A pilot study of the effects of non-invasive mechanical ventilation on the prognosis of ischemic cerebrovascular events in patients with obstructive sleep apnea syndrome.

Obstructive sleep apnea syndrome (OSAS) is an independent risk factor for ischemic cerebrovascular diseases, and results in worse prognosis and higher mortality. We aimed to investigate the effects of early treatment of OSAS on the prognosis of ischemic stroke. We prospectively evaluated patients with acute supratentorial ischemic stroke and OSAS on admission (acute stage), at second week (subacute stage) and at second month (chronic stage); 11 (73.3%) out of 15 patients put on the non-invasive mechanical ventilation treatment within 48 h of stroke constituted the treatment group, and 13 patie

Effectiveness of transoral robotic surgery of the base of the tongue vs. conservative treatment for obstructive sleep apnea, a RCT, the RAPID study protocol.

BACKGROUND: CPAP is the gold standard treatment for obstructive sleep apnea (OSA) but suffers from poor long-term compliance. Alternatives like mandibular advancement devices (MADs) and surgery, such as transoral robotic surgery (TORS), are available, but their comparative efficacy is unclear. STUDY OBJECTIVE: This study aims to evaluate whether base of the tongue (BOT) reduction using TORS is equal or superior to CPAP and MADs in improving quality of sleep and life in patients with moderate to severe OSA eligible for surgery. STUDY DESIGN: A prospective randomized controlled trial. POPULAT

The body mass index-glucose index as a new tool for early detection of the risk of dysglycemia in patients with hypertension and obstructive sleep apnea.

PURPOSE: Currently, there is a lack of early biomarkers to identify the risk of dysglycemia in patients with concurrent hypertension and obstructive sleep apnea (OSA). The aim of our study is to evaluate the efficacy of the recently proposed Body Mass Index (BMI)-Glucose (ByG) index in identifying the risk of dysglycemia in patients with hypertension and OSA. METHODS: A retrospective cohort study of 1,759 adults with hypertension and OSA from the Urumqi Research on Sleep Apnea and Hypertension study (UROSAH) was conducted. Cox proportional hazards models were used to assess the associations b

The impact of a single night of continuous positive airway pressure titration on postural balance in patients with severe obstructive sleep apnea syndrome.

PURPOSE: Obstructive sleep apnea syndrome (OSAS), the most common sleep disorder, leads to impaired executive functions, attention, balance, and coordination. This study aimed to evaluate the impact of a single night of continuous positive airway pressure (CPAP) therapy on the postural balance of patients with severe OSAS using static posturography (SPG). METHODS: This prospective cross-sectional comparative study evaluated patients with snoring, daytime sleepiness, and possible apnea who presented to the outpatient clinic. Patients diagnosed with severe OSAS underwent SPG analysis twice, afte

Effects of continuous positive airway pressure therapy on depression and anxiety in patients with obstructive sleep apnea with and without coronary heart disease.

BACKGROUND: Continuous positive airway pressure (CPAP) treatment brings more benefits to most patients with obstructive sleep apnea (OSA), especially OSA patients with coronary heart disease(CAD). These patients often have emotional disorders such as anxiety and depression, which have a negative impact on their clinical prognosis. OBJECTIVES: To investigate whether CPAP can improve anxiety, depression, and inflammatory cytokine levels in patients with OSA with and without comorbid CAD. METHODS: 72 patients were randomly assigned to a CPAP group or a control group. The CPAP group received con

The role of supine position in obstructive sleep apnea in patients with stroke: A cross-sectional study comparing stroke unit patients with outpatients.

Obstructive sleep apnea (OSA) presents an elevated risk to patients with stroke, especially during the acute phase when prolonged bed rest in the supine position is common in stroke unit (SU). Recent studies suggest that rostral fluid shifts may worsen OSA severity. This study aimed to compare OSA severity between patients with acute stroke in a SU and outpatients with OSA using in-laboratory polysomnography (iPSG) to gain further insight into this relationship. This cross-sectional study was conducted at Samsung Changwon Hospital between April 2021 and October 2022. Patients with acute stroke

Impact of acuinjection at ST36 on leg movements, their associated respiratory events, and electroencephalographic arousals in patients with obstructive sleep apnea: A case series.

