Total talar extrusion: a case report.

UNLABELLED: Total talar extrusion is an extremely rare injury that occurs after a high-energy trauma. There are few reported cases in literature and there is no consensus as to the appropriate treatment of the extruded talus. Historically, the treatment options for open total talar dislocation have been limited to talectomy and fusion or reimplantation after thorough debridement. We report a case of an open dislocation of the talus with total talar extrusion. Immediate surgical debridement, reduction, and external fixation were performed under antibiotic coverage. Antibiotic-impregnated polyme

Cementless Hemiarthroplasty Complication Risk Does Not Support Contemporary Utilization Patterns.

BACKGROUND: Hemiarthroplasty (HA) is commonly performed in the setting of femoral neck fracture to allow for early mobilization. Fixation of the femoral component can be either press fit or cemented. The purpose of this study was to compare cemented and cementless HA utilization and complications. METHODS: A multicenter retrospective analysis was performed on hip fracture patients from 2010 to 2019. The primary outcome of this study was revision due to periprosthetic fracture. Secondary outcomes included operative time, surgical complications, and mortality. Logistic regression was performed

If at first you don't succeed, should you try again? The efficacy of repeated closed reductions of distal radius fractures.

INTRODUCTION: A repeated closed reduction ("re-reduction") of a displaced distal radius fracture is a common procedure performed to obtain satisfactory alignment and avoid surgery when the initial reduction is deemed unsatisfactory. However, the efficacy of re-reduction is unclear. Compared to a single closed reduction, does a re-reduction of a displaced distal radius fracture: (1) improve radiographic alignment at the time of fracture union and, (2) decrease the rate of operative intervention? MATERIALS AND METHODS: Retrospective cohort analysis of 99 adults aged 20-99 years with extra-artic

What Is the Best Treatment Choice for Concomitant Ipsilateral Femoral Neck and Intertrochanteric Fracture? A Retrospective Comparative Analysis of 115 Consecutive Patients.

PURPOSE: Although a concomitant ipsilateral femoral neck and intertrochanteric fracture has been considered to be a rare type of injury, its incidence has been increasing, especially among elderly hip fracture patients. However, there is limited evidence on the optimal treatment option. This study surveys surgical outcomes of different implants in order to assist in selecting the best possible implant for a combined femoral neck and intertrochanteric fracture. METHODS: The postoperative complications after the treatment of a concomitant ipsilateral femoral neck and intertrochanteric fracture

Cemented Versus Noncemented Hemiarthroplasty for Femoral Neck Fractures: A Meta-Analysis of Randomized Control Trials.

INTRODUCTION: The purpose of this study was to conduct a meta-analysis of the randomized controlled trials (RCTs) to compare the outcomes after cemented and noncemented hemiarthroplasty for femoral neck fractures. METHODS: A literature search of three databases was conducted based on the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines. RCTs comparing cemented and noncemented hemiarthroplasty for femoral neck fractures were included. Clinical outcomes were compared using Revman, and a P-value < 0.05 was considered to be statistically significant. RESULTS: Nine RC

Indications, complications, and clinical outcomes of fixation and acute total hip arthroplasty for the treatment of acetabular fractures: A systematic review.

PURPOSE: Acetabular fracture fixation can be challenging, especially in the elderly. Open reduction and internal fixation (ORIF) alone may not allow for early weight bearing and is associated with a high rate of secondary osteoarthritis; therefore, a combined hip procedure (CHP) or ORIF with acute total hip arthroplasty, may be beneficial in this population. The objective of this study was to perform a systematic review of all reported cases of CHP. METHODS: PubMed, Embase, Scopus, and Cochrane databases were searched for studies analyzing acetabular fractures in the elderly managed with a co

Preoperative indications for total shoulder arthroplasty predict adverse postoperative complications.

BackgroundAlthough studies have shown improved pain, function, and patient satisfaction after total shoulder arthroplasty (TSA), preoperative factors predicting poor outcomes are unexplored. Comparison of postoperative complications between osteoarthritis (OA), cuff arthropathy (CA), and fracture patients is important for identifying at-risk patients.MethodsPrimary TSAs from 2014 to 2016 with preoperative OA, CA, and proximal humerus fractures as indications were queried from the National Surgical Quality Improvement Program database. Evaluation of facial nerve following open reduction and internal fixation of subcondylar fracture through retromandibular transparotid approach.

