Hypothesis: Human papillomavirus vaccination syndrome--small fiber neuropathy and dysautonomia could be its underlying pathogenesis.

Vaccination has been one of the most effective public health measures in the history of medicine. However, seemingly inexplicit adverse reactions have been described after the injection of the newer vaccines vs. human papillomavirus (HPV). The symptoms more often reported are chronic pain with paresthesias, headaches, fatigue, and orthostatic intolerance. Adverse reactions appear to be more frequent after HPV vaccination when compared to other type of immunizations. Different isolated cases and small series have described the development of complex regional pain syndrome (CRPS), postural ortho

The spectrum of nonmotor symptoms in early Parkinson disease.

OBJECTIVE: Nonmotor symptoms (NMS) are common in patients with established Parkinson disease (PD) but their frequency in early PD has not been extensively studied. Our aim was to determine the frequency of NMS in a cohort of patients with newly diagnosed PD. METHODS: A total of 159 patients with early PD and 99 healthy controls participated in this study. NMS were screened for using the nonmotor symptom questionnaire. Other assessments included measures of motor disability (Movement Disorders Society-revised unified Parkinson's disease rating scale [MDS-UPDRS]), disease severity (Hoehn & Yahr

Improvement of small fiber neuropathy in a sarcoidosis patient after treatment with infliximab.

We describe a patient with severe small fiber neuropathy (SFN) accompanied by autonomic involvement, who was experimentally treated with infliximab, an anti-tumour necrosis factor-alpha (TNF-alpha) therapy. Six months after this treatment was started his symptoms completely resolved. Until now they did not return. Repeated temperature threshold testing (TTT) as well as cardiovascular autonomic function test clearly improved after one year therapy. This case reveals two important issues. First, it shows that SFN seems not an irreversible disorder, even in severe cases. Second, TNF-alpha may be

High prevalence and severity of cardiac autonomic neuropathy in Vietnamese diabetic patients.

AIMS: Malnutrition is frequent in Vietnamese people and may influence cardiac autonomic neuropathy (CAN). The aim of the present study was to investigate cardiac autonomic function in healthy subjects living in Vietnam and the prevalence of CAN in Vietnamese diabetic patients. METHODS: One hundred and five diabetic patients (BMI = 19.8 +/- 0.3 kg/m(2)), 50 Type 1 and 55 Type 2, living in Hué (Vietnam) were selected and compared with 60 non-diabetic healthy Vietnamese controls (BMI = 20.8 +/- 0.2 kg/m(2)) and also European controls. CAN function was evaluated by five standardized tests: three

Effect of magnesium deficiency on autonomic circulatory regulation in conscious rats.

A close relationship between magnesium and cardiovascular function has been reported; however, the effect of magnesium deficiency on autonomic cardiovascular regulation has not been clarified. We investigated the effect of magnesium deficiency on the autonomic regulation of oscillations of the R-R interval, arterial blood pressure (BP), and renal sympathetic nerve activity (RSNA) by using the maximum entropy method in conscious rats. Its effect on baroreflex control of RSNA and heart rate were also investigated with a logistic function curve. Mean BP in magnesium-deficient rats was higher than

Regional sympathetic denervation in von Recklinghausen's disease with coronary spasm and myocarditis.

Two patients with von Recklinghausen's disease presented with coronary artery spasm, transient high concentrations of norepinephrine, catecholamine-induced myocarditis, and regional sympathetic denervation of the heart that was assessed by I-123 metaiodobenzylguanidine scintigram. It is postulated that regional sympathetic denervation of the heart may precipitate both coronary artery spasm and catecholamine-induced myocarditis in susceptible patients with von Recklinghausen's disease.

[The Kleine-Levin-Critchley-syndrome. A contribution to the solution of differential diagnosis (author's transl)].

The typical symptoms of the so-called Kleine-Levin-Critchley-Syndrome are described according to our own observation of four selected patients (one woman, three men) on the background of the literature on the subject. In contrast to the first descriptions we characterise this syndrome by the Trias: periodic hypersomnia, vegetative disturbances (especially of food intake) and psychopathologic symptoms. Especially young men in their second decade of life suffer from this syndrome. As far as women are concerned the disease is found more seldom but the authors are sure that it exists. A spontaneou

[Principal features of the pathogenesis and pathogenetic treatment of certain neurologic syndromes of the facial region].

Results of many-year studies and literary data on the clinical course, pathogenesis and pathogenetic therapy of motor, sensor, and vegetotrophic disturbances in the facial region are reviewed. A concept is substantiated, that neuralgia of the trigeminal, glossopharyngeal and vagal nerves, secondary contracture of the mimic muscles, and facial hemiatrophy are diseases with a specific pathogenesis having no analogy with pain, motor, and trophic disturbances in other parts of the body. This is reflected in the special methods of the pathogenetic therapy of the diseases.

