Cervical sagittal alignment in idiopathic scoliosis treated by posterior instrumentation and in situ bending.

STUDY DESIGN: Retrospective radiographical analysis of cervical and thoracolumbar sagittal alignment in young adults with idiopathic scoliosis. OBJECTIVE: To analyze cervical alignment types, the relationship between cervical and thoracolumbar alignment and the effect of posterior instrumentation. SUMMARY OF BACKGROUND DATA: Thoracic scoliosis with hypokyphosis may decrease cervical lordosis. Additional adaptive positional changes of the mobile cervical segment may exist, because sigmoid cervical patterns are observed. Sagittal alignment of the instrumented thoracolumbar spine may influence

Isolated pediatric radial head and neck fractures. A rare injury. Analysis and follow up of 19 patients.

PURPOSE: Isolated pediatric radial head and neck fractures are rare. In recent literature, their incidence is estimated to be around 1% of all fractures. High rates of complications are reported. Beside non-operative treatment, head fractures are treated with k-wires, mini-screws or polypins, whereas neck fractures are treated more and more with elastic stable intramedullary nailing (ESIN). Purpose of the study was to evaluate the operative management, complications and clinical outcomes of these injuries. METHODS: Retrospective analysis between 2002 and 2014. 19 children with isolated radial

Current concepts in management of slipped capital femoral epiphysis.

Slipped capital femoral epiphysis (SCFE) is a common hip condition that can be disabling. In this review, we provide an orientation on current trends in the clinical management of SCFE including conventional procedures and specialised surgical developments. Different methods of fixation of the epiphysis, risks of complications, and the rationale of addressing deformity, primarily or secondarily, are presented. Although improved understanding of the anatomy, vascularity and implications of residual deformity have changed management strategies, the best modality of treatment that would restore c

Posterior lumbar fusion by peek rods in degenerative spine: preliminary report on 30 cases.

PURPOSE: Pre-curved peek rods to support posterior lumbar fusion have been available in the market since 4 years. Potential advantages using this new technology are increased load sharing on the anterior column promoting interbody fusion, reduced stress on bone-screw interface decreasing the rate of screw mobilization and, in the long term, reduced incidence of adjacent level disc degeneration. METHODS: The authors retrospectively reviewed 30 cases in which posterior fusion was supported by peek rods, analyzing early complications, rate of fusion and clinical outcome. RESULTS: At an average

Nailing versus plating in humerus shaft fractures: a prospective comparative study.

There is a debate about the choice of operative intervention in humerus shaft fractures requiring surgical intervention. A prospective, comparative study of management of acute humeral shaft fractures treated by antegrade interlocking nail fixation and dynamic compression plating was undertaken over a period of three years. Twenty patients of interlocking nailing and sixteen patients of plating were included after considering the inclusion and exclusion criteria. Functional scoring criteria were used for postoperative assessment and the average follow-up period was one year. A higher rate of e

[Clinical outcome and assessment of spontaneous remodeling of slipping angle in SCFE].

38 patients, and 45 hips has undergone treatment for slipped capital femoral epiphysis between 1995 and 2000. On the basis of the measurement of the head shaft angle on X-ray examination done preoperatively, hips were classified to one of the three groups--mild, moderate, and severe slippage. All patients had undergone pinning in situ as a primary treatment. All were checked out periodically, clinically and radiologically. On the last X-ray before physeal plate closure the head shaft angle was evaluated again. Retrospectively the head neck angle was measured as well. After the average 2.5 year

Lumbosacral arthrodesis using pedicular screws and ringed rods.

Sixty-one patients who had lumbar instability and chronic low back pain or deformity from nontraumatic lumbar pathologies were studied. In all of them a posterior lumbosacral fusion with CUN (Clinic of the University of Navarre) pedicle rod fixation was used. The mean follow-up period was 36 months (range 26-46 months). The consolidation rate was evaluated according to plain and functional radiographs, and a clinical evaluation was made using an analogue pain scale. The rate of fusion was 93.5%. Neurological complications occurred in 3.3%. The incidence of screw failure was 2.3% of all the scr

[The Role of a Modular Universal Tumour and Revision System (MUTARS®) in Lower Limb Endoprosthetic Revision Surgery - Outcome Analysis of 25 Patients].

Introduction/Background: With extended life expectancy, the number of primary joint arthroplasties has also increased. Revision surgery is also more often necessary, due to aseptic or septic loosening of the prosthesis or periprosthetic fracture. Large bone defects often occur in these patients and several non-modular, conventional implants are available to handle this difficult situation. Custom made implants offer an individually designed and defect-adapted shape with perfect covering of the lesion, but may delay the operation. The Modular Universal Tumor And Revision System (MUTARS®) offers

A new method to correct rotational malalignment for closed reduction and percutaneous pinning in pediatric supracondylar humeral fractures.

Open reduction is often necessary for the treatment of supracondylar humeral fractures in children in which rotational malalignment cannot be corrected by closed means. We introduce a new closed reduction technique for the correction of this deformity using a Kirschner wire as a joystick.

[Distal intraarticular humerus fracture in the elderly: prosthesis or osteosynthesis?].

AIM: The distal intraarticular fracture of the humerus, even in elderly patients, was treated so far with internal osteosynthesis. Due to the poor bone stock, in association with a complex fracture site, the achieved results may be disappointing. The use of an elbow joint prosthesis may be a solution for these specific problems in elderly patients as long as one takes the features of the prosthesis into account. METHOD: Eleven patients with a mean age of 77 years were followed up for 12 months after implantation of an elbow joint replacement. In the other group we examined 15 patients (averag

Reverse Total Shoulder Arthroplasty for Failed Osteosynthesis in Proximal Humerus Fractures: Outcomes and Challenges.

Background Proximal humerus fractures (PHF) are among the most common humeral fractures, particularly affecting elderly individuals, often following low-energy trauma. While conservative treatment is often preferred in geriatric patients, surgical management remains indicated for displaced fractures. Reverse total shoulder arthroplasty (RTSA) has increasingly been used as a primary treatment option for complex PHF, especially Neer three and four-part patterns. However, in the setting of failed osteosynthesis, there is a lack of evidence regarding the role and outcomes of RTSA. Methods A retros

Approaches to Distal Upper-Extremity Trauma: A Comparison of Plastic, Orthopedic, and Hand Surgeons in Academic Practice.

BACKGROUND: Hand trauma call duties at university medical centers are traditionally split among plastic surgeons and orthopedic surgeons, frequently without additional fellowship training in hand and upper-extremity surgery. Differences in operative approach between these groups have never been specifically described. The University Health Consortium-Association of American Medical Colleges Faculty Practice Solutions Center database contains comprehensive, factual, billing and coding data from 90 academic medical centers in the United States and can be used to characterize the practice pattern
