Results of labyrinthectomy in the treatment of Menière's disease and delayed endolymphatic hydrops.

Transmastoid total osseous labyrinthectomy for incapacitating unilateral Meniére's disease and the related clinical phenomenon of delayed endolymphatic hydrops (DEH) is a generally accepted treatment modality when hearing preservation is not an issue. Over the past 8 years, 25 patients (13 Meniére's and 12 DEH) underwent labyrinthectomy by the senior author (J.R.) at the Toronto Hospital. This retrospective study compares and contrasts these two clinical groups preoperatively and subsequently analyzes the results of labyrinthectomy. The patients' own responses to a specially designed questionn

Surgical treatment of Ménière's disease.

Several reliable surgical procedures aimed to control the vertigo associated with Meniere's disease exist. Vestibular preservation and ablative procedures are available with the latter divided between hearing preservation and destructive operations. Factors to consider prior to choosing a surgical option include the age, health, and employment status of the patient; the degree of hearing loss in the involved ear; and the status of the contralateral ear.

Distance from the labyrinthine portion of the facial nerve to the basal turn of the cochlea. Temporal bone histopathologic study.

The middle cranial fossa approach to lesions of the geniculate ganglion and internal auditory canal preserves cochlear function and affords access to the lateral internal auditory canal. The labyrinthine portion of the facial nerve tends to course near the basal turn of the cochlea, just beneath the middle cranial fossa floor, and is usually dissected in this approach. To determine the distance from the labyrinthine portion of the facial nerve to the basal turn of the cochlea, measurements were obtained in the temporal bones of 24 subjects (48 ears) 9 to 76 years of age. These subjects had no

Idiopathic perilymphatic fistulae: a temporal bone histopathological study. Clinical, surgical and histopathological correlations.

The temporal bone histopathological findings herein reported are the first to be from a patient who had, during life, the diagnosis of idiopathic perilymphatic fistulae. The patient received surgical treatment followed by rapid improvement of his vestibular symptoms. These specimens histopathologically demonstrate the patencies of the labyrinth capsule that were diagnosed (predicted) clinically during life, the qualities of the membranous labyrinth in both the operated and unoperated ears, and the elements of the surgical repair. Notably there is no evidence of endolymphatic hydrops. The diagn

Immunological approach to Ménière's disease: vestibular immune injury following immune reaction of the endolymphatic sac.

Following direct antigen challenge to the endolymphatic sac (sac), the relation of caloric test results and spontaneous nystagmus to the histological changes of the sensory epithelium of the vestibular organ and perilymph antibody levels was investigated in the guinea pig. In a secondary keyhole limpet hemocyanin (KLH) challenge to the sac, irritative nystagmus was followed by paralytic nystagmus and a suppression of caloric reaction. These findings correlated well with the degree of degeneration of the sensory epithelium of the vestibular organ and the levels of perilymph antibody. On the oth

Temporal analysis in normal and impaired hearing.

The ear contains an array of filters that separate the components of a complex signal into "channels" tuned to different center frequencies. Temporal analysis can be considered as two processes: analysis of the time pattern occurring within each channel, and comparison of the time patterns across channels. Within-channel acuity can be characterized by tasks such as gap detection, or by the ability to detect amplitude modulation as a function of modulation rate. The smallest detectable gap duration for a white noise stimulus is 2-3 ms. The results can be modeled by an array of filters, with eac

The combined supra- and infratentorial approach for lesions of the petrous and clival regions: experience with 46 cases.

The combined supra- and infratentorial approach has been subdivided into three variations: the retrolabyrinthine technique (petrous bone resection with preservation of hearing); the translabyrinthine technique (greater petrous bone resection and sacrifice of hearing); and the transcochlear technique (maximum petrous drilling, sacrifice of hearing, and transposition of the facial nerve). These three variations maximize temporal bone drilling and therefore provide exquisite exposure of the clivus and petrous regions with minimal or no brain retraction. The superior petrosal sinus is always sacri

Medical imaging in cochlear implant candidates.

A retrospective study of preoperative radiographic surveys obtained in 85 cochlear implant patients was performed. A total of 90 cochlear implant patients were seen at our institution during the last 10 years. In three, the implant was placed at another institution. In two otosclerosis patients, no preimplant radiographic examination was obtained. Meningitis was the cause of deafness in 45 of 90 patients (50 percent); 28 of 40 pediatric age group patients (70 percent), and in 17 of 50 adult patients (34 percent). Thirty-three of 45 postmeningitis patients (73 percent) showed characteristic rad

The translabyrinthine approach for the removal of large acoustic neuromas.

The results and the sequelae are reported from a series of consecutive 400 translabyrinthine removals of acoustic neuromas comprising almost all such tumors in Denmark during a 12-year period from 1976 to 1988. There were two intrameatal, 137 median-sized, 112 large and 149 advanced tumors, the latter having a diameter exceeding 40 mm. The mortality rate in the overall series was 2%. The incidence of cerebrospinal fluid leak occurred in 11%, and persisted for more than 2 weeks in 4%. Facial nerve function was completely normal in 67% of the patients, while only 5% required a nerve anastomosis.

