Eutectic mixture of local anesthetics in adult urology patients: an observational trial.

BACKGROUND AND OBJECTIVES: The effectiveness of EMLA eutectic mixture of local anesthetics, (ASTRA Co, France) cream in minor surgery on the penis and its acceptability in unpremedicated outpatients were assessed. METHODS: EMLA cream was applied 1 hour before surgery (fremulum plasty, circumcision or dorsal section for phimosis, and condyloma accuminatum) in addition to a subcutaneous infiltration of lidocaine 1%, just before incision in cases of circumcision. Verbal Rating Scale (VSR) was assessed during the surgery and the acceptance 15 days later by a questionnaire. RESULTS: Thirty-two pa

Incidence of deep vein thrombosis following vascular surgery.

OBJECTIVE: The incidence of deep vein thrombosis (DVT) has been well documented in general and orthopaedic surgery but there have been few reports of the incidence of DVT following vascular surgery. Our aim was to perform a prospective study of the incidence of DVT in patients managed on the Vascular Unit at our institution. EXPERIMENTAL DESIGN: One hundred and forty two consecutive patients presenting for repair of abdominal aortic aneurysm, reconstruction of lower extremity arterial occlusive disease or amputation were studied. Venous duplex ultrasound examination was performed preoperative

Corneal topography in cataract surgery.

Keratometry and corneal topography remain the most important means of evaluating induced corneal changes after surgery and have comparable sensitivities in the paracentral region of the cornea. However, keratometry gives no information about the peripheral cornea or about asymmetry of the cornea. Videokeratography should be performed after cataract surgery in cases in which best-corrected visual acuity is not adequate and there are no other obvious causes for poor vision to determine whether corneal irregularities are present. The recent literature on corneal topographic evaluation of induced

Adrenaline, cyclic AMP and potassium during general anaesthesia with and without epidural analgesia.

Twenty patients undergoing abdominal surgery under general anaesthesia were studied to determine whether beta 2-adrenergic receptor sensitivity and adrenaline-induced hypokalaemia are related to preceding adrenergic stress. Half of the patients were given epidural analgesia with bupivacaine-adrenaline before starting surgery and then a booster dose after 60 min of surgery. The others were given only the epidural dose of bupivacaine-adrenaline at 60 min. Despite marked increases in the plasma adrenaline concentration after the intra-operative epidural dose, there was no decrease in the serum po

[Antibiotic prophylaxis in Danish orthopedic alloplastic surgery].

In order to assess the routine use of prophylactic antibiotics (AB) in arthroplastic surgery in Denmark, questionnaires were sent to all Danish orthopaedic departments and all general surgical departments that perform orthopaedic surgery. Fifty-six departments (93%) returned the questionnaires. All departments use prophylactic AB in primary knee and hip arthroplasty and in revision arthroplasty. In addition, all departments but one use prophylactic AB in arthroplasty secondary to osteosynthesis. The largest group of departments uses penicillinase-resistant penicillin (PRP) in their standard pr

Paediatric emergencies.

During the last decade neonatal surgical results have improved considerably. Except for infants born with serious congenital heart disease, diaphragmatic hernia or exomphalos, postoperative mortality rates for infants with single anomalies have fallen to the region of 10%. This dramatic success story has been marred by a corresponding increase in the number of individuals with several anomalies entering late childhood with severe chronic handicaps. During the remainder of this century much effort will be expended in devising programmes of investigation which will attempt to predict which indiv

Clinical and pathologic review of 48 cases of chordoma.

The results of treatment of 48 patients with the diagnosis of chordoma during the period 1931 to 1981 at the Massachusetts General Hospital were reviewed. Fourteen patients were treated with surgery alone: eight patients with primary tumors in the sacrococcygeal region were treated with radical surgery and four are alive with no evidence of disease (NED) with follow-up of 8 to 20 years. Recurrent tumors in six patients were treated with surgery alone resulting in long palliation (3-25 years). The actuarial survival rate at 5 years for all patients treated with surgery was 76%. Nationwide Burden and Hospital Costs of Surgical Site Complications in France: A Retrospective Analysis of the French National PMSI Administrative Database.

