[Madelung's disease].

Madelung's disease (Md) is a rare metabolic disorder which presents with multiple symmetrical nonencapsulated fatty masses situated mainly around the head, neck and nape. The main indications for Md treatment are: shortness of breath, head's movement restrictions, swallowing disorders (dysphagia), aesthetic considerations, and the fear of cancer. The standard treatment of Md is the surgical excision of pathological tissues. The disease is progressive, relapses after removal of tissue are often observed. The discussion is based on a three-year observation of Md patient operated by the authors.

Hugo Karl Liepmann and apraxia.

John Hughlings Jackson (1835-1911) first clearly described apraxia in 1861, though he did not provide the specific name. Apraxias are subtle motor disorders in which there is an interruption of the organisation of movement mainly located in the left hemisphere. Hugo Karl Liepmann (1863-1925) was responsible for their elucidation, distinguishing ideomotor, limb-kinetic or innervatory, and ideational apraxias that affect distinct central associational areas of the cortex with characteristic clinical results. This notion was later expanded and clarified by Geschwind's 'disconnection syndromes'. T

Orthodontic treatment of patients using bisphosphonates: a report of 2 cases.

The pharmacologic agent bisphosphonate has recently received much attention in the dental literature. Bisphosphonates in oral or intravenous forms are used to treat various diseases such as certain cancers, bone- and calcium-related disorders, osteoporosis, and osteopenia. Bisphosphonates inhibit bone turnover and result in increased bone-mineral density. The most serious dental side effect of bisphosphonate treatment (particularly when it is administered intravenously) is, paradoxically, osteonecrosis of the mandible or the maxilla represented by exposed nonhealing bone. Other related complic

NMDA receptor-mediated excitotoxicity contributes to the cerebral hypoxic injury of a rat model of posthypoxic myoclonus.

Cardiac arrest-induced cerebral hypoxic injury could induce posthypoxic movement disorders. Here we investigated the effects of memantine, an NMDA receptor channel blocker, on the neurodegeneration occurred in an established rat model of posthypoxic myoclonus. We found that administration of memantine for 7 days significantly reduced cerebral hypoxia-induced neurodegeneration in the CA1 of the hippocampus, the reticular thalamic nucleus (RTN) and the primary fissure of the cerebellum of the posthypoxic animals. The results suggest that the neurodegeneration observed in specific areas of the br…

The assessment and treatment of specific phobias: a review.

Specific phobia is one of the most common and easily treated mental disorders. In this review, empirically supported assessment and treatment procedures for specific phobia are discussed. Exposure-based treatments in particular are highlighted given their demonstrated effectiveness for this condition. The format and characteristics of exposure-based treatment and predictors of treatment response are outlined to provide recommendations for maximizing outcome. In addition, several other treatments for specific phobia are reviewed and critiqued, including cognitive therapy, virtual reality, eye m

Insights from studying human sleep disorders.

Problems with sleep are one of the commonest reasons for seeking medical attention. Knowledge gained from basic research into sleep in animals has led to marked advances in the understanding of human sleep, with important diagnostic and therapeutic implications. At the same time, research guided by human sleep disorders is leading to important basic sleep concepts. For example, sleep may not be a global, but rather a local, brain phenomenon. Furthermore, contrary to common assumptions, wakefulness, rapid eye movement (REM) and non-REM sleep are not mutually exclusive states. This striking real

Hypocalcemic generalised seizures as a manifestation of iatrogenic hypoparathyroidism months to years after thyroid surgery.

Hypoparathyroidism is a relatively common side effect of a thyroidectomy and leads to hypocalcemia. Carpopedal spasm and tetany are typical manifestations and usually occur within weeks after surgery. The first signs can be less typical and include movement disorders such as chorea, as well as symptoms of increased intracranial pressure or epileptic seizures. We describe two cases with generalised tonic-clonic seizures as the first manifestation of postoperative hypoparathyroidism, appearing months and years after thyroidectomy. Iatrogenic hypoparathyroidism needs to be considered in the diffe…

Recurrent jaw dislocation after botulinum toxin treatment for sialorrhoea in amyotrophic lateral sclerosis.

Botulinum toxin (BTX) has been used successfully to treat various movement disorders, and is increasingly used for many other medical conditions. Sialorrhoea is a disabling symptom in many neurological patients including those with Parkinson's disease, stroke and amyotrophic lateral sclerosis (ALS). BTX has recently been shown to be effective for treating sialorrhoea. We report an ALS patient who developed recurrent jaw dislocation following BTX treatment for sialorrhoea to highlight the observation that intraparotid BTX may be complicated by jaw dislocations in some at-risk ALS patients. Clin…

[Diagnostic steps in disk dislocations and other frequent illnesses of the temporomandibular joint].