Results of surgical treatment for Menière's disease.

In 1985 the American Academy of Otolaryngology-Head and Neck Surgery Committee on Hearing and Equilibrium established revised guidelines for reporting treatment results for Menière's disease. Since then little information regarding the newly adapted criteria and their effects on the evaluation process has appeared. Thus we compared the results of different surgical procedures for Menière's disease using both the 1985 and 1972 guidelines. One hundred nine surgical procedures from 1969 to 1985 were reviewed. Six different surgical procedures were evaluated: the Cody-Tack, cochleosacculotomy, end

The Auto-Tilt Test in patients with positional vertigo and Menière's disease.

As a new method of testing the righting reflex, which has been considered to be closely related to the otolith function, we used the Auto-Tilt Test (ATT) on 24 patients with positional vertigo, on 5 patients with Meniere's disease before and after cochlearendolymphatic shunt, and on 10 normal subjects. The subject is required to stand on an electrically driven tilt board with an autoregulating switch in hand, and to perform bilateral tilt swings of 5 degrees and 10 degrees wearing a helmet with a Head Inclinometer or tilt sensor. The result of this test in patients with positional vertigo reve

Shunt surgery in Meniere's disease: a follow-up report.

Between December 1969 and May 1986, 140 patients were treated for Menière's disease using the endolymphatic sac subarachnoid shunt. The technique and results are contrasted with an earlier report. The surgical technique described includes modifications of the originally described procedure. A microcomputer was used for the storage and manipulation of data based upon 1972 Academy criteria. Control of vertigo was accomplished in 83% of patients; 51% of patients were graded class A or B, according to the 1972 Academy criteria. No serious complications occurred. Problems included the potential for

[Does pure-tone audiometry provide inferences for a vascular cause of sudden deafness?].

The theory that sudden hearing loss is caused by some form of vascular catastrophe has not been confirmed by pathological evidence, but it agrees with a number of clinical observations. But there are several pieces of experimental and clinical evidence that cast serious doubt upon a vascular hypothesis, especially the considerable variability in site and degree of cochlear and vestibular impairment. A review of the vascular anatomy of the inner ear makes it clear that many clinically observed audiometric patterns and associated partial vestibular deficits in idiopathic sudden hearing loss cann

Endolymphatic sac surgery for Menière's disease: eighteen years' experience with the Kitahara sac operation.

From 1968 to 1983, 162 cases of Meniere's disease were treated surgically with our intramastoid drainage technique. One hundred forty of these cases could be followed up for two years or more. Treatment results were calculated according to the 1972 AAOO criteria and the 1985 AAO-HNS criteria. Using our technique, definitive vertigo spells could be controlled completely in 78% of cases and substantially controlled in 94%. Hearing worsened in only 15% of all cases. Therefore, prevention of worsening of hearing, especially in bilateral cases, and control of vertigo spells in patients with frequen

The vestibular functions of the manganese-deficient rat.

Vestibular functions were examined in 25 animals of the second generation of rats raised on a manganese-deficient diet; 9 animals appeared to be normal and 16 showed behavioural abnormalities, characterized by inability to swim and lack of tonic eye deviations. The nystagmic responses elicited by rotatory stimulation appeared to be normal. Histological studies showed that the only defect was the absence of both utricular and saccular otoconia. These rats' swimming disability was similar to that of bilabyrinthectomized rats in normogravic condition and to that of normal rats immersed in water d

Endolymphatic hydrops in guinea pigs after cauterizing the sac with silver nitrate.

There have been several reports of perisaccular fibrosis of the endolymphatic sac in some autopsied cases of Meniere's disease. We attempted to initiate perisaccular fibrosis of the sac in guinea pigs by injecting a small amount of 10% silver nitrate solution that cauterizes the tissue readily to produce scar, ie, fibrosis. Ninety-nine guinea pigs out of 108 (92%) that were treated with silver nitrate showed the endolymphatic hydrops in varied extent, from slight to profound. The extent of hydrops might depend on the extent of destruction of the sac following the cauterization. The lumen of th

Acoustic neuromas presenting as sudden deafness.

Caution is required in diagnosing even apparently "obvious" causes of hearing loss. A patient is described who came to medical attention through referral from a lawyer for sudden deafness allegedly caused by exposure to noise in an industrial setting. Thorough evaluation revealed an acoustic neuroma. Review of the literature suggests that presentation of acoustic neuromas as sudden deafness is more common than generally recognized. Clinicians must be alert to this possibility and diligent in their evaluation of any unilateral ear symptoms.

Motion sickness.

Motion sickness affects approximately 90 percent of the population at some time during life, and for many the problem is recurrent and severe. Labyrinthine-defective individuals are "immune." Adaptation is highly specific to one type of motion. Recent studies have focused on the role of the limbic system in mechanisms of motion sickness. Medications are sometimes useful, but they have a high incidence of side effects. Oral agents are ineffective after symptoms develop.
