XunZi, an AI biologist, reveals disease-modifying targets.

Hypothesis generation in biomedicine is constrained by human cognitive limitations in synthesizing insights from fragmented biomedical knowledge and multimodal data sources. Here we introduce XunZi, an AI biologist that integrates logical reasoning and multimodal data fusion to autonomously generate de novo therapeutic target hypotheses with testable mechanisms. XunZi has been trained on 24.4 million publications and 613.6 TB of multisource data spanning 21,008 human genes and 5,850 diseases, and outperforms existing methods in both accuracy and interpretability across diverse disease contexts

Multivariate patterns of plasma biomarkers predict region-specific Alzheimer's pathology and cognitive decline across independent cohorts.

INTRODUCTION: Alzheimer's disease (AD) is characterized by the progressive accumulation of amyloid-beta (Aβ) plaques, tau tangles, and neurodegeneration (ATN framework). While neuroimaging detects these changes, it is costly and not widely accessible. Blood-based biomarkers offer scalable alternatives for early detection. METHODS: We integrated plasma biomarkers, neuroimaging, and cognitive data from two large cohorts. We examined multivariate patterns of plasma Aβ42/Aβ40, phosphorylated tau (p-tau) 217, p-tau181, neurofilament light (NfL), and glial fibrillary acidic protein (GFAP) that pred

Human gloss perception reproduced by tiny neural networks.

A key goal of visual neuroscience is to explain how our brains infer object properties such as colour, curvature or gloss. Here we used machine learning to identify computations underlying human gloss judgements-traditionally considered a challenging inference. We rendered thousands of objects with varied shapes using a Ward reflectance model across lighting and viewpoints, then obtained gloss ratings for each image. Observers' judgements were consistent with one another, yet systematically deviated from reality. We compared these ratings with neural networks trained either to estimate physica

The gap in fertility care.

Patients undergoing treatment for infertility experience higher levels of psychological distress than the general population. Current research suggests that the increased stress, anxiety and depression levels in these individuals may be affecting their emotional well-being, treatment drop-out rates and clinical outcomes. To mitigate the effects of mental health symptoms for fertility patients, psychological interventions should be considered a vital part of fertility care. Treatments such as cognitive-behavioural therapy, the Mind-Body Program for Infertility and virtual mental health services

A clinician aligned vision language framework for stepwise interpretation in fundus fluorescein angiography.

Fluorescein fundus angiography (FFA) is essential for diagnosing retinal vascular diseases, yet its interpretation is expertise-intensive. Here, we present Clin-FFA-VLM, a multimodal vision-language framework that mirrors retina specialists' cognitive workflow by decomposing FFA interpretation into three stages: lesion-aware visual perception, clinical report generation, and diagnostic decision support. Trained and tested on a multi-center dataset of 13,178 FFA images with expert label, 21,717 FFA images with 1790 clinical reports and diagnosis across 7 retinal diseases, Clin-FFA-VLM achieves

Subtyping and staging of Alzheimer's disease from routine structural MRI with PHASE-AD.

Structural MRI is routinely acquired in the clinical assessment of Alzheimer's disease, yet quantitative morphometric indices derived from these scans remain largely confined to research settings. Here we present PHASE-AD - a framework that translates such indices into clinically interpretable classifications of atrophy subtype and stage that jointly capture atrophy progression while accounting for inter-individual atrophy heterogeneity. PHASE-AD is trained on MRI scans from 8,415 participants and robustly captures limbic-predominant and hippocampal-sparing atrophy subtypes that were identifie

Delivering Cognitive Behavioral Therapy for Nightmares (CBT-N) in primary care within the Veterans Health Administration: A preliminary report on clinician-perceived barriers and benefits.

Nightmares, which are associated with various biopsychosocial issues, are common and often undertreated among veterans. To address the demand for Cognitive Behavioral Therapy for Nightmares (CBT-N) in the Veterans Health Administration (VHA), experts have trained mental health clinicians in its delivery. Program evaluation data show positive clinician perceptions of CBT-N but reveal implementation barriers, including the incompatibility between CBT-N and certain practice settings. One such setting is Primary Care Mental Health Integration (PCMHI), where brief episodes of care may limit CBT-N f

Cerebro-cerebellar networks facilitate learning through feedback decoupling.

