Management of chronic leg and knee pain following surgery or trauma related to saphenous nerve and knee neuromata.

Patients who present with lower extremity pain following surgery or trauma can occasionally have the saphenous nerve as the offending anatomic cause of their pain. Consistent with its anatomic course, the saphenous nerve can be the source of pain that manifests anywhere along its sensory distribution.Patients who presented to the Georgetown Peripheral Nerve Institute with lower extremity pain were evaluated, and those patients whose pain was suspected to be of saphenous nerve origin were offered surgical treatment. The surgical intervention included excision of the neuroma and/or nerve decompr

Amputations and prosthetics.

The author presents in a condensed way an overview of the principles of limb amputations and further treatment of patients who underwent such a procedure. The metabolic cost of walking, load transfer, and wound healing are reviewed in a concise manner. Particular attention is given to blood supply to the wound and methods to determine adequate perfusion with a clear analysis of the pro and cons of the Doppler method. Pediatric amputations, because of their specificity, are considered apart. Disarticulation of limbs is the method of choice in children, because of it retains growth potential of

Reconstruction of soft tissue and bone defects in lower extremity with free flaps.

This article reports our experiences treating soft tissue and bone defects in the lower extremity with free flaps. One of the most common causes for lower extremity wounds are high-energy injuries. These kinds of injuries contain soft tissue and bone defects beyond neurovascular complications. The rate of infection and nonunion is very high in these injuries. Between 1988 and 1996, we applied 33 flaps to 28 patients. The numbers and kinds of the free flaps are as follows: 12 latissimus dorsi, nine radial forearm, seven lateral arm, three vascularised fibula grafts with skin, one gracilis, and

[The optimization of bone repair in open comminuted fractures of the long bones].

Clinical and experimental researches were conducted to study the possibilities for enhancing the efficiency of treatment of open composite comminuted and crushed fractures of long bones with the help of repercussive action of electrets. The results of roentgenomorphometric, clinical and biomechanical researches have shown the efficiency of electrets that could shorten the healing time of the infected wounds, stop the inflammatory reaction and promote the repairing process of bones. The application of electrets using the repercussive methods in complex treatment of complicated open fractures in

[Radionuclide studies on healing in closed transverse fractures of the long tubular bones].

A total of 86 patients were examined by 99mTc-pyrophosphate in the course of therapy using the transosseous synthesis method. An elevated accumulation of the radiopharmaceutical was observed since the first day following a trauma. Maximum enhancement of metabolic processes in the absence of fragment displacement was noted on the 8th week and in the lack of displacement on the 9th-10th week. Then the intensity of bone formation got weaker. It was found appropriate that G. A. Ilizarov's apparatus should be removed exactly at that time. The content of mineral components in the regenerate was over

Can the Y balance test identify those at risk of contact or non-contact lower extremity injury in adolescent and collegiate Gaelic games?

OBJECTIVES: Lower extremity (LE) injuries are common in Gaelic games and lead to a significant economic and injury burden. Balance is considered a predictor of injury in other sports, however no research has examined its effect on LE injury in Gaelic games. This study aims to present normative data for the Y Balance Test (YBT), determine whether the YBT can identify those at risk of contact and non-contact LE and ankle injuries and generate population specific cut-off points in adolescent and collegiate Gaelic games. DESIGN: Prospective cohort study. METHODS: A convenience sample of 636 male

Effectiveness of the combat application tourniquet for arterial occlusion in young children.

BACKGROUND: Tourniquet use for extremity hemorrhage has become a mainstay in adult trauma care in last 15 years. The efforts of the Stop the Bleed campaign have increased the distribution and use of tourniquets in civilian settings in response to mass shootings and as part of disaster preparedness. Little research or published experience exists regarding the use of tourniquets in the pediatric population. This study sought to determine the minimum patient age on which the combat application tourniquet (CAT) is able to control extremity hemorrhage. METHODS: A convenience sample of pediatric pa

Patient-Reported Pain Outcomes for Children Attending an Emergency Department With Limb Injury.

OBJECTIVES: The aim of this study was to describe patient-reported pain outcomes at various stages of an emergency department (ED) visit for pediatric limb injury. METHODS: This prospective cohort consisted of 905 patients aged 4 to 17 years with acute limb injury and a minimum initial pain score of 4/10. Patients reported pain scores and treatments offered and received at each stage of their ED visit. Multiple logistic regression was used to identify predictors for severe pain on initial assessment and moderate or severe pain at ED discharge. RESULTS: The initial median pain score was 6/10

The relationship between patient factors and the refusal of analgesics in adult Emergency Department patients with extremity injuries, a case-control study.

Background and aims Previous studies have described the phenomenon of oligo-analgesia in Emergency Department patients with traumatic injuries, despite the high prevalence of pain among these patients. Besides aspects related to health care staff, patient related factors might also play a role in suboptimal pain treatment, however evidence is scarce. Therefore, the objective of the current study was to evaluate patient related factors in adult patients refusing offered analgesics during an Emergency Department presentation with extremity injuries. Methods This was a case control study in the E

Rehabilitation after plate fixation of upper and lower extremity fractures.

Post-operative rehabilitation and weight-bearing protocols are important to fracture fixation outcomes, yet there is a dearth in the literature concerning universal treatment guidelines following plate fixation of extremity fractures. There are controversies regarding time to allow weight-bearing and range of motion for most fractures of the upper and lower extremity. This lack of a consensus has led to varying practice guidelines and differing anecdotal protocols between treating surgeons. This review attempts to establish consensus guidelines for the post-operative rehabilitation required fo

The diagnostic and prognostic value of ultrasonography in soccer players with acute hamstring injuries.

BACKGROUND: An injury to the hamstring muscle complex is the most common injury in soccer. Ultrasound of acute hamstring injuries is often used as a clinical tool for diagnosing hamstring injuries and guiding players in when they can return to play. PURPOSE: To (1) investigate the characteristic sonographic findings of acute hamstring injuries in soccer players, (2) compare the mean injury severity (time to return to play) in injured players with and without sonographically verified abnormalities, and (3) correlate the length of the injured area and absence from soccer play (time to return to

Intranasal sufentanil for the treatment of acute pain in a winter resort clinic.

INTRODUCTION: Painful extremity injuries are common patient complaints in resort clinics, urgent care clinics, and emergency departments. We hypothesized that intranasal (IN) sufentanil could provide rapid, noninvasive, effective pain relief to patients presenting with acute extremity injuries. METHODS: This was an unblinded, nonrandomized, observational study that enrolled a convenience sample of patients presenting to a university-affiliated ski clinic with acute moderate to severe pain associated with a traumatic injury between the months of January and March 2011. Patients were excluded i

Symptoms and time to medical care in children with accidental extremity fractures.

BACKGROUND AND OBJECTIVE: Delay in seeking medical care is one criterion used to identify victims of abuse. However, typical symptoms of accidental fractures in young children and the time between injury and the seeking of medical care have not been reported. We describe patient and injury characteristics that influence the time from injury to medical care. METHODS: Parental interviews were conducted for children <6 years old with accidental extremity fractures. Demographic characteristics, signs and symptoms of the injury, and fracture location and severity were described and examined for th
