Advanced practice clinicians' interest in providing medical abortion: results of a California survey.

CONTEXT: California's Reproductive Health Privacy Act, which became law in January 2003, clarified that advanced practice clinicians could legally provide medical abortion. Little is known about the characteristics associated with nonphysician clinicians' interest in receiving medical abortion training or their perceptions of barriers to medical abortion provision. METHODS: In early 2003, a total of 1,176 licensed advanced practice clinicians in California-nurse practitioners, physician assistants and certified nurse-midwives-completed a mail-in survey assessing their personal characteristics

[Functional results after type I thyroplasty with the Montgomery's prosthesis].

Between January 1999 and October 2000, 16 patients with glottic insufficiency of varying etiologies underwent Montgomery type I thyroplasty at the E.N.T. Department of the University of Brescia, Italy. In 2 cases revision surgery was required for the onset of delayed complication (cutaneous and endolaryngeal exposure of the prosthesis). All subjects underwent a battery of clinical-instrumental tests pre- and postoperatively as well as 2, 6 and 12 months after surgery. These tests included videolaryngostroboscopy, subjective perception analysis using the GRBAS scale proposed by Hirano on vowels

Physician intention to recommend complete diagnostic evaluation in colorectal cancer screening.

Primary care physicians (PCPs) often do not recommend complete diagnostic evaluation (CDE; i.e., diagnostic colonoscopy or the combination of flexible sigmoidoscopy and barium enema X-ray procedures) for patients with an abnormal screening fecal occult blood test (FOBT+) result. Information is needed to understand why PCPs do not recommend CDE. In the spring of 1994, a telephone survey was carried out using a random sample of 520 PCPs in Pennsylvania or New Jersey who had patients that were targeted for an FOBT screening program. Survey data were obtained from 363 (70%) PCPs on physician pract

Sympathetic skin response differentiates hereditary sensory autonomic neuropathies III and IV.

OBJECTIVE: To evaluate whether sympathetic skin response (SSR) differs in patients with hereditary sensory autonomic neuropathy (HSAN) types III and IV. BACKGROUND: HSAN types III and IV are rare autosomal recessive disorders that cause many similar autonomic, sensory, and motor dysfunctions, but different sweating characteristics. HSAN III patients have preserved and at times, excessive sweating, whereas anhidrosis is characteristic of HSAN IV. SSR reflects the integrity of sympathetic sudomotor fibers and the activation of sweat glands through the change in skin resistance in response to an

Knowledge, perceptions and behaviour of mothers toward intestinal helminths in Upper Egypt: implications for control.

Operational research was conducted in Qena Governorate, Upper Egypt, to gather the information needed to design an integrated control programme for intestinal helminths in the area. To this aim, qualitative and quantitative techniques were used on a sample representative of the entire governorate. Mothers were questioned about their knowledge and perception of intestinal helminths, their hygienic habits and health-seeking behaviour. At the same time, a coprological survey was conducted on all the children 2 to 12 years of age living in the selected households (n = 2657) to provide baseline dat

The relationship between ventilation and oxygen consumption in man is the same during both moderate exercise and shivering.

1. Four naïve subjects clothed in protective suits were immersed up to the neck in water on twenty-two occasions, the minimum skin temperature was 20 degrees C and the average skin temperature 26 degrees C. Subjects at first rested, initially demonstrating an increase in thermal muscular tone which did not lead to perceptible limb movements. Later, while resting, visible shivering with small unco-ordinated limb movements supervened. The subjects then exercised while still immersed by stationary cycling. 2. Steady-state minute oxygen consumption (VO2) and minute ventilation (VE) were measured o

Changes in cognitive task performance across the menstrual cycle.

Menstrual-cycle-related changes in estrogen were expected to differentially affect various cognitive tests. Specifically, the estrogen peak occurring at midcycle in ovulatory women was expected to facilitate performance of highly practiced "automatized" tasks and to impair performance of "perceptual-restructuring" tasks, compared with performance of these tasks in the postovulatory phase of the cycle when progesterone is thought to counteract the action of estrogen. Perceptual-restructuring tasks are defined as tasks in which the initial percepts to obvious stimulus attributes are wrong and mu

Usefulness of the Parental Electronic Diary During Medical Rounds in a NICU.

Parental involvement in the care of their baby in family rooms in neonatal intensive care units (NICUs) can be improved. This could be done with an electronic medical report completed by the parents, which is then linked to the patient record system. The parents selected for this study completed an electronic diary during their stay in the NICU, while the staff answered a questionnaire about their opinion on the usefulness of the parents' diary. The length of stay, length of time the baby spent in Kangaroo care, breastfeeding, time given to breastfeeding, feeling of tiredness, the capability o

Effect of Donepezil on Speech Recognition in Adult Cochlear Implant Users: A Pilot Randomized Clinical Trial.

OBJECTIVES: Speech recognition outcomes following cochlear implantation vary widely among users, potentially due to differences in neuroplasticity. The cholinergic system plays a critical role in cortical reorganization and sensory learning; yet, its potential to enhance auditory plasticity in cochlear implant (CI) users has not been explored. The aim of this study is to investigate the effects of donepezil, a cholinergic agonist, on speech perception and cortical plasticity in adults with CIs. DESIGN: This pilot randomized clinical trial was conducted at a single tertiary academic center fro

Chitin-glucan improves important pathophysiological features of irritable bowel syndrome.

