After stapes surgery, patients with mixed or moderate hearing loss have limited possibilities for hearing improvement. We are reporting on a patient who underwent stapedotomy bilaterally 20 years ago and had sensorineural and mixed hearing loss. Recurrent otitis externa prevented the use of hearing aids. This patient was treated bilaterally with the Vibrant Soundbridge (Med-El, Innsbruck, Austria) successively. The Schuknecht piston stapes prostheses remained in situ. The Floating Mass Transducer (FMT; Med-El) was coupled to the round window (RW) and provided good acoustic reinforcement bilate

In 1985, at a conference sponsored by the National Institutes of Health, Martin Rothenberg first described a form of nonlinear source-tract acoustic interaction mechanism by which some sopranos, singing in their high range, can use to reduce the total airflow, to allow holding the note longer, and simultaneously enrich the quality of the voice, without straining the voice. (M. Rothenberg, "Source-Tract Acoustic Interaction in the Soprano Voice and Implications for Vocal Efficiency," Fourth International Conference on Vocal Fold Physiology, New Haven, Connecticut, June 3-6, 1985.) Activation of the medial olivocochlear (MOC) efferents attenuates cochlear gain and reduces the amplitudes of mechanical, electrical, and neural cochlear outputs. The functional roles of the MOC efferents are not fully understood, especially in humans, despite postulations that they are involved in protection against acoustic trauma, facilitation of transient-sound perception, etc. Delineating the frequency tuning properties of the MOC efferents would provide critical evidence to support or refute these postulated functional roles. By utilizing spontaneous otoacoustic emissions (SOAEs), a coch

OBJECTIVE: To investigate effects of recreational noise exposure on olivocochlear efferent function.
METHODS: efferent suppression of DPOAEs and acoustic reflexes were tested in 32 young personal listening device users with normal hearing and compared with that of healthy, non-noise-exposed young adults.
RESULTS: The results showed that the efferent suppression of DPOAEs was mainly at low frequencies (0.75 and 1.0 kHz) in both groups and the efferent suppression of DPOAEs and acoustic reflexes in the noise exposure group was slightly lower than that in the control group, with no significant

Efficacy of two routine surgical methods was compared in patients with otitis media purulenta chronica having cholesteatoma and fistula. Twenty five patients were operated on the middle ear with preservation of cholesteatoma matrix fragment on fistula of the labyrinth (FL). Forty five patients were operated with plastic reconstruction of the fistula with autotissues. The acoustic and vestibular functions were studied before and after the operation, 3 years later using roentgenography of the temporal bones and high performance computed tomography of the temporal bones. As the results of surgery

Arytenoid adduction procedures involve approximation of the arytenoid cartilages with the goal of reducing posterior glottal gap size and improving voice. However, voice outcomes after arytenoid adduction are not always optimal and may be improved by precise use of suture placements, forces, and direction angles. The development of intraoperative methods of assessing optimal suture direction appears critical for achieving the best voice outcome. The goal of this study was to examine the relationship of visual classification of glottal configuration, digital measures of the glottis, acoustic an

Recognizing a change in our workload and variabilities in referral patterns, we reviewed our previous activity from 1994 to 2000 retrospectively and examined prospectively formal referrals to the Unit in 2001. Arteriovenous malformations still constitute 30% of referrals and treatments, although as a proportion of our workload, this has declined. Radiosurgery is increasingly being chosen as a first line treatment for small and moderate-sized acoustic neuromas, although referral rates vary widely, some neuroscience units referring all and others none of their acoustic neuromas at least for a ra

Auditory cortical processing of speech-like sounds was studied in 9 dyslexic and 11 normal-reading adults. Noise/square-wave sequences, mimicking transitions from a fricative consonant to a vowel, were presented binaurally once every 1.1 sec and the cortical responses were recorded with a whole-scalp neuromagnetometer. The auditory cortices of both hemispheres were less reactive to acoustical changes in dyslexics than in controls, as was evident from the weaker responses to the noise/square-wave transitions. The results demonstrate that dyslexic adults are deficient in processing acoustic chan

Formation of a Teflon granuloma may lead to progressive dysphonia and airway compromise. Excision of the granuloma by lateral laryngotomy allows preservation of the uninvolved lamina propria. A sternothyroid muscle flap or Silastic implant to medialize the vocal fold restores a straight glottal edge and optimizes voice production. Fifteen patients underwent removal of a Teflon granuloma via a lateral approach. Analysis of data revealed improved acoustic and aerodynamic parameters of voice following surgery. Indices of acoustic perturbation were reduced, and vocal pitch normalized. While the dy

