Modified Blair tibiotalar arthrodesis for post-traumatic avascular necrosis of the talus: a case report.

Surgical treatment of post-traumatic avascular necrosis of the talus coupled with collapse often results in limited treatment options. Of those options, the Blair tibiotalar arthrodesis has been beneficial in preserving limb length and subtalar motion. The complications associated with Blair tibiotalar arthrodesis have led to modifications to improve stability and functional outcomes with rigid internal fixation. We present the case of a 29-year-old female with a history of an open fracture dislocation of the talus 10 years previously, with subsequent development of avascular necrosis of the t…

Indications for anterior lumbar interbody fusion.

Anterior lumbar interbody fusion (ALIF) has become a widely recognized surgical technique for degenerative pathology of the lumbar spine. Spinal fusion has evolved dramatically ever since the first successful internal fixation by Hadra in 1891 who used a posterior approach to wire adjacent cervical vertebrae in the treatment of fracture-dislocation. Advancements were made to reduce morbidity including bone grafting substitutes, metallic hardware instrumentation and improved surgical technique. The controversy regarding which surgical approach is best for treating various pathologies of the lum

Interosseous membrane reconstruction for forearm longitudinal instability.

Forearm longitudinal instability results from an axial load to the forearm with fracture or dislocation of the radial head and disruption of the interosseous membrane and that of the distal radioulnar joint. Patients may present in the acute or chronic setting with radioulnar instability as manifested by persistent or new forearm discomfort and wrist and elbow pain. Reconstruction of the interosseous membrane has been described to restore longitudinal forearm stability. We describe reconstruction of the central band of the interosseous membrane with a bone-patellar tendon-bone graft. This proc

Current concepts in the management of complex elbow trauma.

Complex instability of the elbow, also known as fracture dislocation of the elbow, is one of the most challenging injuries of the musculoskeletal system. A clear understanding of the biomechanics of the joint with special focus on the relative contributions of the articular components and the ligamentous constraints is critical to the proper management of these injuries. The accurate recognition and implications of associated injuries offers a rationale for approaching these difficult problems by appropriately addressing the elements of the trauma. Further, doing so in a timely fashion enhance…

Knee locking secondary to osteochondral fracture of the patella: an unusual presentation.

Acute locking is a sign of internal derangement of the knee and is an orthopaedic surgical emergency. Differential diagnoses of this condition are torn meniscus, intra-articular loose bodies and torn anterior cruciate ligament. We present a 15-year-old schoolboy who developed acute locking of the knee during dancing. Arthroscopy revealed that he had sustained an osteochondral fracture of the patella and the loose fragment from it caused locking of the knee. That this was not pseudo-locking was confirmed by examination of the knee under anaesthesia. Locking of the knee by a loose osteochondral

Vertebral artery anomaly with entry at C4--avoiding a surgical pitfall: a case report.

We present a case of an unusual course of the vertebral artery (VA) with intra-foraminal entrance at C4. A patient with traumatic fracture of C3 and C4 with dislocation C3/C4, spinal cord compression and ensuing quadriplegia presented with unilateral entrance of the VA at C4 detected on preoperative magnetic resonance imaging (MRI). The patient was surgically decompressed and stabilized by an anterior-posterior approach without intra-operative complications. Apart from anatomical findings no clinical case of entrance of the VA at C4 had been described in recent clinical literature. A physiolog

Management of neuropathic injury to the tarsal navicular.

The goal of treatment is to have a foot that will fit in a shoe or brace without likelihood of skin pressure and ultimate ulceration. Surgery is indicated in the acute and chronic phases of neuroarthropathy related to navicular fracture of dislocation. For patients who have acute injury with deformity that threatens the skin or cannot be maintained in a stable position without manipulation in a boot or cast, surgery with arthrodesis is recommended. Patients who subsequently develop deformities that cannot be managed successfully with extra-depth custom shoes or braces, and who are not candidat

Degenerative sternoclavicular arthritis and hyperostosis.

Symptomatic arthritic involvement of the sternoclavicular joint is relatively uncommon and can be a result of distant trauma, infection, and sternocostoclavicular hyperostosis, post-menopausal arthritis, condensing osteitis of the proximal clavicle, or secondary to an underlying arthropathy. Patients with degenerative osteoarthritis due to trauma most commonly have had either an anterior or posterior dislocation, subluxation, or periarticular fracture. Medical claviculectomy with or without ligamentous stabilization is indicated only in situations of painful primary and secondary rheumatoid ar

A single-stage combined surgical approach for vertebral resections.

Five patients with a destructive vertebral lesion (infectious or tumorous) in the thoracolumbar or lumbar spine were treated with single-stage resection, anterior spine fusion, and posterior Harrington distraction instrumentation. Twelve patients with a fracture-dislocation were also so treated. Correction of vertebral instability and deformity and early mobilization of the patient were the advantages realized. Of the seventeen patients, ten with an incomplete neural deficit improved, three of them to complete recovery. The conditions of five patients with traumatic, complete paraplegia remain

Constrained acetabular liner total hip arthroplasty versus conventional hemiarthroplasty for unstable comminuted femoral intertrochanteric fractures in elderly patients with neurological disorders.

BACKGROUND: To evaluate whether constrained acetabular liners (CAL) used in total hip arthroplasty (THA) reduce the risk of dislocation and related complications in patients with neuromuscular disorders and unstable comminuted femoral intertrochanteric fractures (ITF). METHODS: This retrospective cohort study included patients diagnosed with Evans type III-V ITFs who underwent hemiarthroplasty or THA at our institution from July 2011 to July 2016. Patients were divided into two groups: a hemiarthroplasty group and a CAL group. Radiological data, clinical records, and laboratory test results w

Creation of a replicable anatomic model of terrible triad of the elbow.