The objective of this study was to evaluate any damage to the facial nerve after a retromandibular transparotid approach for open reduction and internal fixation (ORIF) of a subcondylar fracture. We studied 38 patients with 44 subcondylar fractures (3 bilateral and 38 unilateral) treated by ORIF through a retromandibular transparotid approach. All patients were followed up for 6 months. Postoperative function of the facial nerve was evaluated within 24h of operation, and at 1, 3, and 12 weeks, and 6 months. Variables including type of fracture, degree of mouth opening, postoperative occlusion,

Epidemiological study of orthopedic injuries in hemodialysis patients in Taiwan: a fixed cohort survey, 2004-2008.

BACKGROUND: The burden of chronic kidney disease (CKD) is a growing concern worldwide. The prevalence of hemodialysis in Taiwan is the highest in the world, and this may increase the prevalence of orthopedic fractures. The aim of this study was to explore the incidences of various orthopedic injuries and the related risk factors. METHODS: A nationwide prospective study based on the Taiwan National Health Insurance dataset was conducted during 2004-2008. A total of 82,491 CKD patients were selected as the fixed cohort population. The International Classification of Diseases 9-CM diagnosis code

Management of high-energy foot and ankle injuries in the geriatric population.

By the year 2035 almost 20% of the US population of 389 million people will be 65 years and older. What this group has, compared with aged populations in the past, is better health, more mobility, and more active lifestyles. From January 1989 through December 2010, a total of 494 elderly patients with 536 foot and ankle injuries were identified. Within this group, 237 (48%) patients with 294 injuries were sustained as a result of a high-energy mechanism. These mechanisms consisted of 170 motor vehicle accidents, 30 as a result of high (not ground level) energy falls, 2 from industrial accident

[Surgical treatment method of calcaneal intraarticular fractures].

Open repositioning and internal fixation of intraarticular calcaneal fractures has become a standard surgical method, however, concerns over its feared complications, including, in particular, secondary wound healing followed by osteomyelitis, continue fo be discussed. The aim of this study is to inform the surgical public about the surgical method. From February 2004 to September 2005, 26 intraarticular calcaneal fractures in 22 patients were operated using open repositioning and internal cast fixation. In four cases, the fracture was bilateral. The patients were indicated for the procedure b

Multifocal arterial haemorrhage in a partially stable pelvic fracture after a crush injury: a case report.

INTRODUCTION: Most pelvic haemorrhages are thought to be caused by injury to small arteries or veins in the fractured cancellous pelvic bone or in the surrounding soft-tissues, and only 6-18% of patients with unstable pelvic fractures have haemorrhage from larger arteries. When arterial injuries are present, the majority involve branches of the internal iliac artery with only few published reports of injuries to the external iliac artery or its branches. MATERIALS AND METHODS: We report of a patient who sustained a combined pelvic and acetabular fracture with multifocal bleeding involving bra

[The supracondylar fracture of the humerus in children (author's transl)].

To find out the cause of posttraumatic varus and valgus deformity of the elbow a long-term follow-up examination of 183 dislocated and 20 undislocated supracondylar extension-fractures of the humerus was done. There were different methods of treatment: In most of the cases closed reduction was performed and fixation in acute angle-plaster or by percutaneous radial or radial and ulnar wires. 75% showed radiologically and 55% clinically an alteration of the carrying angle. The clear reason for this deformity was a rotation displacement, which leads in oblique fractures directly, in transverse fr

Halo immobilization of cervical spine fractures. Indications and results.

Thirty-three patients with a spectrum of cervical spine fractures or subluxations were treated with immobilization by a halo apparatus. All spines were assumed to be unstable because of the nature of the fracture or because of a subluxation noted on spine films. Treatment consisted of immobilization and fracture reduction followed by application of a halo plaster cast or molded halo plastic vest. Patient acceptance was high. Complications were few and minor. No patient experienced neurological deterioration during treatment. Reduction was well maintained during an average halo immobilization p

A large solitary fibroma of the deep compartment of the sole.

A 50-year-old lady presented with a right foot mass and pain for 1year. Magnetic resonance imaging showed a 3.9×3.2×5cm mass on plantar side of the forefoot deep to the major flexor tendons and plantar aponeurosis and inferior to 1st-4th metatarsals. The mass extended dorsally through the intermetatarsal space to the foot dorsum. Ultrasound guided biopsy was performed, which confirmed it was a fibroma. This case was further complicated by pathological fracture of the 3rd metatarsal. It was resected through a dorsal incision and surgical dislocation of the 2nd and 3rd tarsometatarsal joints. Ma