Behavioural feedback is critical for learning in the cerebral cortex. However, such feedback is often not readily available. How the cerebral cortex learns efficiently despite the sparse nature of feedback remains unclear. Inspired by recent deep learning algorithms, we introduce a systems-level computational model of cerebro-cerebellar interactions. In this model a cerebral recurrent network receives feedback predictions from a cerebellar network, thereby decoupling learning in cerebral networks from future feedback. When trained in a simple sensorimotor task the model shows faster learning a

[Accuracy of Classification of Cerebral Blood Flow Reduction Patterns Using Statistical Analysis Images Generated with Simulated SPECT Datasets via Deep Learning].

PURPOSE: The aim of this study was to evaluate the classification accuracy of specific blood flow reduction patterns in clinical images by deep learning using simulation data. METHODS: We obtained Z-score maps for 100 cases each of simulated Alzheimer's disease (AD), simulated dementia with Lewy bodies (DLB), and simulated normal cognition (NC) by performing statistical analysis of the simulation data that provided defects and healthy patient data. The clinical images were determined by reference to radiological reports, and Z-score maps of AD (n=33), DLB (n=20), and NC (n=28) were used. A ne

Interfacing brain with computer to improve communication and rehabilitation after brain damage.

Communication and control of the external environment can be provided via brain-computer interfaces (BCIs) to replace a lost function in persons with severe diseases and little or no chance of recovery of motor abilities (ie, amyotrophic lateral sclerosis, brainstem stroke). BCIs allow to intentionally modulate brain activity, to train specific brain functions, and to control prosthetic devices, and thus, this technology can also improve the outcome of rehabilitation programs in persons who have suffered from a central nervous system injury (ie, stroke leading to motor or cognitive impairment)

Transitory glutathione deficit during brain development induces cognitive impairment in juvenile and adult rats: relevance to schizophrenia.

Glutathione (GSH) metabolism dysfunction is one risk factor in schizophrenia. A transitory brain GSH deficit was induced in Wistar (WIS) and mutant (ODS; lacking ascorbic acid synthesis) rats using BSO (l-buthionine-(S,R)-sulfoximine) from post-natal days 5-16. When GSH was re-established to physiological levels, juvenile BSO-ODS rats were impaired in the water maze task. Long after treatment cessation, adult BSO-WIS/-ODS rats showed impaired place discrimination in the homing board with distributed visual or olfactory cues. Their accuracy was restored when a single cue marked the trained posi

The physiological and psychological effects of combat ration feeding during a 12-day training exercise in the tropics.

The health and psychological effects of combat ration pack (CRP) feeding during 12 days of military training in a tropical environment were investigated. Three groups of air defence guards (ADGs) received either: freshly prepared foods (fresh group, 15 mega joule (MJ), 3,600 kcal, N=13), full CRP (15 MJ, 3,600 kcal, N=10), or one-half CRP (7.5 MJ, 1,800 kcal, N=10). Underconsumption by the full CRP group resulted in CRP groups experiencing similar weight loss, protein catabolism, and immune suppression (both cell-mediated and humoral), whereas the fresh group maintained their weight and protei

A review of neuropsychological services in the United Kingdom for patients being considered for epilepsy surgery.

We report a review of the current practice of neuropsychologists working within epilepsy surgery services. The aim of the review was to examine areas of diversity and consensus across current national service provision and to examine progress in service delivery since a previous survey in 1994. Sixteen centres provided information via a questionnaire on three areas of clinical practice: pre- and post-surgery neuropsychological protocols; the intracarotid sodium amytal protocol; patient and family expectations and psychological health, in addition to examining aspects of the context of clinical

Nonverbal learning disabilities and remedial interventions.

