Uncovering the heterogeneity of care trajectories leading up to a rheumatoid arthritis diagnosis: a Swedish 10-year study.

OBJECTIVES: This study aimed to identify types of care trajectories leading to rheumatoid arthritis (RA) diagnosis, and to identify if, and when, such trajectories deviate from those in the general population. METHODS: We included patients with a first-ever RA diagnosis between 2016 and 2020 in the Swedish nationwide multiregister system and matched general population controls. We performed group-based multitrajectory modelling on 9 healthcare components over the 10 years preceding the index date, grouping patients based on their pattern of healthcare use. We described these groups, or 'types

Real-world pharmacovigilance of drug-related bone metabolism disorders: integrating FAERS and VigiAccess with a Bradford Hill-based causal plausibility assessment.

BACKGROUND: Metabolic bone diseases (MBDs) are a group of conditions characterized by imbalanced bone remodeling, including osteoporosis, osteomalacia, and rickets. In recent years, drug-induced secondary MBDs have emerged as a major clinical concern. However, systematic drug risk assessment studies based on large-scale real-world data remain scarce. METHODS: Based on the U.S. Food and Drug Administration Adverse Event Reporting System (FAERS, 2004-2024) and VigiAccess, four complementary disproportionality algorithms (reporting odds ratio, ROR; proportional reporting ratio, PRR; Bayesian con

Low back pain during prolonged sitting on a sofa is associated with pelvic posterior tilt in individuals with non-specific chronic low back pain.

PURPOSE: Nonspecific chronic low back pain (NSCLBP) is a prevalent musculoskeletal disorder often aggravated by prolonged sitting. Previous studies have linked trunk muscle activity and posture to pain development, but their findings were inconsistent. Furthermore, previous studies did not use a backrest, so the study conditions did not reflect real-life conditions. The present study investigated the relationship between pain intensity, pelvic and trunk alignment, and lumbar muscle activity during sofa sitting in individuals with and without NSCLBP. METHODS: Eighteen participants with NSCLBP

Burden of disease-specific late effects after proton beam therapy in pediatric solid and CNS tumor survivors: a single-institution survey in Japan.

BACKGROUND: Proton beam therapy (PBT) has been increasingly adopted in pediatric oncology to reduce radiation-related toxicity; however, its long-term comorbidities remain incompletely understood. Because tumor types and treatment patterns often overlap across diagnostic groups, disease-specific evaluation may help clarify late effects following PBT. METHODS: A mailed questionnaire survey was conducted between 2015 and 2021 among 304 pediatric cancer survivors who had received PBT at the University of Tsukuba Hospital between 1984 and 2020. Responses from 110 patients diagnosed before the age

Infant limb kinematics and muscle force variation during kicking is associated with clinical cerebral palsy risk measures.

Early limb motor development progresses from spontaneous to increasingly complex movements. Restricted movement variety in infancy can indicate neuromotor impairment associated with cerebral palsy (CP) risk. The General Movement Optimality Score (GMOS) used in clinic detects abnormal movement patterns, but requires highly trained physiotherapists, limiting accessibility. Objective biomechanical measures may provide quantitative predictors of CP risk, reducing clinical burden while offering novel physiotherapeutic targets. This study investigated whether variation in lower limb kinematics and m

Sex differences in healthcare use before the first multiple sclerosis-related demyelinating event.

While the multiple sclerosis (MS) prodrome may be prolonged, healthcare use by sex >5 years pre-onset remains underexplored. We examined healthcare use up to 29 years pre-MS onset, stratified by sex. Using administrative data from Ontario, Canada (1991-2020), we compared annual physician visit rates by diagnostic chapter between MS and matched non-MS cohorts, stratified by sex. Quasi Poisson models estimated rate ratios (RRs) with 95% confidence intervals (CI). Included were 35,018 MS and 136,007 matched non-MS individuals (mean onset age ∼43 years; standard deviation ∼14 years; ∼69% female in