Restricted and disrupted sleep: effects on autonomic function, neuroendocrine stress systems and stress responsivity.

Frequently disrupted and restricted sleep is a common problem for many people in our modern around-the-clock society. In this context, it is an important question how sleep loss affects the stress systems in our bodies since these systems enable us to deal with everyday challenges. Altered activity and reactivity of these systems following insufficient sleep might have serious repercussions for health and well-being. Studies on both humans and rodents have shown that sleep deprivation and sleep restriction are conditions often associated with mild, temporary increases in the activity of the ma

The psychology of the glaucoma patient.

Under physiological conditions, intraocular pressure (IOP) is controlled by the autonomic and central nervous systems. Correspondingly, nerve fibers and neurotransmitters are present in ciliary body and trabecular meshwork. IOP responds to physical as well as psychological stimuli in healthy individuals. In patients with dysregulated IOP, e.g., in those with primary open-angle glaucoma, emotional instability without a specific personality pattern could be found. Whereas the statistical association between emotional changes and glaucoma is obvious, the causal relationship remains to be clarifie

[Individual heart functioning variability under exposure to equal physical factors in patients with different vegetative system tone].

The article describes changes in heart rhythm variability (HRV) as a response to equal complex physical factors (hyperthermia of 60 degrees C with low relative humidity of 8 to 15%, air ionization, thalasso, and relaxing music). The subjects of the study were 28 individuals having no verified chronic diseases, who underwent a course of five recovery procedures using exposure to a complex of weak physical factors realized in Ionic House technology (Japan). The study found that changes in heart functioning as a response to equal physical factors depended on the initial vegetative system tone. Pa

Factors involved in cardiac autonomic neuropathy in diabetic patients.

The role cardiac autonomic neuropathy (CAN) plays in diabetes is not well known. The aim of this study was to identify the factors involved in CAN in diabetic patients. One hundred patients, 44 insulin-dependent (IDDM) and 56 non-insulin-dependent (NIDDM), were investigated, using five standard tests. Three of these tests were for parasympathetic control (cardiac response to the lying-to-standing, deep breathing, and Valsalva tests), and the other two measured sympathetic control (testing for orthostatic hypotension and evaluating heart and blood pressure response to the handgrip test). Result

Influence of breed and genotype on the onset and distribution of infectivity and disease-associated prion protein in sheep following oral infection with the bovine spongiform encephalopathy agent.

The onset and distribution of infectivity and disease-specific prion protein (PrP(d)) accumulation was studied in Romney and Suffolk sheep o … es followed by the central nervous system (CNS) and enteric nervous system (ENS) and finally by the autonomic nervous system and peripheral nervous system and other organs. Detection of infectivity closely mimicked this sequence. No PrP(d) was observed in the ENS prior to its accumulation in the CNS, suggesting that ENS involvement occurred simultaneously to that of, or f …

Cardiovascular and sweating dysfunction in patients with Holmes-Adie syndrome.

A cross-sectional study is reported in which 53 patients with Holmes-Adie syndrome have been subjected to a battery of tests of autonomic nervous function referable to the cardiovascular system, to two objective tests of sweating function, and to subjective assessment of sweating by application of quinizarin powder followed by body heating. The majority of patients were consecutive referrals; none was selected because of clinical indications of autonomic dysfunction. Eighty three per cent of these patients had at least one, 57% at least two, and 40% at least three objective test abnormalities,

Acute autonomic modulation during Jacobson's progressive muscle relaxation in normal-weight vs. overweight male recreational athletes.

Heart rate variability (HRV) is a reliable non-invasive marker of autonomic nervous system (ANS) activity and has been increasingly used to assess physiological responses to stress and recovery interventions. Although relaxation techniques such as Jacobson’s Progressive Muscle Relaxation (PMR) are widely implemented, their acute effects on HRV across individuals with different body mass index (BMI) classifications (normal, overweight, obese) remain understudied. This study aimed to analyse HRV-based autonomic modulation during a Jacobson’s progressive muscle relaxation, and to assess differenc…

Anatomy, Imaging, and Clinical Significance of the Cervicothoracic (Stellate) Ganglion.

Background/Objectives: The stellate ganglion (SG), formed by the fusion of the inferior cervical and first thoracic sympathetic ganglia in approximately 80% of individuals, plays crucial roles in cardiac innervation, pain management, and autonomic regulation. This review examines the anatomical variations, histological structure, clinical applications, and therapeutic implications of the SG and stellate ganglion block (SGB), presenting original high-resolution magnetic resonance imaging (MRI) evidence of SG visualization, an underutilized approach in autonomic nervous system research. Methods:…
