Limb lengthening and deformity correction of congenital and acquired deformities in children using the Taylor Spatial Frame.

Background and purpose - Hexapod ring fixators such as the Taylor Spatial Frame (TSF) have shown good outcomes. However, there have only been a few studies comparing the use of TSF with various etiologies of the deformity. We compared the use of TSF in congenital and acquired deformities in children. Patients and methods - We reviewed 213 lower extremity reconstructive procedures with the TSF in 192 patients who were operated between October 2000 and October 2015. 128 procedures (67 proximal tibiae, 51 distal femora, and 10 distal tibiae) in 117 children (median age 14 (4-18) years; 59 girls)

The distraction osteogenesis in midfacial hypoplasia.

Distraction osteogenesis (DO) can generate new bone in a gap between 2 vascularized bone surfaces in response to application of graduated tensile stress across the bone gap. The authors present the clinical result in a cleft patient with severe maxillary deficiency treated by a rigid external distraction (RED) device. A boy complained of both masticatory and psychological problems because of cleft with severe midfacial retrusion. The treatment aimed to create a well-balanced facial profile, increase maxillary incisal display, create proper overjet and overbite, and align his dentition. By the

An evaluation of halo pin insertion torque on outer table penetration in elderly patients.

STUDY DESIGN: An experimental anatomic study performed on elderly cadaveric skulls. OBJECTIVES: (1) To determine the pin penetration depths in outer table of skull at different torques in the elderly population during halo pin insertion and (2) to validate a safe range of torque for use in this population. SUMMARY OF BACKGROUND DATA: The elderly are at an increased risk of falls, which can lead to cervical fractures. The halo pins used to stabilize these injuries present unique problems in this population owing to osteoporosis, and intracranial pin penetration should always be avoided. METH

Reconstruction and limb salvage using a free vascularised fibular graft for periacetabular malignant bone tumours.

We treated four patients with periacetabular malignant tumours by pelvic reconstruction with a free vascularised fibular graft after resection of the tumour. The mean follow-up period was 32 months (9 to 39). The diagnosis was chondrosarcoma in three patients and osteosarcoma in one. In two patients total resection of the hemipelvis was required and in the other two less, but still massive, resection was undertaken. All were treated with an immediate free vascularised fibular graft which included arthrodesis of the hip and reconstruction of the pelvic ring. One patient died. The other three ha

[Injuries to the craniocervical junction].

Injuries to the upper cervical spine (C0-C2) play a major role in surgical treatment of traumatic sequelae in the entire cervical spine. Even though the number of such operations has increased in recent years, there are no clear treatment recommendations for most types of cervical spine injuries. In view of the wide range of injury types and the correspondingly large number of treatment options, this review focuses mainly on the following types of injuries: C0 fractures, occipital condyle fractures (OCF), atlanto-occipital dislocation (AOD), atlas fractures, atlantoaxial dislocation (AAD), and

Effects of fibular strut augmentation for the open reduction and internal fixation of proximal humeral fractures: a systematic review and meta-analysis.

BACKGROUND: There is still a lack of remarkable consensus regarding the clinical efficacy of the application of fibular strut augmentation (FSA) combined with a locking plate for proximal humeral fractures. A systematic review and meta-analysis to assess the effect of the use of FSAs in open reduction and internal fixation of proximal humeral fractures was conducted. METHODS: A literature search was conducted in PubMed, Embase, Cochrane, Web of Science Core Collection, and ClinicalTrials.gov to identify trials that compared the clinical outcomes of proximal humeral fractures treated using a l

Long-term retrospective study of patients with idiopathic clubfoot treated with posterior medial-lateral release.

BACKGROUND: It has been demonstrated that an important component of clubfoot deformity is related to pathologic external rotation of the talus with respect to the remainder of the foot. The purpose of the present study was to review the long-term results for a cohort of patients with idiopathic clubfoot who were managed by a single surgeon with a uniform surgical protocol consisting of extensive posterior medial-lateral release in addition to the use of a temporary Kirschner wire to derotate the talus prior to fixation. METHODS: Eighty patients (120 clubfeet) with idiopathic clubfoot without

Hallux valgus and first ray mobility: a cadaveric study.

BACKGROUND: Several studies have demonstrated that patients with hallux valgus (HV) deformities have increased first ray sagittal mobility. However, the change in mobility that occurs after surgical correction of HV deformities has not been extensively evaluated. This study was done to determine if surgical realignment of the first ray in cadaver specimens with a proximal crescentic osteotomy and distal soft tissue reconstruction (DSTR) would reduce the first ray sagittal motion as measured with an external-type micrometer (the Klaue device). METHODS: Twelve fresh-frozen below-knee cadaver sp

Arthroscopic Bankart suture repair for traumatic anterior shoulder instability: analysis of the causes of a recurrence.

Eighty-two patients with traumatic anterior shoulder instability were treated with an arthroscopic transglenoid multiple suture technique (Caspari's method) and followed-up for more than 2 years. A retrospective analysis of the clinical outcome was performed to determine the factors related to poor results. The mean age at operation was 21 years (range, 13 to 50 years) and the mean follow-up period was 40 months (range, 24 to 70 months). According to the status of the ligament-labrum complex and the glenoid bone defect, the Bankart lesions were classified into five types arthroscopically. Ther

A Modified Self-Growing Rod Technique for Treatment of Early-Onset Scoliosis.

UNLABELLED: Surgical treatment of early-onset scoliosis (EOS) remains challenging as no definitive surgical technique has emerged as the single best option in this varied patient population1-3. Although the available surgical techniques may differ substantially, they all share the same goals of achieving and maintaining deformity correction, allowing physiological spinal growth, and reducing the number of operations and complications. Herein, we present a modified self-growing rod technique that represents a valid alternative to the existing surgical procedures for EOS. DESCRIPTION: The patie

Where we look, what we know: how hippocampal predictions shape, and are shaped by, eye movements.

As we interact with our environment, information from hippocampal representations guides further cognition and action on a moment-to-moment basis. We propose that eye movements modulate the organization of neural dynamics in humans and other primates with advanced visual systems, allowing reciprocal encoding and retrieval of information in service of predictive action. Gaze fixations and saccades allow the hippocampus to generate, test and update predictions about the external world in real time. Integrating evidence from computational modelling, neurophysiology and neuroimaging, we highlight