Modern diagnosis of the temporomandibular joint (TMJ) consists of multiple steps, based on each other and amplifying one another. The first step is the clinical functional analysis, respecting general medical as well as specific dental aspects. If need arises electronic axiography, recording the movement of TMJ hingeaxis, is being added as well as occlusal analysis on mounted plaster casts. In case of open questions at this point of analysis, magnetresonance- and computer-tomographic imaging is brought in. The present paper is an overview describing the current step by step diagnosis of TMJ di

[Motor paroxysmal disorders].

OBJECTIVE: In this paper we review and bring up to date non-epileptic paroxystic motor disorders, since knowledge of these is necessary to avoid confusion with epileptic crises. DEVELOPMENT: Particular attention has been paid to the etiopathogenesis, classification and treatment of tics, including Tourette syndrome. Familial paroxystic choreoathetosis and alterations of movement induced by drugs are considered in the light of new etiopathogenic concepts, such as hyperekplexy, spasmus nutans and Sandifer syndrome. We also mention the clinical aspects of shudders, paroxystic vertigo, continuous

[Takayasu arteritis and the algodystrophic syndrome].

Algodystrophic syndrome represents a group of symptoms involving pain and oedema of the affected extremity, with movement disorders, vegetative and vasomotor disturbances and focal osteoporosis development. The key role in the pathogenesis of the syndrome plays intensive and prolonged stimulation of pain receptors. The authors present a case report of 47 years old woman with Takayasu's arteritis complicated by skin defects on right instep and sole with consequent development of algodystrophic syndrome on both lower extremities. Combined influence of both types of nociceptive stimuli is demonst…

Electrophysiological target localization is not required for the treatment of functional disorders.

A total of 117 patients underwent 124 functional stereotactic procedures with the Leksell Gamma Knife to treat trigeminal neuralgia, movement disorders and chronic pain. Target identification was performed with stereotactic magnetic resonance imaging alone without electrophysiologic localization. Clinical observation of these patients over 1-41 months indicates that the efficacy of the procedure is comparable with that seen with open stereotactic procedures where electrophysiological localization is used. Using current techniques, the complications of functional neurosurgery with the Gamma Kni…

Trinucleotide (CAG) repeat expansion in chromosomes of Spanish patients with Huntington's disease.

Huntington's disease (HD) is a neurodegenerative and hereditary disease characterized by progressive movement disorders and mental and behavioral abnormalities. The HD gene is an expanding and unstable trinucleotide repeat (CAG repeat sequences). We studied 77 individuals from 38 families with HD in an attempt to obtain information for genetic counselling and differential diagnosis. Our results indicate that individuals with more than 40 repeats will be affected by the disease, whereas those with fewer than 30 will be healthy. There can be some overlap between 30 and 40 repeats, and one should…

Positron emission tomography in the Rett syndrome: clinical, biochemical and pathological correlates.

A consistent constellation of clinical signs and symptoms define the Rett syndrome, the most prominent of which are disorders of movement and tone. Preliminary pathologic and neurochemical data indicate predominant involvement of the nigrostriatal dopaminergic pathways and the cholinergic system of the basal forebrain region. The age of onset differentiates the Rett syndrome from Alzheimer and Parkinson disease with similar lesions. PET scanning makes it possible to relate the chemistry of the brain to function by measuring the number and affinity of neuroreceptors, metabolism in specific brai

The headache profile of idiopathic intracranial hypertension.

The headache profile of idiopathic intracranial hypertension (IIH, pseudotumour cerebri) has not previously been prospectively studied. We administered a questionnaire to 63 cases at the time of diagnosis. Fifty-eight of the cases had headache and 93% of those with headache reported it to be their most severe ever. Patients characteristically noted a pulsatile headache of gradually increasing intensity that had awakened them. Daily headache occurred in 74% of those reporting headache. Pain in a nerve root distribution or retro-ocular pain with eye movement, uncommon with other headache disorde

Phenytoin potentiation of neuroleptic-induced dyskinesias.

Clinically phenytoin-induced movement disorders may resemble neuroleptic-induced tardive dyskinesia (TD). Neuroleptic-induced TD is thought to be related to an increase in the number and affinity of dopamine D2 receptors. We investigated the interaction of phenytoin with dopaminergic systems by measuring dopamine behavior and receptor changes in animals treated with phenytoin alone or in combination with chlorpromazine. Phenytoin alone or in combination produced significant behavioral supersensitivity without alteration of dopamine D2 receptor density or affinity. This animal study suggests th…