IntroductionSurgical site complications (SSCs) such as surgical site infections (SSIs) cause significant morbidity and mortality for patients and raise costs. Our goal was to estimate the frequency and cost of SSCs (including SSIs) in France for orthopedic, cardiac, digestive (gastrointestinal), and bariatric surgeries including costs of index admission and readmissions due to SSCs.MethodsWe used a national hospital database and included the prevalence of SSCs for patients admitted in 2018. Patients were categorized by type of SSC (infection related to surgery; infection associated to surgery;

Robot-assisted resection of abdominal lymphatic malformations in children: a novel minimally invasive strategy for complex lesions.

Robotic-assisted surgery (RAS) represents a promising emerging alternative to conventional laparoscopic surgery (LAS) for abdominal lymphatic malformations (ALMs). We aimed to evaluate the safety and efficacy of robotic-assisted resection of ALMs in pediatric patients. A retrospective analysis was performed on 134 children with ALMs who underwent surgery from December 2018 to December 2025 at the Department of General Surgery, Children's Hospital, Zhejiang University School of Medicine. Patients were stratified into two groups according to the surgical modality: the robot-assisted surgery (RAS…

Circulating Brain Injury Biomarkers: A Novel Method for Quantification of the Impact on the Brain After Tumor Surgery.

BACKGROUND: Clinical methods to quantify brain injury related to neurosurgery are scarce. Circulating brain injury biomarkers have recently gained increased interest as new ultrasensitive measurement techniques have enabled quantification of brain injury through blood sampling. OBJECTIVE: To establish the time profile of the increase in the circulating brain injury biomarkers glial fibrillary acidic protein (GFAP), tau, and neurofilament light (NfL) after glioma surgery and to explore possible relationships between these biomarkers and outcome regarding volume of ischemic injury identified wi

Factors Affecting Surgical Site Infection Rates in Hepatobiliary Surgery.

BACKGROUND: There are several studies regarding risk factors affecting surgical site infections (SSIs); nevertheless, there are an insufficient number of studies focusing on risk factors for SSI in hepatobiliary (HPB) surgery. In this study, we aimed to determine risk factors related to HPB surgery. METHODS: A total of 1,418 patients were included in this study, all of whom underwent hepatobiliary system surgery in a five-year period between January 2005 and December 2009. Demographic data, patient- and surgery-related risk factors, and laboratory parameters were analyzed retrospectively from

Influence of cirrhosis in cardiac surgery outcomes.

Liver cirrhosis has evolved an important risk factor for cardiac surgery due to the higher morbidity and mortality that these patients may suffer compared with general cardiac surgery population. The presence of contributing factors for a poor outcome, such as coagulopathy, a poor nutritional status, an adaptive immune dysfunction, a degree of cirrhotic cardiomyopathy, and a degree of renal and pulmonary dysfunction, have to be taken into account for surgical evaluation when cardiac surgery is needed, together with the degree of liver disease and its primary complications. The associated patho

[Arthroscopic surgery].

Although arthroscopy of the knee joint had already been reported during the 1930's, the general dissemination of this method first began in the 1970's. The main reason for the rapid dissemination of this method was especially the fact that in addition to diagnostics, therapeutic possibilities were recognized and immediately implemented. This meant that arthroscopy had great potential and was made well known since the surgery was minimally invasive. Today we can assume that the technological side of the arthroscopic method is very widely developed and new innovations only arise slowly. Innovati

Multifocal carcinoma of the larynx presenting as subglottic carcinoma.

The results of treating primary subglottic carcinoma (PSC) with radiation or surgery are disappointing, and recurrent disease is common. The purpose of this study was twofold: first, to present our experience of treating two multifocal patients with PSC and second, to report our treatment results using two different clinical treatment sequences. Between September 1991 and June 2002, we treated five patients with primary squamous cell carcinoma of the subglottis at the Tri-Service General Hospital, Taipei, Taiwan. It constitutes 5.2% of all 96 laryngeal cancers. Two patients were treated with p
