[Pediatric knee injuries].

Patella dislocations are the most common knee injuries causing hemarthrosis in children. Flake fractures represent the main complication of these injuries and require fixation. First time dislocations are treated conservatively. Recurrent dislocations are managed operatively. Precise analysis of the underlying dispositional and pathological factors is important to determine the appropriate operative procedure. To protect the growth plate, special techniques are required. Tibial spine fractures are bony avulsions of the anterior cruciate ligament (ACL). Management includes closed reduction in h

Incidence of and risk factors for the development of asymptomatic heterotopic ossification after elbow fracture fixation.

OBJECTIVE: This study was performed to investigate the incidence of and risk factors for asymptomatic heterotopic ossification (HO) after open reduction and internal fixation of elbow fractures. METHODS: This retrospective analysis involved 197 patients with elbow fractures treated operatively from 2014 to 2017 at our institution. Patient-related and clinical variables were recorded. Univariate analysis and multivariate logistic regression were performed to reveal independent risk factors for postoperative HO. In addition, a receiver operating characteristic (ROC) curve was performed to asses

The treatment of olecranon fractures in adults.

Olecranon fractures are common. They are usually managed surgically with open reduction and either tension band wiring or plate fixation. Currently, there are few studies comparing fracture treatments. We aim to review the available literature to guide the orthopaedic surgeon on the management of these fractures. A literature review of peer-reviewed publications in international orthopaedic journals detailing olecranon fracture treatment was conducted. An additional focus was placed on the evidence base for and surgical outcomes of tension band wiring for common two-part fractures. Our novel i

Variation in hip fracture treatment: are black and white patients treated equally?

To examine disparity in race for hip fracture treatment, we identified femoral neck fractures and pertrochanteric fractures from a 20% sample of 1999-2003 Medicare part B claims data and stratified patients by treatment: total hip arthroplasty (THA), hemiarthroplasty (HA), open reduction and internal fixation (ORIF), and nonoperative management (NM). Covariables included patient race, age, sex, and Charlson Comorbidity Index score. The geographic variable was the hospital referral region (306 such US regions are defined in The Dartmouth Atlas of Health Care). Logistic regression was performed

Patella fracture after post total knee replacements.

A retrospective study was done to determine the incidence, clinical function, and complications of treatment in a large series of patella fractures after total knee replacement (TKR). Between 1983 and 1996, 4583 primary AGC TKRs done at our institution. One hundred seventy-seven fractures were identified in 135 patients for a fracture incidence of 3.8%. Twenty-two fractures were vertical and had a stable implant and intact extension mechanism (Type 1). Twenty-one fractures had disruption of the extensor mechanism of less than 1 cm (Type 2A). Seventeen fractures had disruption of the extensor m

Heterotopic ossification prophylaxis with indomethacin increases the risk of long-bone nonunion.

Indomethacin is commonly administered for the prophylaxis of heterotopic ossification (HO) after the surgical treatment of acetabular fractures. Non-steroidal anti-inflammatory drugs such as indomethacin, have been associated with delayed healing of fractures and mechanically weaker callus. Our aim was to determine if patients with an acetabular fracture, who received indomethacin for prophylaxis against HO, were at risk of delayed healing or nonunion of any associated fractures of long bones. We reviewed 282 patients who had had open reduction and internal fixation of an acetabular fracture.

Comparing approaches to expose type C fractures of the distal humerus for ORIF in elderly patients: six years clinical experience with both the triceps-sparing approach and olecranon osteotomy.

BACKGROUND: Although open reduction and internal fixation (ORIF) is a standard fracture treatment method, the optimal way to expose a fracture prior to ORIF is debated. We compared the effects of two exposure methods, the triceps-sparing approach and olecranon osteotomy, on the functional outcomes of ORIF-treated type C distal humerus fractures in elderly people. METHODS: From January 2006 to January 2011, 75 elderly patients with type C distal humerus fractures were treated with ORIF, and we retrospectively reviewed their medical records, radiographs, and follow-up charts to identify any com

Treatment of periprosthetic femoral fractures with the Bicontact revision stem.

PURPOSE: Periprosthetic fractures in cases without prior loosening of the stem can be treated with open reduction and internal fixation, but cases with preexisting loosening and/or bone defects present specific challenges to the surgeon. The keys to the success of intramedullary stabilization of femoral fractures--reconstruction of length, axis and rotation rather than meticulous reduction of the fragments and minimal impact on fragment vascularization by the surgical approach--can be transferred to the treatment of periprosthetic fractures. METHOD: The Bicontact revision stem can be regarded

Treatment of AO/OTA 43-C3 Pilon Fracture: Be Aware of Posterior Column Malreduction.

Treatment of pilon fractures remains challenging due to the difficulty of fracture reduction and associated soft tissue complications. The aim of this study was to evaluate the pitfalls and strategies of posterior column reduction in the treatment of complex tibial pilon fractures (AO/OTA 43-C3). Thirteen AO/OTA classification 43-C3 type pilon fractures treated between January 2013 and January 2016 were retrospectively analyzed. Nine cases were treated by external fixation within 26 hours (range, 6-56 hours) after injury. The definitive open reduction and internal fixation (ORIF) was performed

Assessment of Satisfaction Rate After Closed Reductions of Nasal Bone Fractures: A Cross-Sectional Study.

BACKGROUND: The nose is the central and most protruded part of the face, most vulnerable to facial trauma. Treatment for nasal bone fractures involves open reduction and closed reduction. Closed nasal reduction is a simple procedure that can be performed under local or general anesthesia with an acceptable success rate. AIM: The current study aimed to assess the patient's satisfaction following the closed reduction of nasal bone fractures, both in aesthetic and functional values, and causes of dissatisfaction. METHODOLOGY: A community-based descriptive cross-sectional study was conducted in

Norian SRS cement compared with conventional fixation in distal radial fractures. A randomized study.

BACKGROUND: A prospective, randomized multicenter study was conducted to evaluate closed reduction and immobilization with and without Norian SRS (Skeletal Repair System) cement in the management of distal radial fractures. Norian SRS is a calcium-phosphate bone cement that is injectable, hardens in situ, and cures by a crystallization reaction to form dahllite, a carbonated apatite equivalent to bone mineral. METHODS: A total of 323 patients with a distal radial fracture were randomized to treatment with or without Norian SRS cement. Stratification factors included fracture type (intra-artic

Broken bones and orthopedist groans: can an acute care surgeon fix both?

BACKGROUND: Increasing reluctance of specialty surgeons to participate in trauma care has placed undue burden on orthopedic traumatologists at Level I trauma centers and prompted the exploration of an expanded role for general trauma surgeons in the initial management of select orthopedic injuries (OI) as an acute care surgeon. This study characterizes OI sustained by trauma patients (TPs) to analyze the feasibility of this concept. METHODS: The National Trauma Data Bank was queried for specific information relating to the profile of OI. International Classification of Diseases-9th Revision c
