Unilateral upper and lower subtotal maxillectomy approaches to the cranial base: microsurgical anatomy.

OBJECTIVE: The relationship of the maxilla, with its thin walls, to the nasal and oral cavities, the orbit, and the infratemporal and pterygopalatine fossae makes it a suitable route for accessing lesions involving both the central and lateral cranial base. In this study, we compared the surgical anatomy and exposure obtained by two unilateral transmaxillary approaches, one directed through an upper subtotal maxillectomy, and the other through a lower subtotal maxillectomy. METHODS: Cadaveric specimens examined, with 3 to 40x magnification, provided the material for this study. RESULTS: Both

Surgical anatomy of the infratemporal fossa: the styloid diaphragm revisited.

INTRODUCTION: The infratemporal fossa (ITF) gives passage to most major cerebral vessels and cranial nerves. Dissection of the ITF is essential in many of the lateral cranial base approaches and in exposure of the high cervical internal carotid artery (ICA). We reviewed the surgical anatomy of this region. METHODS: Direct foraminal measurements were made in seven dry skulls (14 sides), and the relationship of these foramina to each other and various landmarks were determined. Ten ITF dissections were performed using a preauricular subtemporal-infratemporal approach. Preliminary dissections of

Reconstruction of pediatric cranial base defects: a review of a single microsurgeon's 30-year experience.

BACKGROUND: For the past 30 years, microsurgical free tissue transfer has enabled the reconstruction of pediatric cranial base lesions formerly believed to be refractory to surgical therapy. Due to the relative rarity of these oncologic processes and the highly specialized requirements for their treatment, few large-scale reviews of microsurgical reconstruction of pediatric cranial base lesions have been published to date. METHODS: A review of all free tissue transfer reconstructive procedures undertaken by a single microsurgeon for pediatric cranial base defects was performed for operations

Congenital skull fracture as a presentation of Menkes disease.

We report the rare presentation of Menkes disease with a congenital skull fracture, intracerebral bleeding, and seizures. The diagnosis was made at 3 months of age based on the characteristic features of the syndrome, by which time the child experienced uncontrollable seizures. Following progressive neurodegeneration, death occurred at 3 years of age. The prognosis in Menkes disease is dependent on early copper-histidine therapy. Effective treatment has led to children surviving into adulthood. Diagnosing the syndrome during the neonatal period is difficult. There are no published reports of c

New diagnostic approach of the different types of isolated craniosynostosis.

In this study, we diagnose skull shape deformities by analysing sinusoid curves obtained from standardized computed tomography (CT) slices of the skull for the common craniosynostoses (scaphocephaly, brachycephaly, trigonocephaly, right- and left-sided anterior plagiocephaly). Scaphocephaly has a high forehead peak and low troughs, in contrast to brachycephaly. Anterior plagiocephaly has asymmetry and shifting of the forehead peak. Trigonocephaly has a high and narrow frontal peak. Control patients have a symmetrical skull shape with low troughs and a high and broader frontal peak. Firstly, we

Arterial relationships to the nerves and some rigid structures in the posterior cranial fossa.

The close relationships between the cranial nerves and the arterial vessels in the posterior cranial fossa are one of the predisposing factors for artery-nerve compression. The aim of this study was to examine the relationships of the vertebral and basilar arteries to some skull and dural structures and the nerves in the posterior cranial fossa. For this purpose, the skull bases and brains of 70 cadavers were studied. The topographic relationships of the vertebral and basilar arteries to the cranial nerves in the posterior cranial fossa were studied and the distances between the arteries and s…

[The posterior part of the skull. Classification of dysmorphism. Original treatment: turned biparietal flap transposition].

The deformation of the posterior part of the skull (occipito-vertebral region), induced directly or indirectly, occurs in numerous pathological situations. Its significance is frequently overlooked. Lesions of the cranial content, alterations of the lambdoid suture or other premature synostosis, abnormal constraint related to posture or to muscular activity can modify the posterior curvature of the skull, generally flattening it. The authors propose a classification based on three points: -Intracranial pathology: alterations of the brain or CSF fluid can induce either insufficient (microenceph

Impact of the skull contour definition on Leksell Gamma Knife® Icon™ radiosurgery treatment planning.

INTRODUCTION: The Gamma Knife® planning software (TMR 10, Elekta Instruments, AB, Sweden) affords two ways of defining the skull volume, the "historical" one using manual measurements (still perform in some centers) and the new one using image-based skull contours. Our objective was to assess the potential variation of the dose delivery calculation using consecutively in the same patients the two above-mentioned techniques. MATERIALS AND METHODS: We included in this self-case-control study, 50 patients, treated with GKRS between July 2016 and January 2017 in Lausanne University Hospital, Swit

Cochlear isolation in neonatal mice for in vitro and ex vivo models: an anatomical landmark-based protocol.