RATIONALE: Leg movements (LMs) frequently co-occur with obstructive sleep apnea (OSA), and respiratory-related LMs can trigger arrhythmias and cause diagnostic challenges during polysomnography (PSG). While the cardiovascular burden of periodic limb movements in sleep is recognized, little is known about the role of LM suppression during PSG in improving diagnostic accuracy. This case series examines the impact of acuinjection at ST36 on LM suppression during PSG in patients with OSA. PATIENT CONCERNS: All 3 patients underwent diagnostic PSG due to suspected OSA. PSG was temporarily paused du

Pathophysiological Traits in Pediatric Obstructive Sleep Apnea. Associations with Patient Characteristics and Responses to Therapy: A Secondary Analysis of the CHAT Clinical Trial.

Rationale: In children with adenotonsillar hypertrophy, there is substantial variation in pediatric obstructive sleep apnea (pOSA) severity, which may be driven by differences in pathophysiological traits (endotypes), including pharyngeal collapsibility, dilator muscle compensation, arousal threshold, and chemoreflex loop gain. Objectives: To determine if pathophysiological traits for pOSA vary with participant characteristics, correlate with pOSA severity, and change after adenotonsillectomy. Methods: Traits estimation, requiring adequate nasal pressure data quality, was performed during slee

No impact of a high-fat meal coupled with intermittent hypoxemia on acute kidney injury biomarkers in adults with and without obstructive sleep apnea.

Obstructive sleep apnea (OSA) is characterized by chronic intermittent hypoxemia, which is associated with progressive loss of kidney function, where postprandial fluctuations in renal physiology may further compromise oxygen supply and kidney function. Therefore, we measured biomarkers of acute kidney injury (AKI) following a high-fat meal with and without intermittent hypoxemia. Eighteen healthy young men (mean age [SD]: 22.7 years [3.1]) and seven middle-aged to older individuals with OSA (54.4 years [6.4]) consumed a high-fat meal during normoxia or intermittent hypoxemia (~15 hypoxic cycl

Effects of CPAP and Mandibular Advancement Devices on depressive symptoms in patients with obstructive sleep apnea: a meta-analysis of randomized controlled trials.

PURPOSE: Studies show that patients with obstructive sleep apnea (OSA) are more likely than the general population to have psychological disorders such as depression. However, it is less clear how OSA treatment affects this association. This meta-analysis aimed to assess whether or not continuous positive airway pressure (CPAP) and mandibular advancement devices (MADs) reduce depression symptoms in patients with OSA. METHODS: We searched Pubmed, Embase, Web of Science, and Cochrane Library from creating the databases until November 2022. Our analysis included RCTs that examined CPAP and MAD t

Revisiting the Epworth sleepiness scale : Is excessive daytime sleepiness still a valid screening tool for obstructive sleep apnea in a population at risk?

OBJECTIVE: Obstructive sleep apnea (OSA) is a common medical problem with numerous comorbidities and high costs. Since the introduction of the Epworth sleepiness scale (ESS), excessive daytime sleepiness (EDS) has been considered the most common and prominent symptom of OSA. Aim of this study was to re-evaluate the ESS for detection of OSA in a population at risk compared to the gold standard overnight polysomnography (PSG). METHODS: A total of 266 patients (mean age 57.9 ± 11.6 years; 189 men and 77 women), referred to our sleep laboratory for probable OSA, were given ESS followed by an over

Placebo response in objective and subjective measures of hypersomnia in randomized clinical trials on obstructive sleep apnea. A systematic review and meta-analysis.

Sleepiness is one of the outcomes most used in randomized clinical trials (RCT) on the effect of treatments for obstructive sleep apnea (OSA). Furthermore, it is known that there is a placebo effect, especially in subjective measures. Therefore, given that sleepiness is a subjective measure, the objective of this systematic review with meta-analysis and three-level meta-regression was to assess the response to different placebos (pills and sham-CPAP) used in RCTs in OSA, both on subjective (Epworth Sleepiness Scale [ESS]) and objective (Multiple Sleep Latency Test [MSLT], Maintenance Wake Test
