Non-linear threshold effects of kinesiophobia on exercise adherence in older adults with COPD: a segmented regression analysis.

OBJECTIVE: To explore the threshold effect of kinesiophobia on exercise adherence in older adult patients with chronic obstructive pulmonary disease (COPD). METHODS: A cross-sectional survey was conducted on 357 older adults with COPD were assessed using standardized questionnaires for general information, exercise adherence, and kinesiophobia (Tampa Scale for Kinesiophobia, TSK). Multiple linear regression identified independent factors affecting adherence. A restricted cubic spline model analyzed the non-linear relationship between kinesiophobia and adherence. RESULTS: The mean scores for

Neuromodulation of heart rate and heart rate variability in a randomized controlled trial of tDCS-augmented in vivo exposure for specific fears.

This study evaluated the prediction that tDCS-augmented in vivo exposure (IVE) for specific fears would result in durable changes in heart rate (BPM) and heart-rate variability (HRV) during and just after exposure to feared targets. In a double-blind, placebo-controlled trial, participants with contamination- and animal phobia (N = 49) were randomized to active tDCS (1.7 mA, 20 min; n = 27), or sham tDCS (1.7 mA, 30 s; n = 22), followed by a single session of 30 min of IVE. Active tDCS targeted excitation of the left mPFC and inhibition of the right dlPFC. BPM and HRV were acquired during beha

Impact of NPSR1 gene variation on the neural correlates of phasic and sustained fear in spider phobia-an imaging genetics and independent replication approach.

The functional neuropeptide S receptor 1 (NPSR1) gene A/T variant (rs324981) is associated with fear processing. We investigated the impact of NPSR1 genotype on fear processing and on symptom reduction following treatment in individuals with spider phobia. A replication approach was applied [discovery sample: Münster (MS) nMS = 104; replication sample Würzburg (WZ) nWZ = 81]. Participants were genotyped for NPSR1 rs324981 [T-allele carriers (risk) versus AA homozygotes (no-risk)]. A sustained and phasic fear paradigm was applied during functional magnetic resonance imaging. A one-session virtu

Study protocol: a randomized controlled trial comparing the efficacy of therapist guided internet-delivered cognitive therapy (TG-iConquerFear) with augmented treatment as usual in reducing fear of cancer recurrence in Danish colorectal cancer survivors.

BACKGROUND: Cognitive therapy has been shown to reduce fear of cancer recurrence (FCR), mainly in breast cancer survivors. The accessibility of cognitive behavioural interventions could be further improved by Internet delivery, but self-guided interventions have shown limited efficacy. The aim of this study is to test the efficacy of a therapist guided internet-delivered intervention (TG-iConquerFear) vs. augmented treatment as usual (aTAU) in Danish colorectal cancer survivors. METHODS/DESIGN: A population-based randomized controlled trial (RCT) comparing TG-iConquerFear with aTAU (1:1) in n

Fear of cancer recurrence in childhood cancer survivors: A developmental perspective from infancy to young adulthood.

AIMS: Fear of cancer recurrence (FCR) is among the most prevalent and distressing concerns reported by cancer survivors. While younger age is the most consistent predictor of elevated FCR, research to date has focused almost exclusively on adult cancer survivors. This is despite the fact that children with cancer are more likely to survive compared to adults, and will become regular, lifelong users of the medical system to mitigate the effects of toxic treatment. The early experience of cancer yields unique challenges and circumstances that may impact children's capacity for and experience of

Effects of cognitive behavior therapy on regional brain volume in spider-phobic patients: preliminary results.

Successful cognitive behavior therapy (CBT) for spider phobia is able to change patients' brain activation during visual symptom provocation. The present voxel-based morphometry study investigated whether this therapy approach can additionally affect brain structure. We analyzed gray matter volume of 12 spider-phobic patients prior to CBT and in a six-month follow-up investigation, and contrasted the results with data from 13 non-phobic controls. CBT provoked a dramatic decrease in syndrome severity in the clinical group as indexed by self-report and by a behavioral approach test. This was acc

Impaired visuocortical discrimination learning of socially conditioned stimuli in social anxiety.

In search of causative factors of social anxiety disorder (SAD), classical conditioning has been discussed as a potential trigger mechanism for many years. Recent findings suggest that the social relevance of the unconditioned stimulus (US) might play a major role in learning theories of SAD. Thus, this study applied a social conditioning paradigm with disorder-relevant US to examine the electrocortical correlates of affective learning. Twenty-four high socially anxious (HSA) and 23 age- and gender-matched low socially anxious (LSA) subjects were conditioned to 3 different faces flickering at

Current issues in the treatment of specific phobia: recommendations for innovative applications of hypnosis.

Specific phobia is the most common and treatable of the anxiety disorders. Exposure-based therapies are the treatment of choice and empirically validated protocols are available that promise rapid and effective results. In many cases, however, patients are reluctant to comply with demanding schedules of exposure, increasing the risk of treatment failure. Furthermore, in clinical practice, patients often present with multiple phobias and other Axis I and Axis II disorders that can further complicate therapy. This article covers four important issues that have been addressed in the literature: (

Training less threatening interpretations over the Internet: Does the number of missing letters matter?

BACKGROUND AND OBJECTIVES: Cognitive Bias Modification to reduce threat interpretations (CBM-I) trains individuals to resolve ambiguous scenarios via completion of word fragments that assign benign meanings to scenarios. The current study tested: 1) whether Internet-based CBM-I can shift interpretations to be more positive/less negative, and 2) whether varying the number of letters missing in the word fragments (assumed to increase task difficulty) moderates CBM-I's effects. METHODS: Participants (N = 350) completed a brief online version of CBM-I, followed by assessments of interpretation bi

Exposure treatment in multiple contexts attenuates return of fear via renewal in high spider fearful individuals.

BACKGROUND AND OBJECTIVES: Research has demonstrated that after exposure treatment, re-exposure to a previously feared stimulus outside of the treatment context can result in renewal of fear. The current study investigated whether conducting exposure treatment in multiple real-life contexts can attenuate renewal of fear. METHODS: Forty-six moderate to high spider fearful individuals were randomly allocated to groups that received exposure treatment in either one context or three contexts. Follow-up testing was conducted one week and four weeks after exposure in the treatment context or a nove