INTRODUCTION: Ex vivo and in vitro models utilizing inner ear tissue from neonatal rodents are vital for developing treatments for hearing loss caused by damage to hair cells or auditory neurons. The cochlear isolation from the skull is associated with a high risk of mechanical damage to the soft tissues enclosed in the otic capsule. Such damage may result in artifactual hair cell loss and morphological alterations of SGNs. This stage of cochlear dissection requires more detailed description. To overcome technical difficulties, we described a protocol based on the use of key anatomic lines of

Effects of the freezing and thawing process on biomechanical properties of the human skull.

The aim of this study was to determine if biomechanical investigations of skull samples are reliable after skulls have been subjected to a freezing and thawing process. The skulls were obtained from 105 Japanese cadavers (66 males, 39 females) of known age that were autopsied in our department between October 2012 and June 2013. We obtained bone specimens from eight sites (four bilaterally symmetrical pairs) of each skull and measured the mass of each specimen. They were then classified into three groups (A, B, C) based on the duration of freezing of the experimental samples. The left-side sam

Implementation and validation of finite element model of skull deformation and failure response during uniaxial compression.

Numerical studies aimed at evaluating head injury due to externally applied loading can be made more biofidelic by incorporating nonlinear mechanism-based and microstructurally-inspired material models representing the mechanical response and fracture (failure or injury) of the human skull bone. Thus, incorporation of these mechanism-based models would increase the ability of simulations of mechanical impact to identify more realistic fracture-based injuries at clinical relevancy, such as linear (tensile), depressed (compressive), or penetration (shear). One of the challenges for accurate mode

Performance of a through-transmit aberration correction method for transcranial focused ultrasound: Simulation and experimental evaluation.

Transcranial focused ultrasound is a promising neuromodulation tool, but skull-induced aberrations to the ultrasound beam require subject-specific correction. To predict and compensate for aberration, the standard in transcranial focused ultrasound is to perform acoustic simulations based on x-ray computed tomography images of the head. However, computed tomography-based methods face shortcomings due to imaging resolution and uncertainty in acoustic parameter estimates. Relativistic through-transmit (RTT) is a recently proposed imaging-independent method that corrects skull-induced aberrations

What's that big thing on your head? Diagnosis of a large frontoparietal lesion on an Eastern Zhou skull from Henan, China.

We carried out a differential diagnosis of a large frontoparietal lesion on a human skull from a Late Bronze Age archaeological site located on the Central Plain of China, dating to between 771 and 476 BC. The head of this individual was covered in cinnabar, a mercury-based pigment that later was used for medicinal purposes in China. The lesion was well-circumscribed and involved the outer and inner tables of the skull, slight diploë thickening, and coarsening of bone trabeculae with expansion of intertrabecular spaces. We show that the observed changes are most consistent with cavernous heman

Giovanni Battista Morgagni: facial reconstruction by virtual anthropology.

Multidisciplinary research was carried out on human skeletal remains to identify if they belonged to the famous pathologist Giovanni Battista Morgagni, who died at a very old age and was buried in a church in Padua. The purpose of this study was to analyze the skull, creating a virtual model necessary for facial reconstruction, so as to contribute to the identification of the skull. The skeletal remains were found buried in the Morgagni family grave. Based on preliminary anthropological evidence, that the skull might be ascribable to Giovanni Battista Morgagni, a digitized model of the skull w

Correlation of ocular findings and plain skull roentgenogram in pituitary adenoma.

A review of the conditions of 44 consecutive patients diagnosed as having pituitary tumor is presented. In none of those with abnormal visual acuity, visual field defect, or extraocular muscle palsy was the plain skull roentgenogram normal. In all but one patient who had an abnormality on ocular examination), there was erosion of the clinoids on the plain lateral skull roentgenogram and, in that one patient, erosion was found on the side of the visual loss by the use of tomography. Based on these observations, we have stressed primarily the early use of simple plain skull roentgenograms in the

Secular changes in craniofacial morphology of the Portuguese using geometric morphometrics.

This study examines patterns of secular change in cranial morphology in the New Lisbon collection, a documented skeletal collection from Lisbon, Portugal with birth years from 1806 to 1954. This period represents a time when Lisbon was undergoing increased urbanization and population growth, as well as changes in mortality and fertility patterns. Previous studies from the U.S., Europe, and Japan have reported significant secular changes in cranial morphology over the past 200 years. In the current study, secular changes were analyzed using three-dimensional geometric morphometrics methods. The