BACKGROUND: Terrible triad of the elbow (TTE) is a complex dislocation associating radial head (RH) and coronoid process (CP) fractures. There is at present no reproducible anatomic model for TTE, and pathophysiology is unclear. The main aim of the present study was to create and validate an anatomic model of TTE. Secondary objectives were to assess breaking forces and relative forearm rotation with respect to the humerus before dislocation. METHODS: An experimental comparative study was conducted on 5 fresh human specimens aged 87.4 ± 8.6 years, testing 10 upper limbs. After dissection conse

The "sliver sign": a specific radiographic sign of acute lateral patellar dislocation.

OBJECTIVE: The objective of this work is to assess the prevalence of the sliver sign, defined as an intraarticular linear or curvilinear ossific density, in association with knee effusion in patients with acute knee trauma, as a predictor of recent lateral patellar dislocation (LPD). MATERIALS AND METHODS: A retrospective radiology database search for the term 'patellar dislocation' on MRI knee exams performed at our institution over a 7-year period identified 216 studies. Of these, 142 exams met true positive gold standard diagnostic criteria for LPD. Imaging findings of both the retrospecti

The combined talus and sustentaculum fracture: A case series.

BACKGROUND: The sustentaculum tali is a biomechanically important stabilizer of the hindfoot and contributes to articular congruency of the subtalar joint. Sustentaculum injury associated with a talus fracture has been described infrequently and treatment of this combined injury varies. The purpose of this study was to describe and evaluate the outcomes of the combined talus and sustentaculum fracture. METHODS: Retrospective chart and radiographic review was performed on all talus fractures (n = 436) requiring operative fixation over a 21-year period at a single Level-1 trauma center. All tal

Pediatric patients with dog bites presenting to US children's hospitals.

BACKGROUND: To characterize pediatric dog bite injuries presenting to US children's hospitals and identify factors associated with clinically significant injuries. METHODS: We performed a multicenter observational study from 26 pediatric hospitals between July 1, 2010, and June 30, 2020, including patients ≤ 18 years with dog bites, consolidating together encounters from patients with multiple encounters within 30 days as a single episode of care. We characterized diagnoses and procedures performed in these patients. We used generalized linear mixed models to identify factors associated with

The Price of Poor Cementing: A Case Report on Chronic Hip Dislocation and Revision.

INTRODUCTION: Hemiarthroplasty is commonly performed for displaced femoral neck fractures in elderly patients. While generally effective, poor cementing techniques can lead to serious complications, including implant migration and chronic dislocation. Cement interposition in the acetabulum is a rare but preventable cause of persistent instability. This case emphasizes the importance of meticulous cement handling and intraoperative vigilance to avoid avoidable revision surgeries. CASE REPORT: A 76-year-old woman presented 8 months after left hemiarthroplasty with persistent hip pain and immobi

Surgical technique: The capsular arthroplasty: a useful but abandoned procedure for young patients with developmental dysplasia of the hip.

BACKGROUND: Codivilla in 1901, Hey Groves in 1926, and Colonna in 1932 described similar capsular arthroplasties--wrapping the capsule around the femoral head and reducing into the true acetabulum--to treat completely dislocated hips in children with dysplastic hips. However, these procedures were associated with relatively high rates of necrosis, joint stiffness, and subsequent revision procedures, and with the introduction of THA, the procedure vanished despite some hips with high functional scores over periods of up to 20 years. Dislocated or subluxated hips nonetheless continue to be seen

Femoral Stem Dislocation Caused by Trunnionosis Along with Adverse Local Tissue Reaction: A Case Report and a New Technique of Head to Cone Cementing.

In total hip replacement (THR), fretting and corrosion at the modular head-neck junction (trunnionosis) may cause adverse local tissue reaction (ALTR). In this report, we presented a 34 years woman with a history of THR eight years ago, presenting with acute pain and limping. The radiographic assessment revealed stem-head dislocation for which a revision hip surgery was planned. Surprisingly, we observed pseudotumor and tissue necrosis resulting from the body's reaction to cobalt-chromium alloy. The revision surgery entailed pseudotumor debridement and replacing the femoral head with a new met

Extensor Mechanism Disruption in Knee Dislocation.

Disruption of the knee extensor mechanism is a challenging injury with no clear consensus on optimal treatment. Although rare in the setting of knee dislocations, these injuries should not be overlooked. Acute, complete rupture of either the quadriceps or patellar tendon necessitates primary repair with or without augmentation. Surgical management may also be required in the setting of a partial tear if a significant extensor lag is present or nonoperative treatment has failed. Tendon augmentation is used during primary repair if the native tissue is inadequate or after a failed primary repair

Long-term outcome of reconstruction of the hip in young children with cerebral palsy.

We reviewed the long-term radiological outcome, complications and revision operations in 19 children with quadriplegic cerebral palsy and hip dysplasia who underwent combined peri-iliac osteotomy and femoral varus derotation osteotomy. They had a mean age of 7.5 years (1.6 to 10.9) and comprised 22 hip dislocations and subluxations. We also studied the outcome for the contralateral hip. At a mean follow-up of 11.7 years (10 to 15.1) the Melbourne cerebral palsy (CP) hip classification was grade 2 in 16 hips, grade 3 in five, and grade 5 in one. There were five complications seen in four hips (