This research sought to determine if activation of the amygdaloid central nucleus (ACe) modulates the activity of arousal-related neurons within the peribrachial (PB) region. Two categories of neurons were identified. Neurons of the 1st category demonstrated low spontaneous rates and responded with a burst of spikes to acoustic stimuli, characteristics similar to those reported for cholinergic ponto-geniculo-occipital (PGO) wave generator neurons. Neurons of the 2nd category demonstrated significant correlations between their spontaneous activity and the power of delta wave activity in the ele

The need for an objective assessment of voice quality can be seen in the increasing use of acoustic analysis methods for clinical diagnosis and research. The hoarseness diagram allows clinicians to objectively describe even highly disturbed or aphonic votes. Its application possibilities are illustrated in three case studies in which changes in voice quality were monitored during voice rehabilitation. The distributions of voice groups that were defined on the basis of specific pathophysiological phonation conditions were then compared for their acoustic differences. The groups comprised variou

It is well known that sensorineural hearing loss rarely occurs after middle ear surgery. Five cases of postoperative sensorineural hearing loss were reviewed. These operations amounted to 0.9% of all middle ear operations in which the first author was involved. The causes of hearing loss were believed to be labyrinthitis through the fistula in cholesteatoma case 1, and acoustic trauma from the bone-cutting burr in cases 2 and 3. In cases 4 and 5 postoperative sensorineural hearing loss occurred several months after stapes surgery and the causes of hearing loss were unknown. On the basis of the

A new technique for evaluating hypernasality using an acoustic approach is presented. In a preliminary study using this technique, nasal resonance was assessed in 17 normal subjects and 16 subjects judged to be hypernasal. Analyses of the one-third-octave power spectra revealed an increase in power level between the first and second formant, and a reduction in the power level in second and third formant regions among utterances judged to be hypernasal. Factor analysis of the perceptual ratings revealed that the consensus perception of hypernasality accounted for 71% of the total variance. An a

The authors describe the use of stereoscopic short-range magnetic resonance (MR) angiography to diagnose whether and by what means the brainstem is compressed in a case of facial spasm. The MR images were obtained on a 1.5-tesla imaging system with three-dimensional time-of-flight pulse sequence (repetition time 39 msec, echo time 9 msec). Six-source MR images, in which the internal acoustic meatuses were described, were processed using a maximum-intensity projection technique to reconstruct the MR angiograms. The internal acoustic meatuses, the posterior fossa, and the nearby arteries are sho

The possibility of using electrically induced stapedius reflexes as a means of objective hearing evaluation was investigated in the rabbit as an animal model. The contralateral stapedius reflex to acoustic and electrical stimulation was measured in anesthetized rabbits. For electrical stimulation, the middle ear was opened surgically and stimulation was applied by a monopolar electrode placed at or into the round window. Contraction of the middle ear muscle was monitored by digital sampling and averaging of the impedance changes in the contralateral ear. Acoustically evoked reflexes were recor

The stapedius reflex elicited by electrical stimulation was investigated in 21 deaf individuals supplied with an inner ear prosthesis. All patients were implanted with the Vienna cochlear implant using intra or extracochlear electrodes. Analog stimulation with sinus bursts of various durations (25, 50, 100, 300, 500 ms) and frequencies (125, 500, 1000, 2000 Hz) was used. The time course of the contralateral acoustic reflex was monitored using a fast-response impedance meter. The stapedius reflex was observed in about half of the cases. Reflex threshold was observed at a stimulation level close

The surgical anatomy of the temporal bone and the cerebellopontine angle was studied in cadavers, by performing simulated surgery via the extended middle cranial fossa approach. This approach is classified into 3 types according to extension of the drilling area of the temporal bone. In this paper, the surgical anatomy visualized by the EMCF type II and type III is described. In the EMCF type II, mastoidectomy and labyrinthectomy are performed completely through the middle cranial fossa, and in the EMCF type III permitting hearing preservation surgery, sections of the middle cranial fossa dura

While clinical studies almost universally report few speech and voice complications from uvulopalatopharyngoplasty, there is a paucity of prospective studies concerned with formal acoustic and perceptual evaluation of these patients. This study compares preoperative and postoperative recordings of 32 uvulopalatopharyngoplasty patients. Fast Fourier Transform analyses were made of both long-term spectra of a reading passage and the resonant characteristics of three vowels. Fundamental frequency and reading time were quantified. Polysomnographic studies of these patients were also compared. Fina
