCNM(CO)(3)](+)) (M=Tc,Re) complexes were per

Why do homocysteine-lowering B vitamin and antioxidant E vitamin supplementations appear to be ineffective in the prevention of cardiovascular diseases?

Homocysteine has been established as a serious, independent risk factor for atherosclerosis. An elevated plasma homocysteine concentration is accompanied by increased cardiovascular risk; therefore, it can be assumed that lowering the plasma homocysteine level results in a decreased risk. Vitamin B complex (folic acid, and vitamins B6 and B12) substitution therapy decreases the plasma homocysteine level, inhibits oxidative stress, and ameliorates some biochemical and clinical parameters that indicate the progression of atherosclerosis. Vitamin E administration may also reduce atherogenesis thr…

Vitamin Supplementation in Sports: A Decade of Evidence-Based Insights.

Background: Vitamins are micronutrients involved in multiple physiological processes critical for athletic performance. Because athletes are often exposed to increased oxidative stress, higher metabolic turnover, and greater nutritional demands, which can potentially lead to deficiencies in vitamins, understanding vitamin supplementation as a function of sport discipline is of fundamental importance. Methods: This narrative review synthesizes research findings from the past decade, supplemented with earlier studies where necessary, focusing on vitamins A, C, D, E, and the B-complex vitamins. P

Vitamin supplements to Indian women using low dosage oral contraceptives.

Effect of continuous and intermittent vitamin supplements on vitamin status, serum proteins and lipids were examined in Indian women of low-income group, receiving a low-dose oral contraceptive over a period of 3-6 months. To find out non-specific time related changes, vitamin supplemented and unsupplemented control groups of non-OC users were also examined. OC treatment did not change serum chemistry significantly. The significant biochemical changes were: altered tryptophan metabolism and elevated plasma vitamin A levels. Former could be prevented by administering multivitamins containing 10