Adolescents with nonverbal learning disabilities who enroll in private, special secondary schools consistently present a pattern of behaviors which prevents achievement of their potentials in academic areas and impedes their abilities to interact effectively with others. With weaknesses in the fine graphomotor skills for writing and poor organization at all levels, they produce limited written output and often fail to complete academic assignments. Their response to pressure to produce is to become less productive. These students perceive social situations inaccurately; they are not successful

The Society of Thoracic Surgeons was created for practicing thoracic surgeons, but the response to recertification indicates there has been a breakdown in communication between practicing thoracic surgeons and the leadership of this Society and other decision-making organizations of our specialty. To provide greater input and participation by members, it is suggested that regional representatives be elected to the Council, open discussion of issues important to thoracic surgeons be held at the Annual Meetings, small-group clinical workshops be added to the Scientific Sessions to increase oppor

A cost-effective IoT learning environment for the training and assessment of surgical technical skills with visual learning analytics.

BACKGROUND: Surgeons need to train and certify their technical skills. This is usually done with the intervention of experts who monitor and assess trainees. Nevertheless, this is a time-consuming task that is subject to variations among evaluators. In recent decades, subjectivity has been significantly reduced through 1) the introduction of standard curricula, such as the Fundamentals of Laparoscopic Surgery (FLS) program, which measures students' performance in specific exercises, and 2) rubrics, which are widely accepted in the literature and serve to provide feedback about the overall tech

Geriatric Focused ERAS Nursing Practices in Open and Hybrid Aortic Vascular Surgery.

BACKGROUND: Enhanced Recovery After Surgery (ERAS) pathways have been adapted for open and lower extremity vascular surgery and increasingly guide perioperative care for older adults. However, little is known about how perioperative nurses implement ERAS-consistent and geriatric-focused practices for older patients undergoing open and hybrid aortic procedures, particularly in hybrid operating room settings and in Chinese hospitals, where pathway maturity and geriatric integration may vary across centres. PURPOSE: To describe perioperative nurses' implementation of ERAS-consistent and geriatri

Assessing the Clinical Effectiveness of an Exergame-Based Exercise Training Program Using Ring Fit Adventure to Prevent and Postpone Frailty and Sarcopenia Among Older Adults in Rural Long-Term Care Facilities: Randomized Controlled Trial.

BACKGROUND: Frailty and sarcopenia are geriatric syndromes of increasing concern and are associated with adverse health outcomes. They are more prevalent among long-term care facility (LTCF) users than among community dwellers. Exercise, especially multicomponent and progressive resistance training, is essential for managing these conditions. However, LTCFs, particularly in rural areas, face challenges in implementing structured exercise programs due to health care professional shortages. Moreover, older adults often become bored with repetitive exercise training and may lose interest over tim

Behavioural and cognitive-behavioural interventions for outwardly-directed aggressive behaviour in people with intellectual disabilities.

BACKGROUND: Outwardly-directed aggressive behaviour is a significant part of problem behaviours presented by people with intellectual disabilities. Prevalence rates of up to 50% have been reported in the literature, depending on the population sampled. Such behaviours often run a long-term course and are a major cause of social exclusion. This is an update of a previously published systematic review (see Hassiotis 2004; Hassiotis 2008). OBJECTIVES: To evaluate the efficacy of behavioural and cognitive-behavioural interventions on outwardly-directed aggressive behaviour in people with intellec

Ketone monoester ingestion and cognitive and physical performance during exercise-heat stress.

BACKGROUND: Sustaining cognitive and physical performance under extreme environmental conditions is critical for special operation forces (SOF). Hyperthermia negatively impacts cognitive function due to reductions in cerebral blood flow, substrate availability, thermal tolerance, and increased cardiovascular strain. While ketone monoester have demonstrated beneficial effects on cognition, their efficacy during exercise-heat stress remains unexplored. METHODS: Seventeen endurance-trained males (age = 23.8 ± 5.2 y; VO2max = 58.6 ± 3.2 ml·kg-1·min-1) completed a randomized, double-blind, counter
