Surgical management of vestibular Meniere's disease with endolymphatic mastoid shunt.

The early stages of Meniere's disease may consist only of vestibular symptoms. These patients with vertigo alone may be incapacitated by this condition. A great many of these patients are unresponsive to medical therapy and tend to progress to cochleovestibular disease. Early surgery on such individuals may decrease vertigo and give the added benefit of preserving hearing. This paper reports the results of endolymphatic mastoid shunts performed on 24 patients with normal hearing who had medically uncontrolled vestibular Meniere's disease. The patients underwent surgery between 1978 and 1981, a

Endolymphatic hydrops. An overview and classification.

Endolymphatic hydrops (EH) is a pathological condition which is the final common manifestation of a variety of otologic insults. In this paper we develop a classification which, on the basis of clinical and pathological data, distinguishes symptomatic and asymptomatic forms. Clinical case histories and temporal bone studies are presented to illustrate and substantiate this classification. The symptomatic form becomes evident by the hallmark symptoms of episodic vertigo and fluctuating hearing loss. The asymptomatic form is silent. Interconversion from one form to another may occur over time. E

Ototoxicity of netilmicin.

Ototoxicity and efficacy of netilmicin was evaluated in 76 consecutive patients with moderate to severe infections with Gram-negative rods and Staphylococcus aureus. Netilmicin alone was efficacious in 47 patients and in another 23 patients cure or improvement was seen with netilmicin combined with another antibiotic, usually penicillin. Therapeutic failure was seen in 4 cases, 2 of which were regarded as netilmicin failures. Vestibular function and hearing acuity was thoroughly examined by electronystagmography and audiometry. In spite of the extensive examination of the audiovestibular funct

Comparison of subjective symptomatology and responses to harmonic acceleration in patients with Menière's disease.

Vestibular function testing employing caloric stimulation has been of limited value in the evaluation of patients with Menière's disease, with its cardinal shortcoming being poor repeatability. This modality has not been useful in determining appropriate treatment for these individuals. We have used low frequency harmonic acceleration to study patients with vestibular pathology and have noted a high level of repeatability of responses to this type of stimulation. This study was designed to determine if the clinical status of patients with Menière's disease could be correlated with their respon

Quantitative study of Scarpa's ganglion and vestibular sense organs in endolymphatic hydrops.

The density of vestibular hair cells and the number of neurons in Scarpa's ganglion were estimated for 11 temporal bones with endolymphatic hydrops. The ten subjects from which these bones were taken all exhibited decreased caloric response (when tested), fluctuating hearing loss, and episodic vertigo. The degeneration of vestibular sense organs was found within the "normal" range for all except one case, which showed total degeneration of the posterior crista ampullaris. Ganglion cell counts in all cases were low. In three of the ten subjects, counts fell below the lowest values seen in a sam

Histopathology of Endolymphatic subarachnoid shunt surgery for Meniere's disease.

The histopathological findings in the temporal bones of three patients who underwent endolymphatic subarachnoid shunt operation for Meniere's disease are reported. The postoperative follow-up period varied from three to five years. Two patients had a successful operation; the third was unsuccessful. The continuity between th mesothelial lining of the endolymphatic sac and the subarachnoid space was demonstrated in one case; no definite conclusions could be made for the other two cases. Although failure of shunt surgery was presumably due to fibrosis in the shunt area in the reported case, the

Ultrastructural development of the early rat otocyst.

The ultrastructural development and differentiation of cells forming the rat otocyst were studied from the 9th to the 13th postcoital day (PCD). The earliest stage investigated was a simple ovoid structure with a connecting stalk to the surface ectoderm. A process of programmed cellular death involving surface ectoderm, connecting stalk, and lateral otocyst wall rapidly detached the otocyst. The cells forming the otocyst were roughly columnar, the organelles were polarized; mitochondria occurred in greatest number basally and in the supranuclear area; Golgi membranes when present were supranuc

Current status of surgical decompression and drainage procedures upon the endolymphatic system.

At the present time, neither the etiologic factors nor the pathophysiologic mechanisms that lead to the development of endolymphatic hydrops or Ménière's disease are known; however, the anatomic-pathologic alterations in the labyrinth that are produced by the disease are well-documented. Undoubtedly, what is not known is responsible for the controversial status and questionable benefit that surgical decompression and drainage procedures have upon the endolymphatic system.

Surgical management of Ménière's disease with the endolymphatic subarachnoid shunt: A five-year study.

The majority of individuals with Ménière's Disease can be successfully managed by medical means; however, there are some patients who become refractory to this form of therapy. It is this group of patients who are candidates for some type of surgical procedure. This paper reviews 109 patients who underwent 112 endolymphatic subarachnoid shunt procedures for the control of their Ménière's Disease. Medical therapy, indications for surgery, surgical technique, results, and complications are covered in detail.

Cytomegalovirus endolabyrinthitis.

A premature male infant, who died 22 days after birth with hyaline membrane disease, was found to have had cytomegalic inclusion disease at autopsy. Histopathologic examination of the temporal bones showed cytomegalovirus (CMV) infection of the entire endolabyrinth without involvement of the neural and sensory structures. These findings support the thesis that late gestational or perinatal fetal CMV infection results in an endolymphatic labyrinthitis. We hypothesize that blood-borne virus passes from the stria vascularis into the endolymphatic spaces and infects the nonneurosensory epithelium.

Vertigo of delayed onset after sudden deafness.

An entity of episodic true vertigo of delayed onset following sudden and profound sensorineural hearing loss is described. Data on 12 patients and three case reports are presented. The latency between sudden deafness and the onset of the vertigo varied from 1 to 68 years. The vestibular symptoms are identical to the vestibular symptoms of Ménière's disease, and there is some evidence that endolymphatic hydrops in the previously deafened ear represents at least part of the labyrinthine pathology. Labyrinthectomy in the deaf ear was curative. Tentatively, this entity is best considered a variant

Genetic Profiles and Three-year Follow-up Study of Chinese Males With Congenital Hypogonadotropic Hypogonadism.

BACKGROUND: The correlation between long-term treatment outcomes with genotypes in congenital hypogonadotropic hypogonadism (CHH) males is rarely reported. AIM: To investigate the correlations among genotypes, phenotypes, and treatment outcomes for CHH male patients. METHODS: Whole exome sequencing was performed for 73 Chinese CHH males from one academic center. Patients self-selected one of the 4 treatments: pulsatile Gonadorelin pump (PGP), cyclical gonadotropins therapy (CGT), human menopausal gonadotropin monotherapy, or testosterone replacement treatment. Clinical assessments were perfo

Evolutionary tuning of an auditory transduction channel.

TMC1 and TMC2 are mechanosensory ion channels of the vertebrate inner ear that mediate hearing and balance. How these channels open in response to mechanical force remains unresolved. Through comparative analyses of TMCs across eukaryote species, we find that TMC1 and TMC2 arose in vertebrates by gene duplication and evolved elaborate extracellular loops. Structural models demonstrate that the loop between transmembrane domains 1 and 2 arches over the channel pore and lies near TMIE, an auxiliary protein essential for function. In mammals, this loop shows signatures of positive selection and c
