Adrenocorticotrophin, lipotrophin and the related peptides alpha-MSH, CLIP, beta-endorphin and met-enkephalin have been measured, and characterized chromatographically in tumour extracts from seven patients with the ectopic ACTH syndrome. Four of the seven tumours contained the complete family of peptides, although the proportion of one to another varied between tumours. In addition, large molecule weight forms of ACTH and met-enkephalin were seen. The potential clinical importance of these observations is discussed.

In order to investigate whether genetic factors are involved in the response of schizophrenic patients to treatment with gamma-type endorphins, we typed 32 Dutch schizophrenic patients for the HLA-A, -B, -C and -DR antigens. Alterations in striatal dopamine receptor functions are thought to underlie the major symptoms of Tourette's syndrome (TS). In this communication, evidence is presented to suggest that the striatal dopamine receptor supersensitivity in TS may be mediated by alterations in the release and function of dopamine supersensitive factors including prolactin, estrogens, beta-endorphin, melanocyte stimulating hormone (MSH), and adrenal corticotrophic hormone (ACTH). It is suggested that treatment of TS should be directed towards rectifying these endocrine and neuropeptide imbalances.

A seventeen year old boy presented with destructive arthropathy of the lower limbs and discovertebral spaces. Past history yielded recurrent episodes of indolent fractures and progressive knee and ankle deterioration. The patient denied any pain sensation in the past and at examination. Other neurological tests were normal. Beta-endorphin level was elevated in the CSF. Response to the cold pressor test was modified after injection of Naloxone. The nosology and physiopathology of congenital insensitivity to pain are discussed.
