Increased proliferative response of peripheral blood mononuclear cells and T cells to Streptococcus mutans and glucosyltransferase D antigens in the exacerbation stage of recurrent aphthous ulcerations.

BACKGROUND AND PURPOSE: Cytotoxic T lymphocyte-induced lysis of virus-infected oral epithelial cells has been shown to be a cause of early ulcer formation in recurrent aphthous ulcerations (RAU). To test whether bacteria and their associated antigens are involved in the disease process of RAU, the proliferative response (PR) to different streptococcal species in peripheral blood mononuclear cells (PBMC) and T cells isolated from RAU patients at the exacerbation stage was determined. METHODS: PBMC and T cells were isolated from 39 patients with RAU, 21 patients with erosive oral lichen plaus (

Exposure of oral mucosa to bioactive milk factors reduces severity of chemotherapy-induced mucositis in the hamster.

A biologically active extract containing bovine whey proteins, whey growth factor extract-A (WGFE-A) was administered topically to the oral mucosa of hamsters and its ability to prevent and treat chemotherapy-induced oral mucositis investigated. Oral mucositis was induced in Syrian golden hamsters through a combination treatment of the antimetabolite chemotherapy drug 5-fluorouracil (5-FU), and mild abrasion of the cheek pouch. WGFE-A administered to the oral mucosa via hydrogel and liquid treatments, pre and concurrent to 5-FU therapy, resulted in significantly reduced mucosal ulceration. The

Signs of medullar aplasia in the oral cavity: report of case.

Medullar aplasia is a hematological disease characterized by medullar dysfunction that results in a marked decrease of various hematological cellular elements. This produces anemia, infections of different etiologies and also, spontaneous or provoked hemorrhagic syndromes of varying importance. A case of medullar aplasia affecting a child, diagnosed after a tooth extraction is reported, and accompanied by its pathological characteristics. In addition, an easy reading E.L.I.S.A/ test for diagnosing herpes virus type 1 or 2 is presented.

Pharmacokinetics and pharmacodynamics of thalidomide in HIV patients treated for oral aphthous ulcers: ACTG protocol 251. AIDS Clinical Trials Group.

Thalidomide has increasing clinical benefits, including the healing of aphthous ulcers in patients with HIV. Unfortunately, pharmacological information addressing the pharmacokinetics (PK) of this compound in HIV patients is limited. Concern exists as to whether thalidomide may alter its own metabolism owing to in vitro data previously reported. Furthermore, no information is available defining the relationship between drug exposure and clinical response. This study evaluated the PK and pharmacodynamics (PD) of thalidomide in patients enrolled in AIDS Clinical Trials Group Protocol 251. Study

Empirical therapy of febrile neutropenic patients with mucositis: challenge of risk-based therapy.

Nowadays Gram-positive cocci, especially oral viridans streptococci (OVS) and coagulase-negative staphylococci (CoNS), are the most common bloodstream isolates in febrile neutropenic patients. Although in general these cocci are quite indolent, Streptococcus mitis is associated with serious complications such as sepsis and/or adult respiratory distress syndrome. Neutropenia is the most significant predisposing factor but the impact of mucositis, i.e. damage to the mucosal barrier of mouth and intestines (mucosal barrier injury, MBI), is very much greatly underestimated. Oral mucositis is a str

Immunolocalization of tumor necrosis factor-alpha expressing cells in recurrent aphthous ulcer lesions (RAU).

Tumor necrosis factor (TNF)-alpha is a pro-inflammatory cytokine and crucial mediator in many aspects of immunity. Although several studies have shown that recurrent aphthous ulcers (RAU) can be prevented by treatment that prevents the synthesis of endogenous TNF-alpha little is known about the location and distribution of TNF-alpha-expressing cells at disease sites. The aim of the present work is, therefore, to investigate TNF-alpha and its cellular distribution in RAU lesions compared with those in induced oral traumatic ulcers (TUs). Twelve biopsies of RAU lesions of oral mucosa were obtain

Parameter on acute periodontal diseases. American Academy of Periodontology.

The American Academy of Periodontology has developed the following parameter on the treatment of acute periodontal diseases. Patients should be informed about the disease process, therapeutic alternatives, potential complications, expected results, and their responsibility in treatment. Consequences of no treatment should be explained. Failure to treat acute periodontal diseases appropriately can result in progressive loss of periodontal supporting tissues, an adverse change in prognosis, and could result in tooth loss. Given this information, patients should then be able to make informed deci

[Behcet's disease].

Three women aged 30, 38 and 34 years, originating from Morocco, the Dominican Republic and Turkey, had been suffering since several years from, among other things, oral aphthae, vaginal ulcers and skin lesions without specific abnormalities at examination of blood and biopsies. The diagnosis of Behçet's disease was made and immunomodulating treatment instituted, following which the patients improved. Behçet's disease is a chronic inflammatory multisystemic disorder of unknown aetiology. Originally it was characterised as a triad of recurrent oral aphthae, genital ulcers and inflammatory eye di

[The use of benzyl alcohol and amyl-m-cresol (Strepsils) in the oral cavity. (Review of the literature and first clinical experiences)].