BACKGROUND: Irritable bowel syndrome (IBS) is one of the most frequent and debilitating conditions leading to gastroenterological referrals. However, recommended treatments remain limited, yielding only limited therapeutic gains. Chitin-glucan (CG) is a novel dietary prebiotic classically used in humans at a dosage of 1.5-3.0 g/d and is considered a safe food ingredient by the European Food Safety Authority. To provide an alternative approach to managing patients with IBS, we performed preclinical molecular, cellular, and animal studies to evaluate the role of chitin-glucan in the main pathoph

Surgery for nonarteritic anterior ischemic optic neuropathy.

BACKGROUND: Nonarteritic ischemic optic neuropathy (NAION) is characterized by sudden and painless loss of vision in the eye, accompanied by pallid swelling of the optic disc. Its etiology is unknown and no medical therapy has been proven effective in treating this condition. Optic nerve decompression surgery, a proposed treatment for NAION, involves making two or more slits or a window in the tissue surrounding the optic nerve, thereby allowing cerebrospinal fluid to escape, and theoretically reducing the pressure surrounding the optic nerve. OBJECTIVES: The objective of this review was to a

Development and evaluation of a new measure of children's play: the Children's Play Scale (CPS).

BACKGROUND: There is increasing recognition of the importance of children's play from a public health perspective, given the links between play and children's physical and mental health. The present research aimed to develop and evaluate a new parent-report questionnaire that measures the time children spend playing across a range of places and includes a supplement to evaluate how adventurously children play. METHODS: The questionnaire was developed with input from a diverse group of parents and experts in children's play. It was designed to yield a range of metrics including time spent play

Sensory and cognitive event-related potentials in myalgic encephalomyelitis.

Myalgic Encephalomyelitis (ME) is a form of post viral fatigue syndrome resulting in myalgia and fluctuating fatiguability. Symptoms reflecting central nervous system dysfunction are common and include muscle weakness, headache, sensory disturbances, poor short term memory and impairment of concentration. In view of the fact that sensory and cognitive disturbances are experienced by many patients objective evidence was sought with multi-modality sensory evoked potentials and auditory event-related cognitive potentials in a group of ME patients both with and without the enteroviral antigen, VP1

Cognitive state monitoring for neuroadaptive information visualization.

In today's digital world, we face an overwhelming influx of information daily. Information Visualisation (InfoVis) serves as a promising approach to alleviate this "information overload" issue by providing timely, effective, and adaptive visual representations. Despite its potential, previous research has under-explored the critical role of users' cognitive states in facilitating more natural interactions. This article bridges this research gap by first presenting a comprehensive review of current methodologies for assessing cognitive states through neurophysiological signal analysis. Specific

Present tense: a phenomenology of grief in neurosurgery.

This paper applies phenomenological insights to the neurosurgeon's lived experience of grief, approaching it not as a discrete psychological reaction, but as an enduring aspect of professional life shaped by repeated encounters with loss, complication, and responsibility. For neurosurgeons, grief is not limited to moments of patient loss but emerges gradually through the cumulative weight of adverse outcomes, moral uncertainty, and the recognition of medicine's limits. Despite its pervasiveness, grief is often treated as something to be managed, resolved, or privately endured. Drawing on pheno

Transient Frontal Fracturing: A Theoretical Account of Hyperfocus.

Many people have experienced being so engrossed in an activity that they lost awareness of their surroundings, had difficulty stopping the activity, and found their perception of time condensed. This experience is known colloquially as hyperfocus. There is a small but quickly growing body of peer-reviewed research on hyperfocus. Most of this research is dependent upon self-report measures with little consideration given to the neurocognitive mechanisms responsible for hyperfocus. To advance hyperfocus research, this nascent field must move beyond self-report to uncover the neurocognitive mecha

Behavioral consequences of blunting fear with acetaminophen.

The widely used painkiller acetaminophen (also known as paracetamol) has been found to blunt various emotional states and evaluations, possibly through the same mechanisms by which it dulls the affective component of physical pain. However, there are limited investigations into the behavioral consequences of blunting emotions pharmacologically. Previous work has demonstrated that acetaminophen lessens risk perception and increases risk-taking in tasks with low-stakes incentives, though no studies have tested its effects in fear-inducing contexts that require evaluating safety risk. As fear pro

Training-induced brain network reorganization predicts location specificity and transfer in perceptual learning.

Visual perceptual learning is often highly specific to the trained retinal location, a hallmark observation that is traditionally interpreted as evidence that training-induced plasticity occurs in retinotopic early visual cortex. However, under properly designed training conditions, such as double training, adaptation prevention, and attention modulation, learning can transfer fully to untrained locations, challenging this classical view and raising the question of what determines whether learning remains specific or generalizes. Here, we analyzed EEG data from a Vernier learning task in which

Augmented reality and 3D visualization integrated with magnetic-assisted robotic procedures: first clinical cases using combined cutting-edge technologies.

BACKGROUND: Current minimally invasive surgery techniques face persistent challenges: limited depth perception, assistant-dependent retraction, and added invasiveness from multiple ports. We present the first clinical experience combining three novel technologies: Magnetic-Assisted Robotic Surgery (MARS), 3D visualization, and augmented reality (AR). METHODS: This IRB-approved retrospective review included 10 patients who underwent reduced-port laparoscopic surgery using the MARS system with Meta Quest 3 AR headset and EinsteinVision 3D camera in October 2024. Procedures included cholecystect

Body core compression enhances interoception leading to altered risk choices.

Interoception, the perception of internal states, is suggested to be important in decision-making, by guiding responses to external cues, based on internal states. Body compression can amplify visceral signals, and thereby potentially change interoception and adapt decision-making accordingly. Although crucial for intervening in interoceptive and decision-making dysfunctions, causal evidence of interoceptive change and its relation to decision making is lacking. In a within-subject experimental study, we investigated the effects of shapewear compression on interoception and decision-making. He