The authors summarised the literature data of benzylalcohol and amyl-m-cresol. In the clinical practice they used Strepsils in the treatment of 22 patients suffered from oral inflammatic diseases and in the 20 prophylactic cases of oral surgical interventions. The preliminary clinical report shows that, the Strepsils seems to be very effective in the treatment of oral inflammations and in the prophylaxis too.

Aphthous ulcers and vitamin B12 deficiency.

Aphthous ulcers of the oral cavity are frequently encountered in general practice. Although the exact pathophysiology remains obscure, many factors can contribute to the pathogenesis of these lesions, such as immunological factors, local trauma, smoking, stress, hormonal state, family history, food hypersensitivity, and infection. We describe three patients in whom a clear relationship appeared to exist between recurrent aphthous ulcers and a deficiency of vitamin B12. It is concluded that in all patients with recurrent aphthous ulceration, deficiency of vitamin B12 should be considered.

[Prevention and therapy of acute radiation injuries of the skin and mucosa. I. Results of a German multicenter questionnaire].

BACKGROUND: The acute radiation related morbidity is an essential factor for the patient's outcome in radiotherapy. The prophylactic and therapeutic management of acute side effects has a wide clinical range between different radiation oncology departments. In this work, it was to evaluate the remedies, which are used for prevention and therapeutic management of acute radiation related morbidity of the skin and mucosa (mouth, pharynx, esophagus, small and large bowel, rectum and vagina). METHODS: A questionnaire was sent to 130 radiotherapeutic departments in Germany in July 1995. The questio

Amlexanox oral paste: a novel treatment that accelerates the healing of aphthous ulcers.

Five percent Amlexanox oral paste is a novel treatment for aphthous ulcers. In 3 controlled clinical studies that evaluated 1,124 immunocompetent patients with mild to moderate aphthous ulcers, 5% Amlexanox oral paste (Aphthasol) was shown to accelerate healing of these ulcers. Treatment with Aphthasol reduced the median time to ulcer healing and to complete pain resolution in a statistically significant manner. This was true both when treatment with 5% Amlexanox oral paste was compared to treatment with a vehicle and when treatment with the Amlexanox paste was compared to no treatment. Study

Manifestations and treatment of xerostomia and associated oral effects secondary to head and neck radiation therapy.

Xerostomia is one of the most common side effects of head and neck radiation therapy. Other oral effects are mucositis and radiation caries. Because xerostomia resulting from radiation therapy may be of a more permanent nature than xerostomia resulting from other causes, treatment is typically more extensive. Numerous regimens treat symptoms of xerostomia and associated caries and mucositis. Among them is the daily application of a fluoride gel, recommended to prevent or minimize dental caries. For patients with severe, chronic xerostomia who have some residual salivary tissue, the use of a si

Treatment of neutropenic oral ulceration in human immunodeficiency virus infection with G-CSF.

We report the novel use of granulocyte colony-stimulating factor in the treatment of severe oral ulceration in human immunodeficiency virus-associated neutropenia. A 38-year-old man with acquired immunodeficiency syndrome developed severe refractory oral aphthous ulceration that persisted for 6 months despite a range of empirical treatments including corticosteroid. Rapid resolution of the ulceration occurred once the neutrophil count was restored to normal with granulocyte colony-stimulating factor.

Quencher-enhanced specificity of psoralen-photosensitized virus inactivation in platelet concentrates.

BACKGROUND: Treatment with psoralens and UVA (PUVA) has been shown to be efficacious in eliminating the risk of virus transmission by platelet concentrates (PCs). It has previously been demonstrated that, during the inactivation of cell-free vesicular stomatitis virus (VSV) by aminomethyltrimethylpsoralen (AMT) and UVA in PCs, platelet function could be protected either by oxygen removal before irradiation or by inclusion of a type I free radical quencher, such as mannitol. STUDY DESIGN AND METHODS: Under previous PUVA treatment conditions for PCs (25 micrograms/mL AMT; 30 min UVA at 7 mW/cm2…

Recurrent aphthous ulceration and food sensitivity.

Forty patients with recurrent aphthous ulceration (RAU) were investigated to ascertain the possibility of food sensitivity being a significant precipitating factor in their recurrent oral ulceration. All patients were hematologically normal and did not respond to vitamin B1 and B6 therapy. Patch testing (Standard European Series) was undertaken in 21 patients. Twenty patients showed a positive reaction to a number of substances which were considered clinically relevant. Professional advice on avoidance of the allergen was given, resulting in an improvement in eighteen patients. It is concluded

Hamster cheek-pouch testing of dental soft polymers.

The hamster cheek pouch provides a suitable model system for the mucous-membrane irritation testing of dental materials. Poor retention of materials or difficulties in histopathological interpretation caused by surgical artifacts have been reported in published techniques. We describe a new "pouch-in-pouch" technique for mucous membrane irritation tests. The retention rate of polymer discs was 97% and 87% at 14 and 35 days, respectively. Clear differentiation was obtained between the tissue reaction to the test materials and the surgical procedure. Polymer discs containing dibutyltin diacetate
